Проблема послеоперационной тошноты и рвоты (ПОТР) в течение длительного времени привлекает внимание специали стов разных областей хирургии и на сегодняшний день не утра тила своей актуальности, являясь предметом активного обсуж дения в отечественной и зарубежной литературе [1][2][3][4].ПОТР не только одна из самых основных причин не удовлетворенности пациентов лечением [5]. Помимо негатив ной эмоциональной окраски, ПОТР нередко приводит к уве личению сроков реабилитации пациентов. Возрастает время пребывания их в стационаре, продолжительность листа нетру доспособности, а также нагрузка на медицинский персонал и расходы на лечение в целом [6]. Кроме того, ПОТР может стать причиной весьма серьезных осложнений послеопераци онного периода, таких как аспирация, гемодинамические рас стройства у пациентов со скомпрометированной сердечно со судистой системой, расхождение хирургических швов, кровотечение, дегидратация и нарушения электролитного ба ланса [7]. К счастью, частота развития тяжелых случаев рвоты невелика и составляет 1 на 1000 случаев ПОТР (0,1%) [8].В настоящее время установлено, что локализация опера тивного вмешательства, его продолжительность и травматич ность, а также индивидуальные особенности пациентов влияют на частоту развития ПОТР [2,4]. Современная онкохирургия, руководствуясь стремлением к радикализму оперативных вме Послеоперационная тошнота и рвота (ПОТР) -одно из самых частых и неприятных осложнений послеоперационно го периода. Она мучительна для пациентов, влечет за собой увеличение затрат на лечение и может стать причиной весьма серьезных осложнений раннего послеоперационного периода. Частота ПОТР в различных разделах хирургии неодинакова и варьирует в зависимости от характера оперативного вмешательства, приобретая особое значение в он кохирургической практике, где вероятность ее развития, по ряду причин, очень высока. В настоящее время большое количество исследований посвящено разработке схем профилактики ПОТР. Определены основные факторы риска, появляются новые рекомендации по проведению анестезиологического пособия с антиэметогенных позиций, однако описанные методики пока не способны в полной мере избавить пациентов от опасности развития осложнения. Наибо лее часто в схемы профилактики ПОТР включаются препараты группы блокаторов серотониновых 5 НТ 3 рецепторов и метоклопрамида гидрохлорид в сочетании с глюкокортикостероидами и бутирофенонами. Появляется все больше данных об антиэметогенной активности β β 1 адреноблокаторов и агонистов α α 2 адренорецепторов и возможности их ис пользовать в рамках анестезии. В данном обзоре мы попытались суммировать накопленные на сегодняшний день зна ния в области фаторов риска и спсобов профилактики ПОТР, а также обозначить некоторые новые и персперктивные направления в профилактике ПОТР в онкохирургии. Цель исследования-оценить эффективность и безопасность различных комбинаций «истинных» антиэметиков и адъювантов анестезиологического пособия в профилактике послеоперационной тошноты и рвоты у онкологичес кого контингента больных. Материал и методы. Исследовано 165 пациенток, которым в условиях внутривенной общей анестезии выполнялись оперативные вмешательства по поводу злокачественных новообразований женской половой сферы II-III стадии. В зависимости от используемой схемы профилактики послеоперационной тошноты и рвоты, исследуемые были рандомизированы на 4 группы: группа С (n=43)-группа сравнения, в которой огра ничивались только профилактикой ондансетроном 8 мг и дексаметазоном 8 мг на этапе индукции; больные группы Э (n=40) дополнительно получали эсмолол в виде непрерывной инфузии 0,59±0,94 мг/кг/ч; пациенткам группы Д (n=41) на травматичном этапе операции внутривенно болюсно вводили дроперидол 0,038±0,055 мг/кг; в группе К (n=41) на травматичном этапе вмешательства внутривенно болюсно вводили клонидин 0,0013±0,0004 мг/кг. Фик сация инцидентов послеоперационной тошноты и рвоты осуществлялась в течение 24 часов после операции. Ре зультаты. В группе С послеоперационная тошнота и рвота развилась у 15 из 43 больных (34,9%). В группе Э это осложнение возникло у 8 из 40 пациенток (20%), в группе Д у 8 из 41 больных (19,5%). В группах Э и Д не было достоверных отличий по частоте послеоперационной тошноты и рвоты от группы С. В группе К данное осложне ние наблюдали у 6 из 41 пациентки, что составило 14,6% и оказалось достоверно ниже (р=0,03), чем в группе С. Интенсивность послеоперационной тошноты и рвоты была ниже во всех группах исследования в сравнении с груп пой С. Заключение. Включение в схему профилактики послеоперационной тошноты и рвоты клонидина способст вует достоверному снижению частоты развития данного осложнения и его интенсивности. Использование эсмоло ла или дроперидола способствует уменьшению интенсивности послеоперационной тошноты и рвоты. Study objective: to assess the efficacy and safety of different combinations of antiemetic drugs and adjuvants as part of anesthetic care in prevention of postoperative nausea and vomiting in oncological patients.
