Як відомо, адекватна аналгоседація забезпечує антистресовий захист при проведенні інтенсивної терапії, у тому числі й у пацієнтів з тяжкою черепно-мозковою травмою. Тому метою нашого дослідження було вивчення впливу дексмедетомідину як компонента аналгоседації на напруженість стресорних реакцій організму у пацієнтів з тяжкою черепно-мозковою травмою. Проведено проспективне дослідження, що включало 80 пацієнтів з ізольованою тяжкою черепно-мозковою травмою у післяопераційному періоду. Хворим 1-ї групи (n = 40) аналгоседацію здійснювали за допомогою морфіну та натрію оксибутирату, у хворих 2-ї групи (n = 40) додатково застосовували α2-адреноміметик дексмедетомідин. На етапах дослідження (1-ша, 3-тя, 5-та та 7-ма доба) визначали рівень інсуліну, кортизолу, глюкози у сироватці крові, розраховували індекс HOMA. Вже на 3-тю добу вивчені маркери стресу мали вірогідно нижчий рівень у хворих 2-ї групи. Тому отримані результати дозволяють зробити висновок, що при додаванні дексмедетомідину як компонента аналгоседації зниження напруженості стресорних реакцій відбувається раніше.
Риносинуситы (РC) -это одна из форм инфек-ций верхних дыхательных путей и наиболее частых патологий, с которой сталкиваются как специали-сты в амбулаторной практике в виде монопроблемы, так и специалисты стационаров в качестве сопут-ствующего осложнения практически любого забо-левания. По данным американских исследовате-лей, 4,6 % всех обращений к врачу общей практики приходится на случаи риносинуситов [1]. В Европе острая форма риносинусита каждый год фиксирует-ся с частотой 1 : 200 -1 : 300 человек.В последние годы все большее внимание уде-ляется нозокомиальному синуситу как гной-но-септическому заболеванию носоглотки, ча-сто встречающемуся в отделениях интенсивной терапии. Инфекции, приобретенные в палате интенсивной терапии, в большинстве случа-ев относятся к внутрибольничным [2]. Частота инфекций в отделениях интенсивной терапии больниц развитых стран Европы колеблется от 7 до 32 %, увеличиваясь до 48-79 % у больных, на-ходящихся на искусственной вентиляции легких [2][3][4]. Частота развития инфекционных ослож-нений существенно возрастает при увеличении сроков пребывания больного в отделениях ин-тенсивной терапии [3].
