A Hipertensão Arterial Sistêmica (HAS) enquadra-se como uma doença crônica não transmissível (DCNT), caracterizada por aumento contínuo da pressão arterial. Quando elevada, a pressão sistólica é igual ou superior a 140 mmHg e/ou pressão diastólica igual ou superior a 90 mmHg. Essa aferição deve ser efetuada com aparelho adequado em, no mínimo, duas situações distintas e sem o uso de medicamentos com finalidade anti-hipertensiva. O Diabetes Mellitus (DM) é uma doença crônica, que ocorre, no momento em que o pâncreas não é mais capaz de produzir insulina ou quando o corpo não consegue fazer bom uso da insulina que produz. A DM pode ser classificada em Tipo 1, uma doença autoimune, em que o sistema imunológico do corpo ataca as células beta-pancreáticas. Assim, ocorre uma deficiência completa na produção de insulina, tornando-se necessária a reposição com insulina exógena; e o tipo 2 na qual, a hiperglicemia resulta da produção inadequada de insulina e incapacidade do corpo para responder plenamente a este hormônio, definido como resistência à insulina. A utilização de aplicativos móveis na área da saúde está em crescente expansão, pois esse tipo de suporte pode proporcionar aos profissionais alcançarem mais precisão e agilidade em seus trabalhos. Assim, como o monitoramento remoto, o apoio ao diagnóstico e o apoio à tomada de decisão. Existem tanto no mercado nacional quanto internacional inúmeras ferramentas com o intuito de auxiliar o farmacêutico na prática da atenção farmacêutica, o que pode resultar em uma melhoria significativa na qualidade de vida do paciente hipertenso e diabético. Objetivos: Descrever o efeito de uma intervenção telefônica do farmacêutico para com paciente na identificação de barreiras à adesão na melhoria dos efeitos farmacológicos dos medicamentos entre pacientes não aderentes com hipertensão e/ou diabetes mellitus. Ainda analisar e informar a importância de uso das tecnologias digitais como ferramentas no auxílio do profissional farmacêutico na prestação do serviço de atenção farmacêutica nas doenças hipertensivas e diabetes mellitus, de forma prática e eficiente. Dessa forma, como resultados deste estudo, a literatura mostra-se o quanto é importante as tecnologias e aplicativos móveis que estão colaborando para a construção de uma nova modalidade na atenção farmacêutica e assistência em saúde como nas doenças crônicas com pessoas hipertensas e pessoas diabéticas, no qual, os aplicativos móveis torna-se uma ferramenta de informações e comunicações referentes à saúde destas pessoas.
Os fármacos são importantes no tratamento das doenças, sendo responsáveis pela melhora da qualidade de vida da população. Entretanto, sabe-se que automedicação é uma forma comum de atenção à saúde, levando em consideração o objetivo de tratar, aliviar sintomas, doenças percebidas, ou mesmo de promover a saúde, independentemente da prescrição profissional. Apesar do fácil acesso proporcionar alívio rápido da dor ao paciente, o que é uma questão particularmente importante quando há sistemas de saúde congestionados, e obter consultas médicas, podem ser um problema pois existem muitos riscos associados aessa prática. Os MIPs (Medicamentos Isentos de Prescrição) são medicamentos que possuem ativos como qualquer outro, capaz de gerar reações adversas, efeitos colaterais e intoxicação se usado de forma errada e excessiva. Os riscos ao se automedicar são grandes levando em conta que muitos pacientes que procuram por esses medicamentos possuem algum problema psicológico.
