The article presents data on the prevalence, classification, diagnosis, and treatment of burning mouth syndrome (BMS). Given the variety of etiological factors, special attention is paid to the differential diagnosis of idiopathic (primary) and secondary BMS. The article also examines topical and systemic causes of burning tongue, which should be excluded when examining patients with complaints specifically attributed to BMS. The information presented in the article allows physicians of various specialties to make an individual plan of diagnostic measures for each patient, taking into account the comorbid background and anamnestic data. The main cause of treatment difficulties in BMS is a complex, not fully investigated etiopathogenesis. Due to the fact that the multifactorial nature of this disorder is most likely to involve topical, systemic and psychogenic causes, a multidisciplinary method involving physicians of various specialties (neurologist, dentist, therapist, psychiatrist) is optimal in the patient management with BMS. Spreading of actual information about vestibular migraine is important due to its underdiagnosis. Knowledge of clinical features is helpful for diagnosis and differential diagnosis. Treatment approaches according to guidelines and last research should be used.
Распространенность орофациальных болей у взрос-лых составляет до 10%, мужчины страдают вдвое чаще женщин, что отличается от общих гендерных тенденций распространенности болевых синдромов, чаще встречаю-щихся у женщин. Эпидемиологический анализ особенно-стей лицевых болей (ЛБ) у 7124 респондентов показал, что у 7% отмечались боли в орофациальной области в течение последней недели, а при наблюдении на протяжении 12 мес этот показатель возрастал до 16%. Другим эпидемио-логическим парадоксом орофациальных болевых синдро-мов является снижение их распространенности с возрас-том, в то время как распространенность болевых синдро-мов другой локализации у пожилых нарастает [1,2].Орофациальные болевые синдромы сложно диагно-стируются ввиду многообразия анатомического строения отдельных образований и структур области лица и рта, морфологических и функциональных особенностей пе-риферического и центрального отделов нервной системы, обеспечивающих их афферентную и эфферентную иннер-вацию, влияния психологических факторов на клиниче-скую картину ЛБ. The current outbreak of coronavirus SARS-CoV‑2 (COVID-19) has raised great concern worldwide, but its impact on nervous system still needs more investigation. Thirty per cent of symptomatic patients with COVID‑19 will have symptoms that last longer than the typical two weeks, 10 % have symptoms longer than 3 months and this is called ‘long-COVID’. These symptoms affect not only people with severe disease, but also those with milder cases. Many long-haulers experience the same symptoms they had during their initial fight with COVID‑19, such as fatigue, cognitive impairment (or brain fog), difficulty breathing, headache, depression, insomnia and loss of the sense of taste and\or smell. Treatment of those complications with citicoline, ethylmethylhydroxypyridine succinate and vitamin B improves these symptoms in patients but most of them also need cognitive therapy for dehypochondrisation.
Резюме Полинейропатии у больных сахарным диабетом проявляются поражением периферической сенсомоторной и вегетативной нервной системы. Различная степень тяжести поражения сенсомоторных и вегетативных волокон при типичных и атипичных формах диабетических полинейропатий (ДПН) требует дифференцированного подхода к терапии, но не отказа от ее проведения. В междисциплинарном консенсусе ведущих специалистов по ДПН из разных регионов РФ обсуждаются современные методы диагностики и оценки степени тяжести ДПН, определяющие алгоритм лечения больных.
