La discitis y el absceso de psoas son patologías infrecuentes cuya ocurrencia está relacionada con factores de riesgo como diabetes mellitus, edad avanzada y estados de inmunosupresión. Dado su curso evolutivo insidioso y su cuadro clínico inespecífico, representan un desafío diagnóstico, en donde la tomografía axial computarizada y la resonancia magnética juegan un rol importante. Presentamos el caso clínico de una paciente con antecedentes de diabetes mellitus, que ingresa por un cuadro clínico de infección complicada del sistema urinario, que se manifestó con fiebre y dolor lumbar persistente; en el curso de su evolución ante la sospecha clínica, fue evaluada con tomografía y resonancia magnética, pudiéndose diagnosticar discitis y absceso de psoas secundario, con aislamiento de E. coli, requiriendo de antibioticoterapia y drenaje percutáneo de la colección.
