The analysis of the vaccine preventive status of infectious diseases in adults in Russia and various countries of the world is presented in the article. The problems and directions of improving the immunization of the adult population in Russia are identified.
Aim. The study of the prognosis of epidemiological and economic effects of pneumococcal vaccination in laboring males with various chronic diseases. Materials and methods. Within the predictive (Markov) model, based on the published data, assessment of 5-year horizon pneumococcal vaccination of laboring males perceived effectiveness in reducing mortality, avoiding morbidity and economic loss in the country had been processed. According to the official statistics in the Russian Federation 21 575 887 laboring males are within the high risk group for contracting pneumonia. Data source for the cost of the healthcare for disease had been the compulsory health insurance system 2016 state tariffs, for the vaccination cost results of the PCV13 procurement bidding had been used. Results. Цель исследования-оценить возможную эпидемиологическую и экономическую эффективность вакцинопрофилактики пневмококковой инфекции у мужчин с различными хроническими заболеваниями. Материал и методы. На основе данных литературы построена прогностическая (марковская) модель. Оценена предполагаемая эффективность вакцинации против пневмококковой инфекции у мужчин трудоспособного возраста в снижении смертности, предотвращенной заболеваемости и экономического ущерба в 5-летней перспективе. По данным официальной статистической отчетности, группа высокого риска заболевания пневмонией в РФ включает 21 575 887 мужчин трудоспособного возраста. Источниками данных о стоимости заболевания служили государственные тарифы в системе ОМС за 2016 г., а о стоимости вакцинопрофилактики-результаты конкурсных торгов по закупкам 13-валентной пневмококковой вакцины (ПКВ13). Результаты. Результаты экстраполяции данных отечественных и зарубежных исследований на когорту лиц с хроническими заболеваниями органов дыхания, болезнями системы кровообращения или сахарным диабетом показали значимое снижение риска развития осложнений основного заболевания (OP=0,58, p<0,05), числа госпитализаций (OP=0,02; p<0,05) и ожидаемой смертности на фоне вакцинации. Затраты на вакцинацию оцениваемой группы пациентов составили 25 869,5 млн руб. Применение ПКВ13 статистически значимо снижает количество обострений и соответственно госпитализаций, что позволяет экономить в каждый последующий год после вакцинации около 14 359,9 млн руб. Таким образом, уже в течение 2 лет суммарная экономия бюджетных средств может составить 2850,30 млн руб. В 5-летней перспективе будет сохранено как минимум 61 702 жизни пациентов при условии однократной вакцинации ПКВ13. Заключение. Результаты данного исследования свидетельствуют о высокой эпидемиологической и клинической эффективности вакцинации против пневмококковой инфекции мужчин трудоспособного возраста, страдающих хроническими заболеваниями. Это вмешательство снижает заболеваемость, количество обострений и госпитализаций, а также смертность в группе вакцинированных и является экономически выгодной инвестицией в здравоохранение.
