BACKGROUND: There are various conventional and emerging surgical modalities for management of fistula in ano. But every modality has its own pros and cons. The aim of our study was to compare various surgical modalities in term of its recurrence rate, cost and their complication rates Materials and methods: A prospective observational study was conducted with 50 patients from July 2016 to June 2018. Prospective data was collected on proforma basis regarding clinical presentation, imaging findings, various surgical modalities, complications and reoperation rate. RESULTS: The mean age of the patients was 40.04 ± 10.77 years. The most common type of surgery was Fistulectomy (28%) and Fistula plug (28%) followed by Fistulotomy (20%), Seton Application (16%) and Video Assisted Anal Fistula Treatment (VAAFT) (8%).8 (16%) patients required reoperation. There was no significant association of reoperation and type of surgery in patients (p>0.05). The mean values of post-operative recovery (VAS Score) at different time intervals (post-op, post-op 3 months and post-op 6 months) was comparable and statistically not significant as per ANOVA test (p>0.05). There was no significant association of post-operative recovery (VAS Score) and type of surgery in patients. There was no significant association of post-operative complications after 3 months and 6 months and type of surgery in patients (p>0.05). CONCLUSION: All the surgical modalities used in our study revealed comparable success rate and no significant differences in complication rates. A large randomised controlled trial comparing various surgical modalities is required in future to decide the best surgical method to treat fistula in ano.

Background: After abscess formation, an anal fistula is a common consequence, with crypto-glandular infection being the most commonly accepted causative cause. The goal of this study was to see how well closure of the inter-sphincteric fistula tract affects the outcome of trans-sphincteric fistula surgery. Methods: Patients with perianal trans-sphincteric fistulas who underwent ligation of inter-sphincteric fistula tract (LIFT) were studied prospectively in this cohort study. All patients had the identical anesthetic approach, followed by LIFT with two years’ follow-up. Results: length of the fistulous tract had a substantial effect on the result. The presence of piles with fistula among the studied patients has no effect on the outcome. Mean operative time was 36.2 min and post operative pain was mild in most patients and concomitant anal fissure or piles has no effect on final result of LIFT procedure The mean time for wound healing was four weeks. Three patients had hidradenitis suppurativa and no any unusual histopathology results shown in the excised parts. Conclusion: LIFT was an effective and straightforward sphincter-preserving technique in treating simple trans-sphincteric anal fistulas with excellent long-term control of trans-sphincteric fistula. More similar studies with a large number of patients are still needed to assess this surgical technique for trans-sphincteric fistula. Highlights

The medical literature has no study evaluating the effectiveness of different materials used as setons in the treatment of perianal fistulas; therefore, there is no evidence of availability of a more effective material than others for this purpose. Objective To evaluate the inflammatory response induced by different materials used as seton in perianal fistulas in rats. Method Thirty Wistar rats, which were initially submitted for the construction of a perianal fistula by passing transfixing steel wire into the anal canal, were used. The rats were kept for 30 days; after this period, and with confirmation of the formation of the perianal fistula, the setons were introduced (10 rats – cotton thread #0; 10 rats – rubber; and 10 rats – silastic); after 30 days the animals were euthanized, and then the area of the fistula repaired by the seton was resected, and the material retrieved was submitted to histological analysis. The results were analyzed statistically. Results The mean degree of inflammatory process observed by histological analysis after 30 days was 2.3 for the cotton group; 1 for the rubber group; and 1.2 for the silastic group. Conclusion A greater inflammatory response was observed in the group treated with a cotton seton. In the remaining groups, a lower inflammatory response, with equal intensity for rubber and silastic-treated rats, was noted.
