IntroduçãoO gânglio esfenopalatino tem sido implicado na gênese das cefaleias pós-punção dural (CPPD). Sugere-se que o bloqueio do gânglio esfenopalatino (BGEP) pode aliviar os sintomas dessa cefaleia que diminui a qualidade de vida de vários pacientes. O objetivo deste estudo é averiguar se o BGE se mostrou uma opção terapêutica segura e eficaz no manejo da CPPD.Material e MétodosTrata-se de uma revisão sistemática da literatura, delineada em dois dos quatro critérios da estratégia PICO, nos bancos de dados PubMed e Lilacs, com os descritores: “Sphenopalatine Ganglion Block AND Headache”, totalizando 98 artigos. Os critérios de inclusão foram: ensaios clínicos randomizados, relatos de caso, publicação até 10 anos, língua inglesa. Excluiu-se os estudos duplicados, aqueles artigos ainda não concluídos.ResultadosEm um ensaio clínico randomizado, duplo cego, evidenciou- se que os pacientes que realizaram BGEP com 0,3 ml de bupivacaína a 0,5%, apresentaram uma queda significativamente na escala de dor (NRS score), 15 e 30 minutos após administração do tratamento, durando por 24h após o procedimento. Em consonância, um relato de caso, em que foi realizado um BGEP em contexto ambulatorial, verificou- se que a administração de levobupivacaína a 0,5% no tratamento para CPPD causou uma melhora significativa dos sintomas por mais de 24 horas após o procedimento. Outros dois relatos de caso, um com três pacientes (lidocaína viscosa a 2%) e outro com um paciente pediátrico, concluíram que o tratamento com BGEP reduziu significativamente a intensidade da CPPD. Em desacordo, um dos ensaios clínicos randomizados concluiu que o BGEP bilateralmente com 1 ml de anestésico local (lidocaína 4% e ropivacaína 0,5%) ou placebo (solução salina) não teve efeito estatisticamente significativo na intensidade da CPPD após 30 minutos. Por fim, um relato de caso concluiu que dois BGEP transnasal com lidocaína a 4% em cada narina de uma paciente grávida resultou em melhora significativa no mesmo dia, o estudo ressaltou que o bloqueio deve ser considerado para tratamento da dor de cabeça em grávidas, pelo profundo alívio da dor e a prevenção de medicamentos sistêmicos.ConclusãoO BGEP se mostrou seguro e eficaz na redução da intensidade da CPPD durante as primeiras horas, no entanto os estudos apresentaram discordância a respeito da sua eficácia a longo prazo, necessitando de estudos de maior evidência científica sobre esse assunto.
IntroduçãoA migrânea é um tipo de cefaleia primária que apresenta alta prevalência na sociedade, ocasionando prejuízo na qualidade de vida dos acometidos. Embora ainda questionado a relação entre obesidade e migrânea, observa-se uma maior prevalência dessa cefaleia em indivíduos obesos. O objetivo desta revisão é buscar na literatura atual se há alguma relação da obesidade com a manifestação da migrânea, avaliando a eficácia de dietas menos calóricas e redução de peso.Material e MétodosFoi realizada uma revisão sistemática da literatura nos bancos de dados do PubMed e Lilacs, com os descritores: “Obesity AND Migraine”, sendo selecionados os relatos de caso, metanálises e ensaios clínicos publicados nos últimos 10 anos (n= 22 artigos). Foram excluídos os estudos que ainda não foram concluídos e aqueles que não se enquadravam nos objetivos, assim restaram 13 artigos.ResultadosOs estudos concluíram que há uma relação direta entre a obesidade e a pré-obesidade com o aumento da intensidade e frequência da migrânea, independentemente da idade, sendo o sexo feminino mais afetado. Um dos artigos apontou que a dieta cetogênica pobre em calorias é eficaz para a melhoria rápida e a curto prazo das crises de migrânea em pacientes com excesso de peso, no