La enfermedad celíaca (EC) es un trastorno inflamatorio crónico causado por una respuesta inmune anormal inducida por una intolerancia permanente al trigo, la cebada y el centeno en la dieta. Dicho diagnostico se realiza por medio de pruebas sanguíneas de tamizaje en conjunto con confirmación endoscópica y biopsia. La EC se ha asociado a otras enfermedades autoinmunes y manifestaciones sistémicas. En los últimos años se ha estudiado con mayor relevancia la relación de EC con la infertilidad no explicada, dentro de los mecanismos fisiopatológicos se destaca la deficiencia nutricional y procesos autoinmunes especificos ya sea en el momento de la concepción o que estos mecanismos se desarrollen previo provocando deficiencia ovárica. Se ha propuesto realizar el tamizaje de EC en pacientes infertiles, sin embargo ha generado controversia debido a su utilidad. Si bien es cierto la relación entre dichas patologías es fuerte, se ha demostrado en los diferentes estudios que la utillidad en realizar dicho tamizaje de rutina depende del tipo de población y su prevalencia
La rabdomiólisis se trata de una patología caracterizada por la destrucción del tejido muscular esquelético y la consecuente liberación al torrente sanguíneo de sustancias intracelulares como creatinquinasa (CK), mioglobina, lactato deshidrogenada y electrolitos. Fisiopatológicamente, esta disrupción se produce fundamentalmente por un desbalance hidroelectrolítico intracelular, principalmente por el aumento del calcio citoplasmático, ocasionado por daños en la membrana de las fibras musculares o por la disminución de la producción de ATP. Si bien las posibles etiologías de este síndrome clínico son amplias, en esta revisión se enfatizará en la rabdomiólisis asociada al ejercicio físico; la cual se puede ver precipitada por la condición física y la experiencia deportiva, la intensidad y duración del ejercicio, el tipo de actividad física, los hábitos alimenticios y el ambiente donde se lleve a cabo el entrenamiento. Se enfatiza en el proceso diagnóstico de la patología, el manejo del cuadro basado principalmente en una hidratación adecuada y en las medidas preventivas a la hora de realizar ejercicio para disminuir el riesgo de rabdomiólisis.
El síndrome de intestino irritable (SII) es el diagnóstico gastroenterológico más común, con una elevada prevalencia global que oscila entre el 5% al 21%. Esta enfermedad de curso recurrente crónico se caracteriza por su traslape con otros trastornos funcionales y su afectación en la calidad de vida de los pacientes. Actualmente son pocos los tratamientos que ofrecen un alivio sintomático satisfactorio, por lo que en el manejo efectivo del SII se ha dado importancia a la consejería sobre cambios en el estilo de vida, actividad física, dieta, entre otros. Este enfoque de esfuerzo personal por parte del paciente se sustenta en la relación del estado de estrés crónico con la exacerbación de los síntomas de SII. Las interacciones cerebro-intestino pueden jugar un rol en la asociación entre la actividad física y los síntomas en el SII. La mayoría de estudios concuerdan que las intervenciones que propician el aumento de la actividad física mejoran los síntomas gastrointestinales del SII; así como diferentes aspectos de la calidad de vida, fatiga, depresión y ansiedad a largo plazo.
El alcoholismo es una enfermedad que puede ser discapacitante, con una alta prevalencia en población adulta mayor en Costa Rica. Para su diagnóstico se utilizan los criterios del DSM-5. El consumo excesivo de alcohol de forma aguda puede causar un cuadro de intoxicación aguda, así como el cese abrupto de este puede producir un síndrome de abstinencia; ambos casos requieren de un manejo oportuno, para evitar complicaciones. En el manejo integral del adulto mayor, el uso de farmacoterapia efectiva que incluyen medidas farmacológicas y no farmacológicas es de suma importancia para el abordaje de los distintos espectros del abuso del alcohol. Los desórdenes del consumo del alcohol, tienen múltiples consecuencias orgánicas y psicosociales a corto y largo plazo, es por esto que el manejo adecuado y oportuno puede ayudar a prevenirlas o a manejarlas.
El síndrome de realimentación (SR) se encuentra constituido por una serie de alteraciones metabólicas, producidas por el reinicio de la alimentación posterior a un periodo prolongado de ayuno. Durante el periodo de ayuno, el cuerpo entra en un estado de metabolismo catabólico; el cual posteriormente se convierte en un estado anabólico con la reintroducción de la alimentación. Dicho proceso explica las distintas alteraciones hidroelectrolíticas. La prevención constituye el pilar de tratamiento en este padecimiento, el NICE ha elaborado una serie de criterios para agrupar a las personas según el riesgo. Entre los pacientes con mayor riesgo se encuentran aquellos que presentan alguna patología oncológica, enfermedad crónica, trastornos alimenticios, entre otros. El SR requiere de un manejo precoz basado en un adecuado diagnóstico y la correcta administración tanto calórica como electrolítica según cada caso.
