Goal. To improve the results of surgical treatment of chronic pancreatitis complicated by vascular pathology of the parapancreatic zone. Materials and methods. On the basis of the "Grodno University Clinic" in the department of X-ray Endovascular Surgery of the period 2010 to April 2020, were performed 16 embolizations of the arteries of the parapancreatic zone due to complications of chronic pancreatitis. Among the patients there were 13 (81.25%) men and 3 (18.75%) women. They had a history of chronic pancreatitis, which was confirmed using instrumental and laboratory research methods. One of the patients (6.25%) had a stationary aneurysm.15 (93.75%) patients had a bleeding clinic, which required urgent surgical interventions. Results. Angioembolization of the parapancreatic arteries was effective in 15 (93.75%) patients, which was confirmed by the results of control angiograms. Conclusions. The use of intraluminal embolization for vascular pathology of the parapancreatic zone in treatment of chronic pancreatitis complications is a minimally invasive and effective method.

Кровотечение из варикозно расширенных вен пищевода (ВРВП) является одной из ведущих причин смерти у пациентов с циррозом печени. Цель. Анализ результатов выполнения лапароскопической эзофагогастральной деваскуляризации с аппаратной транссекцией пищевода. Материал и методы. Первая в Республике Беларусь успешная лапароскопическая эзофагогастральная деваскуляризация с аппаратной транссекцией пищевода выполнена в УЗ «Гродненская областная клиническая больница» в декабре 2014 г. пациентке с циррозом печени, портальной гипертензией и рецидивирующими кровотечениями из варикозно расширенных вен пищевода. Результаты и выводы. Операция способствовала стойкому отсутствию рецидивов кровотечений из ВРВП в послеоперационном периоде. Данное пособие является высокоэффективным и малоинвазивным. Являясь на сегодняшний день резервными методами, шунтирующие и разобщающие (нешунтирующие) хирургические операции помогают добиться стабильной профилактики рецидивов кровотечений из варикозно расширенных вен пищевода. Хирургические операции азигопортального разобщения более просты в исполнении в сравнении с шунтирующими операциями, ассоциируются с меньшим риском развития энцефалопатии в послеоперационном периоде. Цель исследования-анализ результатов выполнения лапароскопической эзофагогастральной деваскуляризации со степлерной транссекцией пищевода с последующей конверсией в тораколапаротомную резекцию дистальной трети пищевода и проксимальную резекцию желудка. Материал и методы. Лапароскопическая эзофагогастральная деваскуляризация со степлерной транссекцией пищевода с последующей конверсией в тораколапаротомную резекцию дистальной трети пищевода и проксимальную резекцию желудка по поводу обширного дефекта в области аппаратного прошивания в зоне эзофаго-эзофагоанастомоза выполнена в УЗ «Гродненская университетская клиника» в феврале 2019 г. пациенту с циррозом печени, портальной гипертензией и рецидивирующими кровотечениями из варикозно расширенных вен пищевода. Результаты. Пациент выписан на амбулаторное лечение, рецидивов кровотечений из варикозно расширенных вен пищевода за период наблюдения, составляющий 12 месяцев, не было. Лапароскопическая эзофагогастральная деваскуляризация со степлерной транссекцией пищевода, являясь эффективной операцией для лечения и профилактики кровотечений из варикозно расширенных вен пищевода, в ряде случаев сопряжена с осложнениями, связанными с некорректным срабатыванием аппарата циркулярного механического шва. Выводы. Своевременная интраоперационная диагностика данных ситуаций с помощью тщательной ревизии зоны эзофаго-эзофагоанастомоза, проведение тестов на его герметичность, знание оперирующим хирургом техники корригирующих осложнения оперативных вмешательств и готовность к их выполнению позволяют предотвратить фатальные последствия для пациентов. Liver cirrhosis is the result of various chronic liver diseases. Portal hypertension is a serious complication of cirrhosis. Its consequences, in turn, along with other complications are gastroesophageal varicose bleeding, which cause high mortality rates. The article contains analysis of the results of laparoscopic esophagogastric devascularization without esophageal transsection and splenectomy in a patient with liver cirrhosis portal hypertension and recurrent bleeding from varicose veins of the esophagus. First successful surgery according to this method was introduced in the Grodno Municipal Clinical Hospital No. 4 in November 2011. Immediate and long-term results show a rather high efficiency of using this operation in the treatment and prevention of bleeding from varicose veins of the esophagus with cirrhosis. It is advisable to conduct further studies of the effectiveness of using this operation, despite the encouraging results of the use of this modification of laparoscopic esophagogastric devascularization.
