Toll-like receptors (TLRs) are the most studied among all Pattern Recognition Receptors, the main function of which is to initiate innate immune response by recognizing pathogen-associated molecular patterns of various microorganisms on the skin surface. TLR-mediated recognition plays an important role in linking innate and adaptive immunity that ultimately leads to the production of key cytokines, chemokines and antimicrobial peptides. Today, there is growing interest in research on single nucleotide polymorphisms (SNPs) in TLR genes and its influence on susceptibility to inflammatory disease, including atopic dermatitis. The aim of the research was to study the association of the rs5743708 gene polymorphism in the TLR2 gene, the rs4986791 gene polymorphism in the TLR4 gene and the rs352140 gene polymorphism in the TLR9 gene with the risk of developing severe cases of AD. A total of 100 patients with AD were included in the study (38 male and 62 female). The age range was from 18 to 65 years old. All participants were divided into 2 groups according to the SCORAD index (SCORing Atopic Dermatitis). The control group included 72 volunteers over 18 years old. The results of our study showed a statistically significant difference between the moderate AD group and healthy controls in the rs352140 gene polymorphism in the TLR9 gene (Figure 1). The frequency of the GG genotype of SNP rs352140 in TLR9 was 0.169 in the AD group versus 0.329 in the control group (p < 0.05; OR = 0.42; 95% CI = 0.18-0.97).In conclusion, the results of our study showed that the TLR9 rs352140 gene polymorphism may be linked to an increased risk of atopic dermatitis. Moreover, it was found that the GG genotype of SNP rs352140 in TLR9 can be used as a predictor of the risk of developing moderate AD.
Аутоиммунная надпочечниковая недостаточность (АНН) часто является компонентом аутоиммунных полигландулярных синдромов (АПС). АПС 1 типа обусловлен мутацией гена аутоиммунного регулятора AIRE, а предрасположенность к АПС 2 типа и изолированной АНН определяется вариантами генов системы HLA. Не исключается, что в модулировании риска изолированной АНН и АНН в рамках АПС 2 типа, а также в фенотипической вариабельности АПС 1 типа играют роль варианты генов, кодирующих toll-подобные рецепторы (TLR2 и TLR9) и интерферон-λ (IL28B), которые участвуют в иммунном ответе и ассоциированы с рядом других аутоиммунных заболеваний. ЦЕЛЬ: выявить ассоциации полиморфных маркеров в генах TLR9, IL28B, TLR2 с наличием АНН. МАТЕРИАЛЫ И МЕТОДЫ: включено n=63 пациента с АНН: n=54 – с изолированной АНН и АНН в рамках АПС 2 типа (группа 1а); n=9 – с АПС 1 типа (группа 1b); и n=32 условно здоровых лиц (группа 2). Участникам выполнен анализ полиморфных маркеров в генах IL28B (rs12979860 и rs8099917), TLR9 (rs5743836 и rs352140) и TLR2 (rs5743708) методом полимеразной цепной реакции в реальном времени. РЕЗУЛЬТАТЫ: В группе 1 по сравнению с группой 2 выявлено статистически значимое преобладание генотипа СТ полиморфного маркера rs12979860 гена IL28B (р=0,010), а также аллеля Т полиморфизма rs5743836 гена TLR9 (р=0,032). Напротив, генотип СС полиморфизма rs12979860 гена IL28B (р=0,025) и аллель С полиморфизма rs5743836 гена TLR9 (р=0,032) чаще встречались в группе 2, чем в группе 1. При сравни- тельном анализе распределения частот гаплотипов по двум локусам гена IL28B (rs8099917 и rs12979860) обнаружено, что CCTT встречается значительно чаще в группе 2, по сравнению с группой 1 (р=0,024). При сравнении групп 1а и 2 статистическая значимость сохранялась только в отношении частот генотипа СТ полиморфизма rs12979860 гена IL28B (р=0,024) и аллелей Т и С полиморфизма rs5743836 гена TLR9 (р=0,044). В группе 1b по сравнению с группой 2 наблюдалось преобладание генотипа СТ полиморфного маркера rs12979860 гена IL28B (р=0,032) и гаплотипа CTTT по двум локусам гена IL28B: rs8099917 и rs12979860 (р=0,020). ВЫВОДЫ: пациенты с АНН как изолированной, так и в составе АПС 1 и 2 типов отличаются от условно здоровых лиц более частым носительством генотипа СТ полиморфного маркера rs12979860 гена IL28B. Аллель Т полиморфизма rs5743836 гена TLR9 является прогностическим маркером, повышающим ве- роятность развития АНН, тогда как генотип СС по полиморфному локусу rs12979860 гена IL28B, гаплотип CCTT по двум локусам гена IL28B (rs8099917 и rs12979860) и аллель С полиморфизма rs5743836 гена TLR9 – выполняют протективную роль при данном заболевании.
Neonatal pneumonia is one of the most common causes of infant mortality. Moreover, the diagnosis of this pathology represents a difficult task and requires to seek for additional solutions in clinical and laboratory practice. One of the current and promising ways to diagnose rerlies on search of predictive markers among the innate and adaptive immunity genes involved in disease pathogenesis. We used bioinformatics analysis to select genes playing essential role developing neonatal pneumonia. Next, cord blood samples collected from 234 newborns were examined by real-time polymerase chain reaction for nine polymorphic markers in the TNF, IL10, IL17A, IL17F, IL6, NOS3, and MMP9 genes. The results of statistical analysis showed that heterozygous genotypes in the MMP9 and TNF genes were associated with the risk of developing early neonatal pneumonia, and also the protective role of homozygotes AA in the MMP9 gene and GG in the TNF gene. A search for associations with a risk of intrauterine pneumonia revealed an unfavorable role for heterozygous genotypes in the NOS3 and MMP9 genes. Thus, due to difficulties in diagnosis or in case of developing neonatal pneumonia, it may be recommended to add genetic analysis for assessing polymorphic markers in the MMP9 (rs17576), TNF (rs1800629) and NOS3 (rs1549758) genes along with standard test assays.
