RESUMEN: Se describe el tratamiento periodontal en un defecto óseo situado mesial a la pieza 2.3 y distal a la pieza 2.2, mediante la terapia combinada de derivado de la matriz del esmalte (DME) y aloinjerto óseo mineralizado. Se valoró la zona afectada y se planeo su abordaje quirúrgico para la terapia combinada. Seis meses después del tratamiento se observó una reducción significativa en la profundidad al sondeo y ganancia en los niveles de inserción, así como el relleno óseo del defecto en la evaluación radiográfica. Los resultados reafirman la efectividad del DME y el aloinjerto óseo mineralizado en la terapia regenerativa periodontal. RESUMEN: Agenesia es la ausencia de dientes por alteraciones genéticas aisladas o sindrómicas. La agenesia del tercer molar está asociada a malformaciones y es considerada por diversos autores consecuencia de la evolución humana (Larmour et al., 2005). Son los dientes con mayor prevalencia de agenesia junto con segundos premolares e incisivos laterales (Fuller & Denehy, 1984). La prevalencia varía entre 9% y 37% (McNamara & Foley, 2006), en tanto Arboleda et al. (2006 señalan una prevalencia del 20%. La literatura señala variables estadísticas porcentuales, por género, por arcada dentaria, por lado y por diente, con escasos artículos sobre grupos originarios de Chile. La población en estudio consistió en 78 individuos, 42 hombres y 36 mujeres, entre 18 y 40 años, de la etnia aymara, sin exodoncias del tercer molar ni tratamientos ortodónticos y sin malformaciones congénitas. Se determinó el grado de mestizaje mediante técnica serológica de hemo-aglutinación y por aplicación de la fórmula de Bernstein, que demostró 51% de mezcla indígena. A cada individuo se le tomó radiografía panorámica para observar presencia o ausencia de terceros molares. Se determinó un 21,79% de agenesia, con mayor porcentaje masculino. En la muestra total y en hombres hay mayor agenesia de terceros molares mandibulares, en cambio en mujeres existe igual porcentaje de agenesia en ambas arcadas. Predominan agenesias izquierdas, lo mismo se comprueba en el género masculino, mientras que en mujeres se comprueba igual porcentaje bilateral. Predomina la agenesia de un solo molar en ambos sexos. Nuestros resultados fueron comparados con atacameños y con jóvenes antofagastinos y no se determinaron diferencias significativas al 95%, coincidiendo con lo señalado en la literatura. La investigación representa un aporte a la antropología del norte de Chile, pero considerando lo reducido de la muestra no es posible determinar variables étnicas.
RESUMEN:Los dientes impactados presentan muchos problemas. Pueden comprometer la movilidad dentaria, la estética, y los resultados funcionales. Después del tercer molar superior, los caninos superiores presentan la impactación más común, los cuales pueden impactar en posición vestibular o palatina, y son más comunes en mujeres que en varones. Con un manejo terapéutico quirúrgico y ortodoncico adecuado, los caninos superiores impactados pueden erupcionar y ser guíados a una posición adecuada en el arco dental. En este artículo se presenta un caso quirúrgico-ortodóncico de un canino superior impactado vestibular en una mujer adulta, tratada mediante la técnica cerrada con preservación de las papilas.PALABRAS CLAVE: Canino maxilar impactado; Técnicas quirúrgica; Técnicas de ortodoncia. INTRODUCCIÓNEl diagnóstico precoz, la intercepción oportuna y un tratamiento multidisciplinario apropiado hacen posible que un canino maxilar impactado pueda ser reposicionado en el lugar que le corresponde dentro del arco dentario, de ahí la importancia que los clíni-cos le han dado a este tema, debido a que es el diente con mayor frecuencia de impactación seguido por los terceros molares (Bishara, 1992;Ngan et al., 2005). Otros autores lo reportan en segundo lugar después del tercer molar mandibular (Mcsherry, 1998), de los cuales dos tercios se presentan en posición palatina y un tercio en la zona vestibular (Ericson & Kurol, 1988). El 8% de todos los pacientes que padecen impactación de caninos maxilares es de tipo bilateral (Bishara).Mucho tiempo se especuló que los caninos maxilares tenían un trayecto más tortuoso que cualquier otro diente para su erupción y que esta situación estaba relacionada con tres factores: el tiempo requerido para su desarrollo, la posición cambiante durante su desarrollo y la distancia que recorre en su vía de erupción. Teorías actuales apuntan a que la impactación de los caninos maxilares estaría relacionada a la ausencia o deformidad de la raíz del incisivo lateral (Becker, 2007) o a condicionantes genéticas (Peck et al., 1994;Baccetti, 1998), sin embargo, ambas teorías están enfocadas al canino impactado en posición palatina.La erupción ectópica vestibular del canino maxilar está fuertemente asociada con la falta de espacio o el apiñamiento del arco alveolar (Leonardi et al., 2010), y pareciera estar relacionada por largos periodos de erupción, discrepancias del arco dentario maxi-*
