For those patients who fail to respond to PM + PHLP and/or those who have AAST-OIS penetrating liver injuries, Grades IV and V would benefit from immediate intraparenchymal exploration and SVL.
ResumenIntroducción. El trauma vascular es un evento de baja frecuencia, con alta carga de morbilidad y mortalidad, que afecta mayormente a hombres jóvenes y en muchas ocasiones requiere manejo quirúrgico. Se asocia con múltiples complicaciones, que van desde la reintervención quirúrgica, hasta la amputación de la extremidad, las cuales, a su vez, se ven influenciadas por variables relacionadas tanto con el trauma como con la atención hospitalaria. Objetivo. Caracterizar los pacientes mayores de 13 años con traumatismos arteriales periféricos en el Hospital Universitario Hernando Moncaleano Perdomo, de Neiva, Huila, Colombia, en el periodo enero de 2014 a diciembre de 2016. Materiales y métodos. Estudio observacional, retrospectivo, de tipo descriptivo, con población a conveniencia de pacientes mayores de 13 años que sufrieron traumatismos arteriales periféricos y fueron llevados a procedimiento quirúrgico en el Hospital Universitario Hernando Moncaleano Perdomo. Resultados. Se encontraron 54 pacientes con trauma arterial periférico, cifra que representa el 1,5 % de los ingresos por trauma en la Institución durante el período de estudio. El género más representativo fue el masculino (n=49), las heridas por arma cortopunzante el principal mecanismo de trauma (n=22) y las extremidades el área comúnmente afectada (n=47). Se halló un puntaje de gravedad de la lesión promedio al ingreso de 26 puntos, y un tiempo de isquemia de extremidad mayor a seis horas al momento del ingreso en el 31,5 % de los casos (n=17). Conclusiones. Las intervenciones quirúrgicas fueron hechas en mayor medida por cirugía de trauma, seguido de cirugía vascular. El principal manejo fue la anastomosis término-terminal y el injerto de safena invertida. Las complicaciones fueron en mayor medida la trombosis arterial (n=12), seguida de la amputación de extremidad (n=7). Introducción. El manejo de las hernias se ha instaurado como un problema quirúrgico común, estimándose su aumento en los próximos años. El objetivo del presente trabajo fue describir el curso clínico, los aspectos del tratamiento quirúrgico y factores asociados a la presencia de complicaciones en pacientes intervenidos por hernia incisional. Métodos. Estudio descriptivo en el que se analizaron las características de una cohorte de pacientes llevados a corrección quirúrgica de hernia incisional en el Hospital Universitario Hernando Moncaleano Perdomo, un centro de alta complejidad en Neiva, Colombia, entre 2012 y 2019. Los datos fueron recolectados en programa Microsoft Excel® y analizados en SPSSTM, versión 21. Resultados. Se realizaron 133 correcciones de hernias incisionales, 69,9 % en mujeres y la mayoría ubicadas en la línea media (84,2 %). La edad media de los pacientes al momento de la intervención fue de 52 años ±14,6. Las comorbilidades más frecuentes fueron obesidad, hipertensión y diabetes. La causa más frecuente de la hernia fue traumática (61,7 %). La frecuencia de complicaciones fue superior al 50 %, en su mayoría menores; se encontró asociación con obesidad para la presencia de seroma. La mortalidad fue del 2,3 %. Conclusión. La hernia incisional es un problema de salud pública. Consideramos que la obesidad y el uso de malla pueden ser factores de riesgo asociados con la presentación de complicaciones postoperatorias, así como el aumento de los gastos relacionados con días de hospitalización.
Objectives:To analyze the impact of the Drunk-Driving Law on the characteristics and severity of traffic accident victims and to identify risk factors for mortality before and after law enforcement. Materials and methods:A retrospective, quantitative study that analyzed victims of traffic accidents attended at a hospital in São Paulo, Brazil, between 2006 and 2010, through the analysis of medical records. The Drunk-Driving Law was considered the time frame of this research, and the victims were distributed in two groups: Before the law (January 2006 to June 2008) and after the law (July 2008 to December 2010). Pearson chi-square, Mann-Whitney, and multiple logistic regression tests were used, with a significance level of 5%. Results:The sample consisted of 1,405 victims, the majority being males (78.01%), with a mean age of 37.39 years. In the group comparison (before and after the Drunk-Driving Law), there was a significant difference related to the external cause, admission to the ICU, and discharge conditions. Factors associated with prelaw mortality were age, number of injured body regions, and New Injury Severity Score. The length of hospital stay and the Revised Trauma Score were considered as protective factors for this outcome. After the validity of the law, in addition to the variables described earlier, the head/ neck and abdomen regions most severely injured were added as risk factors for mortality. Conclusion:The impact of the Drunk-Driving Law set a positive outcome in the survival of the victims. However, when it comes to the statistics and severity of the trauma, it is necessary to sustain the law with reinforcement of the inspection so that more lives are saved. Clinical significance:The results of this study provide support to managers on the importance of sustaining the law and the need of implementing new trauma prevention strategies. Conflict of interest: None RESUMOObjetivo: analisar o impacto da "Lei Seca" nas características e gravidade das vítimas de acidentes de trânsito e identificar os fatores de risco para mortalidade antes e após a implantação da lei. Resultados: A casuística foi composta por 1.405 vítimas, a maioria do sexo masculino (78,01%), com idade média de 37,39 anos. Na comparação dos grupos (antes e após a Lei Seca), houve diferença significativa em relação à causa externa, admissão na Unidade de Terapia Intensiva e condição de saída hospitalar. Os fatores associados à mortalidade antes da lei foram idade, número de regiões corpóreas acometidas e New Injury Severity Score. O tempo de permanência hospitalar e o escore do Revised Trauma Score foram considerados fatores de proteção para este desfecho. Após a vigência da lei, além das variáveis descritas anteriormente, foram acrescidas as regiões cabeça/pescoço e abdome mais gravemente acometidas como fatores de risco para mortalidade. RESULTS:Of the 617 MVC, 212 were frontal crashes and included in the study. The population was 45% male with average age 12.4 AE 5.0 years. Sex, age and impact rating did not correlate with MAIS. Subcutaneous fat cross-sectional area, visceral fat crosssectional area, bone density and psoas muscle area did not correlate with injury severity for MAIS 2+ abdominal, thoracic or spinal injuries. CONCLUSIONS:Though subcutaneous fat has previously correlated with decreased abdominal and pelvic injury in adults, this study shows that subcutaneous fat does not impact the risk of thoracic, abdominal or spine injuries in children involved in frontal crashes.
