The guide will allow doctors of surgical profile to get information about the meaning of eponymous terms that they face in practice and in the study of special medical literature. The book contains 2008 eponymous symptoms, symptomocomplexes, syndromes and diseases related to the main sections of surgery. The book consists of two parts, the first part of which refers to the semiotics of surgical diseases and presents their symptoms and signs. The second part of the manual includes nosological units, which are divided into 8 chapters – sections of clinical surgery – and systematized in alphabetical order. Their explanation, the historical reference, data on epidemiology of diseases, their etiology and pathogenesis and a clinical picture are given. From the standpoint of modern evidence-based medicine provides information on the diagnosis, treatment and prevention of major surgical diseases. At the end of the book is a subject index, referring to which the reader can easily find the term of interest. The guide is addressed to a wide range of doctors of surgical profile. It will be useful to researchers, teachers of higher medical school.
The article analyzes the results of treatment of 139 patients with chronic lower limb ischemia stage II in three parallel groups. The first group (n=57) received standard conservative therapy in combination with ozone therapy; when this group was divided into two subgroups: Subgroup 1a patients (n=28) was performed intravenous ozonized saline (ODF), 1b subgroups (n=29)-greater autogemoozonoterapiyu (BAP). Patients of the second group (n=62) underwent a comprehensive treatment comprising medical ozone in addition to gravitational therapy (ADT). In this group as two subgroups were distinguished: 2a subgroup (n=31)-patients receiving standard medical therapy in combination with HT and ODF, 2b subgroup (n=31)-in combination with BAP and HT. The third group, control (n=20) included patients receiving only standard medical therapy. The highest efficiency is noted in subgroup 2a. There was a statistically significant increase in pain-free walking distance by 200%, ankle-brachial index-47%.In this same subgroup seen most pronounced positive dynamics of indicators of the quality of life of patients, which was confirmed by the most efficient correction scales questionnaire SF-36: physical component index increased by 32%, index of mental health-29%. Combined application of ozonated saline and gravitational therapy in patients with chronic lower extremity ischemia II step facilitates a significant increase in pain-free walking distance and ankle-brachial index and effectively improves the quality of life of patients.
Актуальность. Первичный гиперпаратиреоз (ПГПТ) в развитых странах рассматривается как третья эндокринная эпидемия, наряду с сахарным диабетом и заболеваниями щитовидной железы. Материал и методы. Прооперировано 36 пациентов. Число женщин превышало и статистически значимо отличалось от числа мужчин и составило 81% (29 человек) (χ 2 = 26,89, р = 0,0000). Возраст больных колебался от 33 до 82 лет. У 17 пациентов была выявлена смешанная форма ПГПТ, у 10 -почечная и у 9 -костная форма. Результаты. Легкая степень гиперкальциемии наблюдалась у 26 (72%) пациентов, средняя -у 4 (11%), тяжелая -у 6 (17%) пациентов. Значение медианы ионизированного кальция до операции у пациентов составляло 1,6 ммоль/л. Медиана паратгормона (ПТГ) до операции составляла 169,95 пг/мл. Тяжесть состояния пациентов с ПГПТ подтверждалась наличием гиперкальциемии, что проявлялось умеренной статистически значимой корреляцией показателя ионизированного кальция с уровнем ПТГ (r = 0,38, р = 0,02) и размерами аденомы (r = 0,48, р = 0,03). Для топической диагностики аденом мы выполняли не менее двух инструментальных методов исследования. Данные об аденоме околощитовижных желез (ОЩЖ) подтверждались при УЗИ и сцинтиграфии не более чем в 79 и 72%, при МРТ -в 88,2% случаев. Всем пациентам удаляли одну-две аденомы ОЩЖ, всего было удалено 65 аденом. В 88% случаев (28 пациентов) отмечали в качестве сопутствующей патологию щитовидной железы, что требовало выполнения симультанных операций на этом органе. Медиана ионизированного кальция на следующий день после операции статистически значимо снижалась до 1,14 ммоль/л (уменьшение на 0,46 ммоль/л -29%, р = 0,00001). Показатель ПТГ после операции значимо уменьшился до 70,45 пг/мл (снижение на 99,5 пг/мл -58%, р = 0,00001). Выводы. ПГПТ чаще встречается у женщин старше 50 лет. Сочетанные поражения щитовидной железы и ОЩЖ наблюдали в 88% случаев, что требует выполнения симультанных операций. В 81% случаев наблюдали более одной аденомы ОЩЖ. Для топической диагностики аденом ОЩЖ необходимо выполнение не менее двух инструментальных исследований. При оперативном лечении необходимо критично относиться к данным дооперационного исследования, иметь в виду возможность множественного поражения ОЩЖ, использовать правило симметричного расположения ОЩЖ. The age of the patients ranged from 33 up to 82. 17 patients had a mixed form of PHPT, 10 patients had a kidney form and 9 patients had an osseous form of the disease. Results. Pre-operative ionized calcium median significance was 1.6 mmol/l. Mild degree of hypercalcemia was observed in 26 patients (72%), moderate degree was observed in 4 patients (11%) and 6 patients (17%) had a severe degree of hypercalcemia....
