Introduction: Medical students’ mental health has been a concern to the scientific community, especially as a result of the epidemic of mental comorbidities that have become commonplace among the various social groups in modern society. Objectives: To evaluate the prevalence of Burnout among first- to fourth- year medical students and compare different criteria to define the syndrome. Methods: A total of 511 students from three universities in Brazil answered validated instruments to assess burnout (The Maslach Burnout Inventory) and quality of life (WHOQOL-BREF), and a questionnaire prepared by the authors to assess sociodemographic data Results: There was a prevalence of 31.1% of three-dimensional burnout, 37% of two-dimensional burnout and 44,8% of one-dimensional burnout. There were worse levels of emotional exhaustion among students with two-dimensional burnout, compared to those with only one-dimensional criterion, and worse levels of depersonalization among students with three-dimensional burnout, compared to those with two-dimensional criterion only. The same assessed variables showed correlation with all three criteria. Considering the four domains of quality of life - psychological, physical, environment and social relationships, the psychological and physical domains were the ones that correlated the most to all three aspects of the three-dimensional criteria. Emotional exhaustion was the dimension with the strongest association with three of the four quality of life domains. Conclusions: We question whether the three-dimensional criterion should really be the gold standard to define Burnout.
Introdução: Abordagens minimamente invasivas no transplante renal foram descritas recentemente e mais pesquisas são necessárias acerca desse assunto. Há carência de informações sobre o manejo perioperatório desses pacientes. Por conseguinte, no presente estudo descrevemos a experiência em um centro transplantador com técnicas de incisão mínima em cirurgias de transplante renal. Além disso, descrevemos os principais resultados dos procedimentos realizados. Objetivo: Descrever a experiência em um centro terciário, o qual realizou transplantes renais de 20 casos de implantes com incisão minimamente invasiva, relatando suas complicações e comparando dados na literatura. Métodos: Estudo de coorte retrospectivo, incluindo 20 receptores de transplante renal com doadores vivos e falecidos, em um centro único, os quais foram submetidos a cirurgia minimamente invasiva entre julho de 2010 e maio de 2011. Foi definida como cirurgia minimamente invasiva aquela com tamanho de incisão de 5 a 9 centímetros. Foram avaliadas complicações cirúrgicas, duração da hospitalização, disfunção inicial do enxerto (DGF) e função renal em 10 semanas. Resultados: Dos 20 pacientes estudados, 12 eram do sexo masculino e oito do sexo feminino. A média de idade foi de 43 anos. 17 pacientes foram transplantados com rins de doadores falecidos. A média de incompatibilidade HLA foi de 4,3 e nenhum paciente teve reação positiva no PRA. O tempo de isquemia fria foi em média 15,7 horas, variando de uma a 26 horas. O tamanho das incisões variou de 5,5 a 9 cm, com média de 7,6 cm. Houve apenas uma complicação com necessidade de reabordagem (hematoma perirrenal). A permanência hospitalar média foi de 19,2 dias. 40% dos pacientes tiveram DGF e não houve perda de enxerto. A creatinina média após 10 semanas foi de 1,5 mg/dl. Conclusões: O transplante renal com incisão minimamente invasiva mostrou-se uma estratégia segura e com baixo percentual de complicações.
Depression is a common mental condition worldwide and a major cause of debility, with the potential to impair the quality of life of affected people. Medical students are more likely to develop psychiatric disorders, with depression, anxiety, and burnout being the most common ones. The aim of this study was to investigate the prevalence of depressive symptoms in medical students and their association with burnout and aspects of life. This is an analytical cross-sectional study. Data collection was performed using four validated scales and a questionnaire in 511 medical students from the first to the fourth year from different universities. Depression was present in 49% of the students. A correlation was found between depression and female gender. Regarding the students' daily sleep time, 54.5% of the students slept only 3 to 6 hours per night. The use of psychoactive substances was reported by 29.9%. Regarding the emotional support offered by universities, about 92.8% of students with depressive symptoms reported not receiving adequate emotional support. As for quality of life, all domains were impaired among students with depression. According to the two-dimensional criterion, 37% of the students have burnout, with correlation between the presence of the syndrome and depressive symptoms. In addition, burnout proved to be an independent risk factor for suicidal ideation and self-mutilation. New methods that can help detect and address factors that trigger stress and depressive symptoms in medical students are needed to reduce the incidence of depression.

A cocaína é uma droga com notória capacidade de afetar adversamente quase todos os órgãos do corpo e pode causar uma infinidade de anormalidades multisistêmicas secundárias. No presente estudo, relatamos três casos de insuficiência hepática aguda complicada por rabdomiólise e lesão renal aguda após consumo de cocaína e álcool. O início da doença, as manifestações clínicas, os dados laboratoriais, o diagnóstico e a terapêutica de cada paciente foram registrados. Nota-se a presença de causas multifatoriais para a ocorrência de Insuficiência Hepática e Rabdomiólise nos três casos relatados. Os testes laboratoriais revelaram que a concentração sérica de transaminases, bem como os de creatinafosfoquinase (CPK) aumentaram e que os sintomas de insuficiência renal aguda estavam presentes, o que forneceu um diagnóstico exato de insuficiência hepática aguda complicada por grave rabdomiólise e lesão renal aguda dialítica. Rabdomiólise é uma condição de hipermioglobinúria, sendo uma dos principais fatores que fomentam uma piora da função renal. A terapia da fase inicial envolveu suporte a ressuscitação volêmica associada a terapia dialítica com rigoroso controle da função renal. Os três pacientes desenvolveram comprometimento de múltiplos órgãos, mas apenas um foi a óbito devido a gravidade do quadro. Considerando os três casos apresentados, concluímos que a insuficiência hepática, complicada por rabdomiólise, secundária ao uso de cocaína e álcool pode ter mau prognóstico clínico, a depender de vários fatores. E os médicos devem estar cientes dos potenciais efeitos causados pela cocaína, a fim de gerir as múltiplas complicações associadas ao abuso destas substâncias.