entanto, necessita-se de mais estudos que apontem a duração efetiva da dieta e se ela se aplica a todos os pacientes obesos. Também, foi apontado que a terapia comportamental e o exercício físico obtiveram efeitos significativos sobre a intensidade e a frequência dos episódios de migrânea nos pacientes obesos. Ademais, um ensaio clínico observou que o programa de intervenção interdisciplinar reduziu significativamente a adiposidade em adolescentes obesos entre 14 e 16 anos, sendo essa mudança no IMC associada com uma melhora significativa da migrânea nos 12 meses após a terapêutica. Acerca da cirurgia bariátrica, dois estudos concluíram que houve relação entre essa técnica cirúrgica e o alívio acentuado da gravidade e duração da enxaqueca, bem como o aumento significativo do número de dias sem crises, principalmente em mulheres obesas na pré menopausa.ConclusõesA redução de peso mostrou impacto positivo no manejo da migrânea em pacientes obesos, dessa forma, a dieta cetogênica pouco calórica, exercícios físicos regulares, a terapia comportamental e, em casos necessários, a cirurgia bariátrica são opções terapêuticas seguras e eficazes na redução da frequência e intensidade das crises de enxaqueca.
IntroduçãoA neuralgia do trigêmeo se caracteriza através de episódios recorrentes de dor severa, localizada em pequenas áreas do rosto. Acredita-se que a sua principal causa seja por uma compressão do quinto nervo craniano por uma alça arterial ou venosa aberrante, levando à desmielinização das fibras sensoriais do nervo trigêmeo. Alguns estudos têm demonstrado que o uso de Toxina Botulínica do tipo A (Tb-A) apresenta efeitos positivos no manejo da neuralgia do trigêmeo. O objetivo desta revisão é avaliar a eficácia terapêutica da Tb-A no manejo da dor em pacientes com NT.Material e MétodosTrata-se de uma revisão sistemática da literatura, delineada nos 4 critérios da estratégia PICO, nos bancos de dados PubMed e Lilacs. Os descritores utilizados foram: “ trigeminal neuralgia AND Botulinum toxin type A”, sendo selecionados apenas os ensaios clínicos randomizados, metanálises e relatos de caso publicados nos últimos 10 anos. Excluiu-se os estudos que não se enquadravam nos objetivos, restando 15 artigos para compor a revisão.ResultadosUm dos estudos analisados concluiu que a Tb-A é uma estratégia eficiente e segura no alívio da dor em pacientes com NT, com impactos positivos na ansiedade, depressão e qualidade do sono, sendo que as taxas de eficácia analisadas após 1, 2, 4 e 6 semanas de tratamento foram 48,28%, 66,67%, 78,16% e 80,46%, respectivamente. Também, um ensaio clínico que avaliou 15 pacientes com NT tratados com Tb-A concluiu que houve redução da frequência e da severidade dos ataques de dor em todo o grupo, sendo que em 7 pacientes (46%) a dor foi completamente erradicada e não houve necessidade de mais medicação, em 5 os antiinflamatórios não esteroides (AINEs) foram suficientes para aliviar os ataques de dor e apenas em 3 pacientes houve a necessidade do uso de medicamentos anticonvulsivantes após a injeção da toxina. Por fim, um relato de caso acompanhou um paciente em uso carbamazepina há 3 anos para tratamento da NT, que foi submetido a injeção de 50 U de Tb-A intramuscular no masseter, concluindo que esse tratamento produziu efetiva diminuição da dor por cerca de 5 meses nesse paciente. Os estudos não encontraram efeitos colaterais significativos secundários ao uso da Tb-A.ConclusãoA Tb-A se mostrou uma opção terapêutica segura e efetiva no manejo da dor dos pacientes com NT, reduzindo o consumo de AINEs e antiepiléticos, aumentando a qualidade de vida e do sono e, consequentemente, diminuindo os escores de depressão e ansiedade.
