The urbanization of visceral leishmaniasis in Brazil has been related to environmental changes, migration, interaction and spread of sylvatic reservoirs and infected dogs to areas with no transmission, and adaptation of the vector Lutzomyia longipalpis to the peridomiciliary environment. From 1980 to 2005, Brazil recorded 59,129 cases of visceral leishmaniasis, 82.5% of which in the Northeast region. Visceral leishmaniasis gradually spread to other regions of the country: in 1998 these other regions reported 15% of all cases, but by 2005 this proportion had increased to 44%. From 1998 to 2005, indigenous cases were reported in 1,904 different municipalities of the country (34.2%). Reservoir and vector control pose major challenges for disease control, since there is a need for better knowledge of vector behavior in urban areas, and control activities involve high operational costs. In recent years the Brazilian Ministry of Health has supported research on the laboratory diagnosis of infection and disease in humans and dogs, treatment of patients, evaluation of the effectiveness of control strategies, and development of new technologies that could contribute to the surveillance and control of visceral leishmaniasis in the country.
An outbreak of American cutaneous leishmaniasis was reported in 2002 in Mariluz, northwestern Paraná State, Brazil. Of 38 humans who were investigated, four had healed lesions, ten showed lesions in the healing process, and 24 had active lesions. Of the 126 dogs, 20 (15.9%) presented suggestive lesions and 24 (19%) had positive serology. Parasites isolated from two patients and three dogs were identified as Leishmania (Viannia) braziliensis serodeme I. The captured sand flies were identified as Nyssomyia whitmani, N. neivai, and Migonemyia migonei. Considering that the region where the cases occurred is similar to other old human settlements in Paraná State, the environmental alterations and remaining forests facilitate the maintenance of the parasite's enzootic cycle and transmission to humans and domestic animals, thereby maintaining the endemicity of American cutaneous leishmaniasis.
Introdução: Leishmaniose Visceral (LV) é uma doença sistêmica que afeta animais e o homem. Incluída entre as seis doenças endêmicas mais importantes no mundo, é de notificação compulsória e apresenta número elevado de óbitos. Objetivo: Descrever o perfil clínico-epidemiológico dos casos notificados de LV em Governador Valadares nos anos de 2008 a 2015. Métodos: Estudo transversal descritivo por meio de dados secundários de domínio público do banco de dados do SINAN-Net. Figuras e tabelas foram confeccionados com os programas EpiInfoTM versão 7.2 e Microsoft Excel®. Resultados: Foram confirmados 154 casos de LV residentes em Governador Valadares distribuídos em cinquenta (59,5%) dos 84 bairros. As taxas de incidência e letalidade acumuladas foram de sete casos por 100 mil habitantes e 13%, respectivamente. Variáveis demográficas: 69,5% eram do sexo masculino; 29,2% foram menores de cinco anos de idade; 40,7% eram de raça/cor parda. As principais manifestações clínicas foram febre (92,8%), fraqueza (77,9%), emagrecimento (74%), palidez (72%) e hepatomegalia (57,1%); 11,7% dos casos eram de coinfectados por HIV; 99 (66,4%) casos receberam a droga de primeira escolha. 87% dos casos foram curados clinicamente. Conclusões: O perfil apesentado foi semelhante ao descrito na literatura. Constou-se alto percentual de cura pelo diagnóstico precoce e tratamento oportuno. Pel;a detecção tardia, os indivíduos demoram a ser tratados e tendem a ter o quadro agravado, evoluindo para o óbito. Com estes dados, espera-se promover o diálogo entre os diversos setores da saúde para implementação de estratégias e políticas públicas controladoras e preventivas da doença.
Objetivo: descrever o perfil epidemiológico e a magnitude dos homicídios ocorridos no município de Maceió, estado de Alagoas, Brasil, no período de 2007 a 2012. Métodos: estudo descritivo dos óbitos por homicídios registrados no Sistema de Informações sobre Mortalidade (SIM) ocorridos em Maceió-AL. Resultados: registraram-se 5.735 homicídios no período, correspondendo à média de 955,8 homicídios/ano, com taxas que variaram de 89,6 óbitos/100 mil habitantes (2012) a 111,4 óbitos/100 mil hab. (2011); 94,8% das vítimas eram do sexo masculino, 66,2% eram jovens de 15 a 29 anos e 79,9% da raça/cor parda; os homicídios ocorreram, majoritariamente, nos períodos noturno e de madrugada (51,5%), com envolvimento de arma de fogo (87,8%), em bairros da periferia da cidade (32,6%). Conclusão: a mortalidade por homicídios foi elevada, acometendo principalmente pessoas jovens, pardas e do sexo masculino; destaca-se a necessidade de políticas públicas efetivas para o combate à violência.
