В ходе работы по созданию оптимального диагностического алгоритма для выявления множественной миеломы (ММ) и мониторинга ее терапии нами была проведена оценка эффективности использования цитогенетических исследований при диагностике и терапии ММ. Был проведен анализ 115 случаев ММ по результатам цитогенетического и цитологического исследований. Выявлено, что генетические нарушения имели место в 82,6% случаев ММ, а количество плазмоцитов в миелограмме могло иметь тенденцию к увеличению в зависимости от повышения сформированной цитогенетическим исследованием группы риска. Использование цитогенетического анализа и прогностическая стратификация на его основе признаны важными для создания алгоритма комплексной диагностики ММ. Показана необходимость цитогенетического анализа и анализа фенотипических особенностей клеток костного мозга в комплексной иммунохимической диагностике ММ и ее роль в скрининговых, мониторинговых и прогностических направлениях диагностики парапротеинемий. In the course of work on the creation of an optimal diagnostic algorithm for the detection of multiple myeloma (MM) and monitoring of its therapy, we evaluated the effectiveness of the use of cytogenetic studies in the diagnosis and therapy of MM. MM 115 cases were analyzed based on the results of cytogenetic and cytological studies. It was revealed that genetic disorders occurred in 82.6% of MM cases, and the number of plasmocytes in the myelogram could tend to increase depending on the increase in the risk group formed by cytogenetic research. The use of cytogenetic analysis and prognostic stratification based on it are recognized as important for the creation of an algorithm for the complex diagnosis of MM. The necessity of cytogenetic analysis and phenotypic features of bone marrow cells in the complex immunochemical diagnosis of MM and its role in screening, monitoring and prognostic directions for the diagnosis of paraproteinemia is shown.
