La hernia diafragmática postraumática, sigue siendo una patología de difícil diagnóstico, por su baja frecuencia, la falta de diagnóstico oportuno, nos lleva a su más temida complicación, la estrangulación, que conlleva una elevada morbimortalidad, de aquí se desprende la importancia del tema. Las lesiones del lado izquierdo son más frecuentes según la literatura revisada, ya que el hemidiafragma derecho es más grueso y está protegido por el hígado. Se presenta el caso de un paciente con antecedentes de accidente automovilístico (atropellamiento) que requirió de colocación de clavo centromedular por fractura de fémur derecho, un año previo a su ingreso, el paciente acudió a urgencias, por presentar dolor abdominal alto, palidez de tegumentos, mal hidratado, náuseas, e hipertermia, timpanismo y rigidez abdominal involuntaria, se observa a su ingreso también datos apendiculares, como el signo de Blumberg, y el de Rovsing, se observó en la citometría hemática leucocitosis y neutrofilia, fue valorada y operada por cirugía general reportando apendicitis fase I, estando en el área de recuperación presentó dolor súbito intenso a nivel torácico y dificultad respiratoria, se tomó radiografía de tórax, encontrando derrame pleural derecho, por lo que se colocó sonda endopleural derecha gastando 1250cc de material achocolatado, finalmente se realizó tomografía axial computada contrastada toraco-abdominal, reportando hernia diafragmática derecha con inclusión de lóbulo hepático izquierdo y colon transverso perforado, por lo que se realizó Toracotomía anteroposterior derecha, reducción del contenido, aseo de la cavidad torácica, hernioplastía diafragmática derecha con puntos separados de polipropileno, más laparotomía exploradora con transversostomía tipo Hartman. Paciente presentó buena evolución, fue egresado a los 28 días de la intervención. A los 6 meses se realizó restitución del tránsito intestinal, con evolución favorable. Por la baja incidencia de estos casos y la presentación atípica de este, se decide su publicación.
El tricobezoar por definición operativa se refiere a la colección de pelo en la cámara gástrica, dónde en contadas ocasiones se puede observar extensión hacia primera o segunda porción del duodeno. Una extraña entidad como el Síndrome de Rapunzel es nulamente perseguido como sospecha diagnóstica, esto gracias a la poca información acerca del diagnóstico oportuno, las manifestaciones clínicas tan variadas y el poco conocimiento de los criterios diagnósticos. Presentación de caso: paciente femenina de 35 años de edad, que acude al servicio de urgencias por presentar dolor moderado en epigastrio e hipocondrio derecho que se irradia a espalda, acompañado de náuseas y vómitos de contenido gástrico así como evacuaciones melénicas, anorexia y saciedad temprana, a la exploración física se palpa una masa indurada en epigástrio, no móvil, dolorosa a la palpación, en la tomografía computarizada de abdomen simple, se observa la cámara gástrica ocupada, en su luz una imagen de aspecto espongiforme la cual se extiende a duodeno en donde se aprecian zonas de alta densidad sugestivas de hemorragia, sugestivas de un probable tricobezoar (síndrome de Rapunzel) con presencia de sangrado a nivel de duodeno en su primera y segunda porción, por lo que se decide manejo quirúrgico
Se realizó un estudio retrospectivo, observacional, descriptivo, unicéntrico, para evaluar de la calidad de las intervenciones quirúrgicas de urgencia en el Hospital General de la Zona Norte de Puebla “Bicentenario de la Independencia”, basándonos en el sistema de calificación del Comité de Intervenciones Quirúrgicas de Cuba, para determinar el tipo de cirugía. Se incluyeron las cirugías realizadas de urgencia del tracto gastrointestinal en su primera intervención, en el ciclo comprendido del 01 de enero hasta el 31 de diciembre del 2019, considerando las 5 patologías más frecuentes acorde a el servicio de estadística. Se incluyeron en el estudio 519 pacientes, encontrando que el 85.5% (444) de los diagnósticos fueron correctos mientras que el 14.5% (75) fueron diagnósticos incorrectos. Además, el 98.5% (511) de las cirugías fueron justificadas mientras que el 1.5% (8) fueron cirugías injustificadas. Por otro lado, observamos que el 90.2% (468) de las cirugías, la intervención realizada fue suficiente, mientras que en el 3.9% (20) de las cirugías, la intervención fue insuficiente y en el 6.0% (31) de los casos la intervención fue exagerada según lo establecido en la guía de práctica clínica vigente. En otras palabras, la calificación de nuestro hospital se encontró, en su mayoría dentro del tipo A y tipo B, es decir: Diagnóstico correcto y operación justificada, y Diagnóstico incorrecto, pero operación justificada, respectivamente.
El Trauma Torácico representa un gran problema de salud pública, siendo este la tercera causa de muerte en pacientes menores de 40 años en Estados Unidos de América, en México, contamos con poca información al respecto, pero encontramos que los accidentes automovilísticos, son los responsables del Trauma Torácico en un 43%, y el suicidio un 29%, seguido del 22% las lesiones secundarias a homicidios.7, 19 Por lo anterior consideramos importante conocer el comportamiento epidemiológico y los resultados de nuestro manejo en el Trauma Torácico, de nuestro Hospital General de la Zona Norte de Puebla BI, que a pesar de ser un hospital de segundo nivel, de haber cruzado por una pandemia donde tuvimos la necesidad de convertirnos en Hospital COVID, y de contar con un Hospital exclusivo para Trauma a unos minutos de distancia de nosotros, no dejamos de recibir pacientes con Trauma Torácico con lesiones tempranas y tardías. Se realizó una investigación descriptiva, retrospectiva de los últimos 5 años en la Atención de Pacientes con Trauma Torácico en nuestro hospital, encontramos que al igual que otras series, el trauma torácico se presentó principalmente en hombres en un 88.9%, la edad promedio fue de 32.2 años, las lesiones que atendimos en nuestro hospital fueron principalmente del tipo trauma penetrantes, por arma punzocortante y la colocación de sonda endopleural fue suficiente como procedimiento invasivo, para resolver la mayoría de los casos, por otro lado, no se observó preferencia entre las técnicas para abordar la cavidad torácica, realizando el mismo número de Toracotomías Axilares Verticales Extendidas, que las Toracotomías Posterolaterales. Finalmente 16 de los 18 pacientes, presentaron buena evolución con alta hospitalaria.
