The aim of the study was to analyze the relationship between monocyte subpopulations and phenotype/ functions of monocyte-derived dendritic cells (DCs), as well as DC sensitivity to the tolerogenic effect of dexamethasone.Materials and methods. The study included 15 healthy donors. DCs were generated by cultivating enriched fractions of CD14+ monocytes with or without CD16+cell depletion (CD16-Mo-DCs or CD16+Mo-DCs, respectively) in the presence of interferon alpha (IFNα) and granulocyte-macrophage colony-stimulating factor (GM-CSF). Monocyte subpopulations were obtained by immunomagnetic negative selection.Results. CD16+Mo-DCs were characterized by higher percentage of mature (CD83+CD14-) and lower number of semi-mature (CD14+CD83+) cells, but were similar to CD16-Mo-DCs by HLA-DR and CD86 expression, involved in the presentation of antigens and activation of naive T-cells. and also to co-inhibitory/ tolerogenic molecules B7-H1 and TLR-2. CD16+Mo-DCs displayed higher allostimulatory activity, which was positively correlated with CD86 expression (rS = 0.69; p = 0.027) and negatively – with TLR-2 expression (rS = -0.72; p = 0.1). Allostimulatory activity of CD16-Mo-DCs was positively correlated with the number of mature CD14-CD83+DCs and semi-mature CD14+CD83+DCs. Addition of dexamethasone (10-6 M) into CD16-Mo-DCs and CD16+Mo-DCs cultures led to the delay of DC maturation, the decrease of CD86 and the increase of TLR-2 expression, as well as the increase of cells with co-inhibitory CD86- B7-H1+ phenotype that was positively correlated with the reduction of DC allostimulatory activity. The decrease of CD86+/TLR-2+ index in CD16+Mo-DC population was due to the reduction of CD86+DCs and in CD16-Mo-DC population – to the increase of TLR-2+cells. Dexamethasone possessed higher inhibitory effect on DC maturation in the CD16+Mo-DC cultures.Conclusion. CD14+ monocytes, both contained and depleted by CD16+ cells, can differentiate into DCs when cultured with IFNα. The presence of CD16+ cells in whole blood monocyte pool is associated with generation of DCs showed a more mature phenotype and higher allostimulatory activity. Both CD16- and CD16+ monocyte-derived DCs are sensitive to suppressive effect of dexamethasone. However, dexamethasone tolerogenic effect involves different mechanisms in CD16-Mo-DCs and CD16+Mo-DCs.

Хроническое рецидивирующее течение инфекции, обусловленной вирусами просто-го герпеса (ВПГ) 1 и 2 типа, представляет серьезную медико-социальную проблему. Учитывая важ-ную роль иммунной системы в контроле за вирусом, перспективы лечения рецидивирующих форм ВПГ-инфекции связывают с активацией антигенспецифического иммунного ответа, в частности с использованием вакцин на основе дендритных клеток (ДК). В настоящей работе представлены ре-зультаты открытого пилотного исследования эффективности и безопасности дендритноклеточных вакцин у пациентов с рецидивирующей ВПГ-инфекцией. В исследование были включены 29 человек, включая 14 пациентов с лабиальным и 15 с генитальным герпесом. ДК генерировали в присутствии GM-CSF и IFNα, нагружали рекомбинантными вирусными белками (HSV1gD или HSV-2gD) и про-водили 2 курса вакцинаций в течение 9 мес. Иммунотерапия ДК не вызывала серьезных нежелатель-ных эффектов и сопровождалась более чем двукратным снижением частоты рецидивов и увеличе-нием длительности ремиссии на фоне 9 мес. лечения. В период последующего 6-месячного периода наблюдалось дальнейшее снижение частоты рецидивов и трехкратное возрастание длительности ре-миссии. Клинический эффект на фоне лечения и в период 6 мес. наблюдения отмечался как в группе с лабиальным, так и генитальным герпесом, регистрировался у большинства пациентов, ассоции-ровался с индукцией антигенспецифического пролиферативного ответа и нормализацией исходно сниженной митогенной реактивности мононуклеарных клеток в ответ на стимуляцию КонА. Анализ отдаленных результатов лечения на основе анкетирования пациентов с давностью наблюдения более 24 мес. показал, что положительный эффект иммунотерапии в виде снижения частоты рецидивов че-рез 48 мес. (медиана наблюдения) сохранялся у 77,8% опрошенных. Антигенспецифический проли-феративный ответ в эти сроки сохранялся у 66,7%. Полученные результаты позволяют рассматривать ДК-вакцины в качестве нового перспективного подхода к лечению рецидивирующих форм лабиаль-ного и генитального герпеса. Chronic viral hepatitis C is the most common cause of liver damage and the global problem worldwide since is characterized by a high prevalence, high chronization rates, and significantly increases the risk of liver cirrhosis and hepatocellular carcinoma. Many studies have shown that antigen-specific СD4 + and CD8 + T-cells play a key role in pathogenesis and outcome of the infection. While the strong sustained antigenspecific multi-epitopic T-cell response predicts successful viral elimination, a deficiency of adaptive immune response is associated with virus persistence. This review presents data about pathogenetic significance of T-cell response in viral elimination, viral persistence and hepatitis development. Possible mechanisms of T-cell response failure in chronic infection are discussed as well.
