BackgroundVestibular vertigo is associated with substantially reduced quality of life. Betahistine is effective in improving vertigo-associated symptoms, with longer treatment periods leading to greater improvements; however, it is not known whether these effects persist after treatment cessation.MethodsVIRTUOSO was a prospective, multinational, non-comparative, post-marketing observational programme investigating the effectiveness of betahistine (48 mg/day) and the course of vertigo after the discontinuation of treatment. Patients with vestibular vertigo who were prescribed 48 mg/day betahistine were enrolled in Russia and Ukraine. Treatment duration was up to 2 months, and patients were followed up for 2 months after discontinuation of betahistine. Efficacy endpoints included clinical response (assessed by change in vertigo severity), monthly attack frequency, and physician and patient grading of overall clinical response and improvement of vertigo-associated symptoms.ResultsOverall, 309 patients were enrolled and 305 completed the study. Clinical response was rated as good, very good or excellent in 74.1% of patients at end of treatment, with vertigo severity significantly decreased from baseline (p < 0.001). Monthly vertigo attack frequency decreased significantly during the 2 months of treatment (p < 0.001 from baseline) and further decreased during the 2-month follow-up (p < 0.001 from end of treatment). Overall, clinical response was graded as good or excellent by 94.4% of physicians and 95.4% of patients. Clinical improvement was considered either good or excellent by 82.6–90.5% of physicians and patients for nausea, vomiting and faintness. Only one adverse event was reported, with no serious adverse events.ConclusionOur findings suggest that betahistine (48 mg/day) therapy is effective in treating vertigo in routine clinical settings. The observed effects persisted for 2 months after treatment cessation, suggesting that betahistine may facilitate lasting vestibular compensation.

Пациен-ты, как правило, уверены, что причиной всех этих наруше-ний служит головокружение, поэтому информацию о пред-шествующих стрессовых и психотравмирующих ситуациях врач может получить только при активном расспросе. Не-редко пациенты отрицают наличие психотравмирующей ситуации или не считают имеющуюся ситуацию таковой, что может затруднять диагностику. Часто у пациентов с фобическим постуральным головокружением с детства отмечаются плохая переносимость транспорта, качелей, каруселей, что рядом авторов расценивается как минимальная дис-функция вестибулярной системы, которая может вызы-вать ощущение неустойчивости [1,[3][4][5]7]. Клинические проявления заболевания возникают во взрослом состоя-нии. Наибольшая частота фобического постурального го-ловокружения наблюдается в 20-50 лет. По данным про-веденного в Германии исследования, в этой возрастной группе фобическое постуральное головокружение было самой частой причиной обращения пациентов с жалобами на головокружение в специализированные отоневрологи-ческие центры [8].Патогенез фобического постурального головокруже-ния до конца не изучен. Ряд авторов объясняет возникаю-щие у пациента ощущения головокружения и неустойчиво-сти при психогенных расстройствах нарушением воспри-ятия окружающего мира как неподвижного. Подобного ро-да нарушение может быть обусловлено несоответствием афферентных вестибулярных и зрительных импульсов, по-лучаемых из внешней среды от органов чувств во время дви-жения, и сформированными в процессе жизнедеятельности ожидаемыми сигналами, которые должны возникать в ре-зультате такого движения. Тело человека постоянно совер-шает колебательные движения, необходимые для поддержа-ния равновесия. Эти импульсы в норме не ощущаются как движение. Ощущение головокружения при психогенных расстройствах в виде пошатывания при ходьбе может поя-виться в результате несоответствия афферентных импуль-сов, возникающих при реальном движении, и импульсов, ожидаемых системой поддержания равновесия. Вестибулярные нарушения -одна из наиболее частых причин головокружения и нарушения устойчивости. Реабилитация пациентов с вестибулярными нарушениями основывается на выявлении основного заболевания и его эффективном лече-нии. Наиболее частыми причинами вестибулярных нарушений являются доброкачественное пароксизмальное позицион-ное головокружение, мигрень-ассоциированное головокружение, болезнь Меньера, вестибулярный нейронит. Однако эти заболевания вестибулярной системы часто не диагностируются, что может быть обусловлено недостаточной осведомлен-ностью врачей о возможных причинах головокружения, методах диагностики вестибулярных заболеваний, а также несо-блюдением протокола исследования вестибулярной системы. В настоящее время разработаны современные эффектив-ные методы лечения и реабилитации пациентов с различными вестибулярными заболеваниями. Необходим комплексный подход к лечению и реабилитации пациентов с вестибулярными нарушениями, включающий сочетание методов физиче-ской реабилитации с медикаментозными препаратами, ускоряющими процесс вестибулярной компенсации. Высокую результативность демонстрирует метод реабилитации на стабилографической платформе с биологической обратной свя-зью.
