Abstract: Pulmonary hypertension (PH), defined as elevated pulmonary artery pressure, is common in the general population and associated with increased mortality. Accordingly, physicians commonly encounter patients with dyspnea, exercise intolerance, and/or right heart failure who have elevated pulmonary artery systolic pressure (PASP) on echocardiography. Although pulmonary arterial vasodilators may often be considered in this setting, these drugs have been predominantly tested in the subset of PH patients with pulmona… Show more

“…11,12 After it is established, PH is often progressive and associated with high morbidity and mortality. 13 Risk factors for PH, such as left ventricular hypertrophy (LVH) and diastolic dysfunction, are common in patients with CKD and may predispose them to PH. 14 However, there are very limited data available evaluating the prevalence of PH in nondialysis-dependent CKD, factors that contribute to PH in this setting, and more importantly, whether the presence of PH is associated with a greater risk of adverse outcomes in this population.…”

“…The recommendations for the use of Pulmonary vasodilators (Sildenafil and Bosentan) are restricted to WHO group I pulmonary hypertension (i.e., primary PH or idiopathic PH) probably because, this is the most commonly studied group in clinical trials [14].…”

“…The appropriate choice of the agent should be made judiciously as an erroneous administration of vasodilators in pulmonary venous hypertension may lead to worsening of venous pressure and possible pulmonary edema [14].…”