Резюме. Инфекция, вызванная вирусом гепатита С (HCV), является серьезной проблемой здравоохранения, поскольку в большинстве случаев (до 85%) приобретает хроническое течение и повышает риск развития цирроза печени, гепатоцеллюлярной карциномы и выраженных внепеченочных осложнений. Причины хронизации инфекции во многом связаны с недостаточностью противовирусного иммунного ответа. Эффективная элиминация вируса требует ранней активации врожденного иммунитета, индукции сильного HCV-специфического мультиэпитопного Т-клеточного ответа и формирования продолжительной иммунологической памяти. Дендритные клетки (DC), которые представляют гетерогенную популяцию антигенпрезентирующих клеток, являются продуцентами интерферонов первого типа, способны активировать натуральные киллерные клетки и индуцировать адаптивный иммунный ответ, выполняя, таким образом, важную роль в противовирусной защите. При этом нарушение функций DC при HCV-инфекции рассматривается в качестве одного из механизмов, позволяющих вирусу избежать иммунного надзора. Настоящий обзор включает современные данные, характеризующие роль DC в иммунном ответе при HCV-инфекции и освещает ряд ключевых вопросов, касающихся изменений фенотипа и функций различных субпопуляций DC у пациентов с вирусным гепатитом С, механизмов нарушения функциональной активности DC и перспектив лечения хронического гепатита С на основе использования генерированных ex vivo DC.
РЕЗЮМЕ Цель. Оценить результаты лечения больных после эндоваскулярных вмешательств по поводу множественных церебральных аневризм (МнА) в отдаленном послеоперационном периоде с учетом качества жизни пациентов. Материалы и методы. Проанализированы данные 172 наблюдений с МнА, оперированных эндоваскулярно. Исследование качества жизни производилось с использованием общего опросника The Short Form-36 (SF-36), Международной классификации функционирования (МКФ), шкалы Рэнкина. Результаты. В отдаленном послеоперационном периоде субарахноидальное кровоизлияние (САК) отмечено в 1,2% случаев. При оценке по шкале SF-36 у больных, перенесших в анамнезе САК, качество жизни снижалось по субшкале «Социальное функционирование» (p = 0,03), у больных с псевдотуморозным типом течения-по субшкалам «Ролевое функционирование, обусловленное физическим состоянием» (RP) (p = 0,004), «Общее состояние здоровья» (GH) (p = 0,049), «Социальное функционирование» (SF) (p = 0,005), «Психическое здоровье» (MH) (p = 0,009). Число оперативных вмешательств >2 также ассоциировано с качеством жизни пациентов (p < 0,05). При оценке активности по МКФ выраженность нарушений по домену d4501-ходьба на близкие расстояния-зависела от перенесенного САК (p < 0,05). Осложнения эндоваскулярного лечения оказывали влияние на активность пациентов по доменам d4501-ходьба на дальние расстояния (p = 0,03), d640-выполнение работы по дому (p = 0,01). Заключение. Уточнить, каковы активность и участие в общественной жизни пациента, позволяет проведение оценки по МКФ. Дополнительную информацию о субъективном восприятии пациента своего состояния позволяет получить шкала SF-36. Учитывая, что качество жизни в отдаленном послеоперационном периоде не у всех пациентов восстанавливается полностью, возникает необходимость в дальнейшем проведении реабилитационных мероприятий, контрольных церебральных ангиографий, а также в усовершенствовании методов хирургического лечения.
