Objetivo: Discutir o caso de um resultado tardio de lábio leporino, sendo feita a ressecção da cicatriz, reconstrução do lábio e rinoplastia para um melhor resultado estético e funcional para a paciente.Método: Estudo descritivo, tipo relato de caso. Todos os dados foram coletados do prontuário da paciente, além de pesquisa bibliográfica em base de dados PubMed.Resultados: Relata-se o caso de uma paciente de 22 anos com resultado tardio de fissura labial, pré-forame incisivo, tendo feito a primeira cirurgia no lábio aos 30 dias de vida, sem abordagem nasal. Além disso, possuía obstrução nasal severa por desvio septal e hipertrofia de conchas inferiores. O plano cirúrgico incluiu rinosseptoplastia e correção de cicatriz com reconstrução da cinta labial. A evolução mostrou-se sem intercorrências, com boa cicatrização, bem como ótima aceitação estética e funcional pela paciente. A maioria das técnicas atuais de queiloplastia, inclusive a usada na paciente anteriormente, limita-se à reconstrução labial e parte do nariz, que costuma permanecer assimétrico. Segundo alguns autores, a reconstrução precoce do assoalho nasal nas rinoqueiloplastias leva a melhores resultados futuros. Se a criança for submetida à rinoplastia primária adequada, habitualmente não se realiza a rinoplastia secundária. Em alguns casos, há recomendação de realizar outra abordagem quando o após os 16 anos.Conclusão: O conhecimento sobre rinoqueiloplastia secundária mostra-se importante pois alguns resultados estéticos e funcionais não são plenamente atingidos nas etapas iniciais do tratamento. Logo, o domínio sobre rinoqueiloplastia secundária em adultos é de extrema importância para propiciar um método terapêutico mais adequado.
