Hypogonadism in male patients defined as testosterone level decrease in serum associated with specific symptoms and/or signs (see the detailed description below) can be observed in case of abnormal changes in testes and/or pituitary such as Klinefelter syndrome, Kallmann syndrome and also in male patients with idiopathic, metabolic or iatrogenic disorders resulting in androgen deficiency. The Guidelines does not review all disorders conditioning development of testosterone deficiency (hypogonadism), but focusing on the options of the clinical conditions of hypogonadism generally observed in male patients. Гипогонадизм у мужчин, определяемый как снижение уровня тестостерона в сыворотке крови в сочетании с характерными симптомами и/или признаками (подробно описаны далее), часто встречается при сахарном диабете (СД). Эти рекомендации не охватывают всего диапазона патологий, обусловливающих развитие дефицита тестостерона (гипогонадизма), а фокусиру-ются на его клинических вариантах, характерных для мужчин с диабетом. В рекомендациях представлены данные о распро-страненности гипогонадизма при СД, его этиологии. В разделе «диагностика» подробно представлены особенности анамнеза пациентов с гипогонадизмом при СД, необходимые методы физикального и лабораторного обследования. Отдельно рассмо-трены факторы риска и клинические последствия гипогонадизма. В разделе «выбор методов лечения» представлены возмож-ные варианты лечения таких пациентов с использованием различных средств андрогенной терапии с учетом потребностей мужчины, сохранения его репродуктивной функции и факторов риска. Особое внимание уделено показаниям, противопо-казаниям и факторам риска андрогенной терапии у мужчин с диабетом, особенно в пожилом возрасте. С учетом этого раз-работаны принципы мониторинга проводимого лечения. На основании большого числа исследований продемонстрированы благоприятные эффекты андрогенной заместительной терапии у мужчин с гипогонадизмом и СД. Hypogonadism in men, defined as a reduction in serum testosterone in combination with characteristic symptoms and/or signs (described in detail later), is common in diabetes mellitus (DM). These recommendations do not cover the whole range of pathologies that cause the development of testosterone deficiency (hypogonadism), but focus on its clinical variants and characteristic for men with diabetes. The recommendations provide data on the prevalence of hypogonadism in diabetes, its etiology. In the section "diagnostics" the features of anamnesis of patients with hypogonadism with diabetes, the necessary methods of physical and laboratory examination are presented in detail. The risk factors and clinical consequences of hypogonadism are separately examined. In the section "choice of treatment methods", there are possible treatment options for such patients using various androgenic therapies, taking into account the needs of the man, maintaining his reproductive function and risk factors. Particular attention is paid to indications, contraindications and risk factors for androgen therapy in men with diabetes, especially in old age. With this in mind, principles for monitoring the treatment are developed. Based on a large number of studies, favorable effects of androgen replacement therapy in men with hypogonadism and diabetes have been demonstrated. Учитывая частое развитие висцерального ожирения у мужчин, актуально исследование его влияния на сперматогенез. Цель. Оценка структуры патозооспермии у молодых мужчин с постпубертатным висцеральным ожирением и неотягощенным андрологическим анамнезом. Методы. В сплошное одномоментное исследование были включены 80 мужчин с постпубертатным алиментарным висце-ральным ожирением в возрасте до 30 лет. Проводилась оценка длины окружности талии, уровня гонадотропинов и общего тестостерона, антиоксидантной активности эякулята, его электронно-микроскопическое исследование. Статистически зна-чимыми считали различия между группами при p<0,05. Результаты. У 24% мужчин патозооспермии выявлено не было. Основным патогенетическим феноменом, выявленным у па-циентов c патозооспермией, являлась астенозооспермия -100%. Также отмечалась высокая распространенность тератозо-оспермии -36%. Распространенность олигозооспермии составила 11% случаев. Повышение содержания сперматозоидов с недостаточно конденсированным хроматином и снижение антиоксидантной активности эякулята были характерны для всех типов патозооспермии у мужчин с ожирением. Снижение уровня тестостерона было характерно для олигозооспермии. При этом были установлены статистически значимые отрицательные корреляции между длиной окружности талии и уровнем тестостерона, количественными показателями эякулята, его антиоксидантной активностью, а также положительная корре-ляция между длиной окружности талии и содержанием сперматозоидов с недостаточно конденсированным хроматином. Заключение. У молодых мужчин с постпубертатным висцеральным ожирением и неотягощенным андрологическим анамне-зом распространенность патозооспермии в нашем исследовании составила 76%. Наиболее редко встречалась олигозооспер-мия, для которой было характерно наличие синдрома нормогонадотропного гипогонадизма, который развивался у пациентов с наиболее выраженным висцеральным ожирением. Повышение содержания сперматозоидов с недостаточно конденсиро-ванным хроматином и снижение антиоксидантной активности эякулята характерно для всех типов патозооспермии у мужчин с ожирением, что обусловлено оксидативным стрессом. 11 1'2015 О б з о р ы л и т е р а т у р ы А ндрогенная заместительная терапия проводится в случаях выявления у мужчин гипогонадизма -клинико-лабораторного синдрома, обуслов-ленного снижением секреции тестостерона яичками. Применение этой терапии во многих случаях является необходимым, так как развитие гипогонадизма у мужчин приводит к половой дисфункции, что значительно сни-жает качество жизни. Эффективность препаратов тесто-стерона в отношении гипогонадизма и обусловленных им нарушений мужской половой функции доказана в боль-шом числе исследований и сомнений не вызывает [1]. При этом существуют данные о высокой эффективно-сти этой терапии и в отношении устранения основных компонентов метаболического синдрома, в том числе ожирения и инсулинорезистентности, лежащей в основе развития сахарного диабета (СД) [2][3][4][5].Попытки использовать заместительную андроген-ную терапию препаратами тестостерона у мужчин с СД и метаболическим синдромом обусловлены высокой распространенностью гипогонадизма при этих заболева-ниях [6-9]. В исследовании Mirzaei M.R. и соавт. (2012), включившем 247 мужчин с СД 2 типа (СД2), распростра-ненность гипогонадизма оценивалась по уровню биодо-ступного тестостерона и составила 61,6% [7]. В другом, более крупном исследовании, включившем 1089 мужчин с СД2, низкий уровень тестостерона был выявлен у 36,5% пациентов [8]. В исследовании Dhindsa S. и соавт. (2010) у 1849 мужчин (1451 без СД и 398 с СД2) был определен уровень свободного тестостерона. Распространенность случаев выявления уровня свободного тестостерона в субнормальных (пограничных) значениях у худых муж-чин, с избыточным весом, и у мужчин с ожирением без СД составила 26% (n=275), 29% (n=687) и 40% (n=489) соответственно (p<0,001), а в аналогичных группах муж-чин, но с СД -4% (n=36), 44% (n=135) и 50% (n=227) соответственно (р<0,05). Средняя концентрация свобод-ного тестостерона у мужчин с диабетом была значительно ниже, чем у мужчин без диабета [9].При этом мета-анализ 26 исследований (проведенных в период с 1990 по 2007 гг.) продемонстрировал наличие связи между низким уровнем тестостерона у мужчин и развитием СД2. In the study was used testosterone undecanoate therapy which causes reduction of obesity, a decrease in the severity of the other components of the metabolic syndrome, improving glycemic profile without significant side effects. Nevertheless, this therapy requires monitoring and management for risk factors.
