Review of survival rates 20-years after conservative surgery for papillary thyroid carcinoma

Abstract: Because of the rate of 20% of contralateral recurrence after a 20-year follow-up, we suggest modification of the surgical paradigm for total thyroidectomy as an initial therapy.

“…The mainstay treatment of PTC is surgical resection, but controversy persists over whether total thyroidectomy or thyroid lobectomy is more beneficial [2]. A review of 31 patients with PTC undergoing conservative surgery of lobectomy with isthmusectomy identified a rate of 20% contralateral recurrence after a 20-year follow-up, and therefore supports total thyroidectomy as the initial therapy [65]. Assessing thyroid cancer aggressiveness by identifying the tumor cell metabolic phenotype can also have implications on the cost of care of thyroid cancer patients in the United States.…”

“…HypoPT may be a consequence of total thyroidectomy, 3 , 20 which is the most frequent treatment for DTC, 21 a neoplasm whose incidence has increased considerably in the last years. 22 , 23 Patients with permanent HypoPT require lifelong treatment with Ca salts to control calcemia, phosphatemia, and the Ca × P product.…”

“…The most important adverse aspects in the current surgical approach to treat papillary thyroid carcinoma is the risk of long-term recurrence and the difficulty in managing metastatic disease, especially in those cases initially regarded as low risk [ 19 , 20 ]. In the recent past, great efforts have been made to define new molecular therapies to potentiate the effectiveness of current cytostatic drugs and 15d-PGJ 2 has recently emerged as a potent antineoplastic molecule [ 21 ].…”
