Abstract: The distances from the round window to the oval window and facial nerve are important parameters in identifying a difficult round window niche. Modification of the electrode may be a better option than drilling off the round window margins for insertion of cochlear implant electrodes.

“…The anatomy of the RWN/RWM region and their surrounding structures respectively anatomical boundaries [23,24] have previously been studied using either dissected cadaveric specimens or histological sections or radiologic imaging of temporal bones and demonstrated significant inter-individual variability [25][26][27][28][29][30]. A recent study conducted by Canzi et al (2019) aimed at radiologically assessing the morphological variations of the RWN, and relating the imaging findings to their endoscopic anatomy.…”

“…The results showed that the size and shape of the feasible target region was highly variable. This could be explained with the high variability of the shape and size of the RW and the spatial relationship between the basal turn and the facial recess ( 38 , 41 , 42 ). The size of the facial recess directly limits the possible orientations of the trajectory and thus the accessibility to the scala tympani.…”

“…Round window has variable anatomy and the shape of the round window could vary from saddle shaped, ovoid to triangular. 17 A number of studies have studied parameters in pre op imaging that might be indicative of ease of electrode insertion through RWM. 16,18,19 Leong et al, mention that imaging did not help them much in predicting the RWM type (although they only had MRI done in their institute).…”
