Применение современных методов химиолучевой терапии привело к значительному увеличению продолжительности жизни онкологических больных. При этом все чаще как онкологи, так и врачи терапевтических специальностей сталкиваются с проблемами осложнений от проводимого противоопухолевого лечения. Одним из распространенных побочных эффектов является кардиотоксичность, которая существенно влияет на прогноз в ближайшей и долгосрочной перспективе. К наиболее тяжелым и прогностически неблагоприятным осложнениям относятся патология миокарда, хроническая сердечная недостаточность и дисфункция эндотелия. Проблема заключается и в том, что эти осложнения длительно могут протекать бессимптомно, и при появлении кардиологических жалоб такие пациенты не считают нужным сообщать о проведенном в прошлом противоопухолевом лечении кардиологу. The use of modern methods of chemoradiotherapy has led to a significant increase in the life expectancy of cancer patients. At the same time, oncologists and physicians of therapeutic specialties are increasingly faced with problems of complications from ongoing antitumor treatment. One of the most common side effects is cardiotoxity, which significantly affect the prognosis in the short and long term. The most severe and prognostically unfavorable complications include myocardial dysfunction and chronic heart failure. The problem also lies in the fact that this complication can be asymptomatic for a long time and when cardiac complaints appear, such patients do not consider it necessary to report the antitumor treatment carried out in the past to a cardiologist.
Evidence is presented for a significantly wider spread of the first two stages (mild and moderate) Colorectal Bradyarrhythmia Syndrome (CBS) compared with the third (severe — constipation) stage of this syndrome. Surveyed more than 2,500 medical professionals by the method of chronoenterographia. It was found that the incidence of mild CBS (about 60% of cases) and moderate stage of this syndrome (about 30% of cases) is almost an order of magnitude higher than the incidence of severe stage (about 10% of cases) of CBS. SCB has been shown to increase the risk of obesity by almost 3 times. It is shown that CBS contributes to a decrease in well-being, activity and mood, as well as a decrease in the quality of life. Screening for CBS and normalizing the circadian bowel rhythm with psyllium (mucofalc) in individuals who consider themselves healthy may contribute to early prevention of the risk of the metabolic syndrome.
В статье представлен анализ истории использования физических упражнений в лечении и профилактике заболеваний. Определены задачи, которые сегодня стоят перед физической культурой в контексте сохранения здоровья трудоспособного населения и продления «активного долголетия». Рассмотрены некоторые проблемные вопросы подготовки специалистов и возможные пути их решения. The article presents a historical analysis of the physical exercises using in the treatment and prevention of diseases. The tasks that today face physical culture in the context of maintaining the health of the able bodied population and prolonging “active longevity” are determined. Some problematic issues of training specialists and possible ways to solve them are considered.
Материал посвящен новому междисциплинарному направлению «Кардиоонкология». Пациенты с онкологическими заболеваниями имеют повышенный риск развития сердечно -сосудистых осложнений, связанных с проводимой противоопухолевой терапией. В статье описаны исторические этапы применения противоопухолевого лечения, сопровождающегося возможным эффектом кардиотоксичности, представлено современное состояние проблемы и рассмотрены перспективы дальнейшего развития. Приведены данные российских и зарубежных исследований в этой области. The material is devoted to a new interdisciplinary direction “Cardioncology”. Patients with oncological diseases have an increased risk of developing cardiovascular complications associated with antitumor therapy. The article describes the historical stages of the use of antitumor treatment leading to the development of cardiotoxicity, presents the current state of the problem and considers the prospects for further development. The data of russian and foreign studies in this area are presented.
