Pooperační fi brilace síní (postoperative atrial fibrillation, POAF) se v časném pooperačním období vyskytuje přibližně u třetiny pacientů po aortokoronárním bypassu (CABG). Patogeneze POAF je multifaktoriální a dosud nebyla důkladně zkoumána. Řada studií se jako na možný základní faktor POAF zaměřila převážně na pooperační zánětlivou odpověď. Je známo, že statiny působí protizánětlivě. Několik studií prokázalo sníženou incidenci POAF po CABG v případech, kdy byly v předoperačním a perioperačním období podávány statiny. Cíl: Provedli jsme metaanalýzu randomizovaných a observačních studií účinnosti podávání statinů v prevenci POAF po CABG. Materiál a metody: Do metaanalýzy byly zařazeny údaje 15 klinických studií s celkem 9 369 pacienty, u nichž byl v posledních deseti letech proveden CABG. Z tohoto počtu jich 5 598 (59,75 %) statiny užívalo a 3 771 pacientů (40,25 %) nebyly statiny podávány. Mezi parametry hodnocené v časném pooperačním období patřily incidence POAF, celková mortalita, celková incidence cévních mozkových příhod a celková incidence infarktu myokardu. Zaznamenávaly se i délka hospitalizace a hodnoty zánětlivých markerů před a po CABG. Výsledky: Léčba statiny snížila incidenci POAF po CABG (OR = 0,48; 95% CI 0,35-0,67; p < 0,001). Podávání statinů zkrátilo i celkovou délku hospitalizace a snížilo hodnoty zánětlivých markerů v séru. Závěr: Výsledky naší metaanalýzy prokázaly nade vši pochybnost protizánětlivý a antiarytmický účinek statinů. Potvrzujeme celkově pozitivní úlohu podávání statinů před CABG jako prevenci vzniku POAF.
Развитие фибрилляции предсердий (ФП) после операций на открытом сердце связано с системным и местным воспалительным процессом. При этом повышается уровень маркеров воспаления. Статины за счет противовоспалительных свойств показали хорошие результаты в профилактике развития ФП после аортокоронарного шунтирования (АКШ). Тем не менее другие исследования не смогли продемонстрировать их антиаритмический эффект. Цель. Оценить роль приема статинов в первичной и вторичной профилактике ФП после АКШ. Материал и методы. Проведен ретроспективный анализ 225 историй болезни пациентов после АКШ, выполненных в 2013 г. Были выделены две группы: в 1-й больные не принимали статины, во 2-йполучали статинотерапию. Ко 2-й группе были отнесены пациенты, которые не менее трех дней до операции и все дни после операции получали статины. Факт развития послеоперационной ФП был определен как эпизод ФП более 5 мин в течение 7 сут после операции. Результаты. В 1-ю группу вошли 93 (41%) пациента, во 2-ю-132 (59%). Частота развития ФП в 1-й группе составила 29%, во 2-й-9%, p=0,0001. Количество лейкоцитов на 4-е сутки в 1-й группе 10,9 (9; 13) против 9,1 (7,6; 10) во 2-й группе, p=0,000002. Было отмечено, что количество лейкоцитов на 1-е сутки также было ниже во 2-й группе, однако без статистической НЕИНВАЗИВНАЯ АРИТМОЛОГИЯ Введение Ф ибрилляция предсердий (ФП)-самое частое нарушение ритма сердца, возникающее после аортокоронарного шунтирования (АКШ). Возникновение ФП ассоциируется с высоким риском развития послеоперационных осложнений: нарушениий гемодинамики, инсульта, инфаркта миокарда. Все эти осложнения приводят к более длительному пребыванию пациента в стационаре и большим экономическим затратам [5-7]. Некоторые исследователи предложили профилактический прием амиодарона и бета-адреноблокаторов для снижения риска развития послеоперационной ФП [8-10]. Стоит учитывать, что

Около 95% случаев развития ФП приходится на первые 6 сут после операции. Возникновение ФП ассоциируется с высоким риском развития инсульта, инфаркта миокарда, с послеоперационной летальностью, а также более длительным сроком госпитализации и, соответственно, большими экономическими затратами. К послеоперационным факторам риска развития ФП относятся: воспаление, обусловленное посткардиотомным синдромом и искусственным кровообращением (ИК), электролитный дисбаланс, дисбаланс жидкости, длительная искусственная вентиляция легких, позднее отхождение от кардиотонической поддержки. Несмотря на многочисленные исследования, патофизиология возникновения ФП в раннем послеоперационном периоде после кардиохирургических операций остается до конца не изученной. В настоящее время воспаление как один из основных факторов риска развития ФП вызывает большой интерес у исследователей. Сходство по времени между пиком возникновения ФП после операций на сердце и активации системы комплемента с выпуском провоспалительных цитокинов (3-6-е сутки после операции) предполагает наличие воспалительного компонента в механизме запуска послеоперационной ФП. В связи с данной гипотезой ряд ученых уделяют внимание исследованию эффективности препаратов с противовоспалительными свойствами в профилактике послеоперационной ФП, касающейся операций на открытом сердце. В частности, последние несколько лет активно изучают эффективность статинотерапии. Ее положительный эффект связан с плейотропными (противовоспалительными, антиоксидантными, мембраностабилизирующими) свойствами статинов. Исследование ARMYDA-3 (Atorvastatin for Reduction MYocardial Dysrhythmia After cardiac surgery)первое исследование, в котором было показано, что предоперационное назначение аторвастатина в дозе 40 мг снижало частоту возникновения ФП после кардиохирургических операций. Для получения более мощной доказательной базы в отношении эффективности статинотерапии в профилактике послеоперационной ФП необходимо проведение более крупных рандомизированных клинических исследований для каждого вида кардиохирургических операций.
