Report on the development of pseudohypopyon immediately after intravitreal injection of triamcinolone acetonide (TAAC). Two phakic patients presenting with a transient pseudohypopyon after having been treated with intravitreal triamcinolone. One had a clinically significant macular edema with cystoid component (CSME with CMS) and the other, active Vogt-Koyanagi-Harada (VKH) with serous retinal detachment of the macula. One eye from each patient developed a pseudohypopyon with crystal deposits adherent to the corneal endothelium. In one case it appeared right after the injection and disappeared spontaneously in 24 hours. In the other patient it appeared on day 3 and disappeared also spontaneously within 2 days. The pseudohypopyon is an important sign that can be observed after intravitreal injection of TAAC, in phakic patients, with spontaneous resolution and without complication. The pseudohypopyon caused by the deposition of TAAC in the anterior chamber immediately after its injection into the vitreous should be differentiated from other forms of hypopyon associated with this type of treatment.
Objetivo: Determinar os efeitos da injeção subtenoniana posterior de corticóide (ISPC) sobre a pressão intra-ocular (Po) e acuidade visual em uma série de pacientes com uveíte. Métodos: Estudo prospectivo de 18 pacientes que foram submetidos à injeção subtenoniana posterior de acetato de triancinolona (Kenalog 40 mg -9 pacientes, 14 injeções) ou acetato de metilprednisolona (Depomedrol 40mg -9 pacientes, 15 injeções) para tratamento de inflamação intra-ocular crônica e/ou edema macular cistóide. Resultados: A acuidade visual final melhorou em 92% dos pacientes após a primeira injeção periocular de corticóide. Cinqüenta por cento melhoraram 1 linha e 42% melhoraram pelo menos 3 linhas, sendo que o tempo médio para a melhora foi de 3 semanas. Aumento da pressão intra-ocular ocorreu em 44 % dos pacientes (8 pacientes) com média de 31 mmHg, variando de 21 a 38 mmHg. O aumento da pressão intra-ocular foi mais freqüente nos pacientes jovens e nos que receberam Kenalog , com início, em média, após 2,5 semanas. Conclusão: A injeção subtenoniana de corticóide é uma forma de tratamento eficaz para a baixa acuidade visual secundária a alguns tipos de uveíte, como uveíte intermediária, doença de Behçet, síndrome de Vogt-Koyanagi-Harada, vasculite retiniana e artrite reumatóide. Porém, pode induzir aumento da pressão intra-ocular em alguns pacientes, especialmente em crianças e jovens. Corticóides de depósito subtenoniano na forma de acetato de triancinolona (Kenalog -40mg) ou acetato de metilprednisolona (Depomedrol -40mg) são amplamente utilizados para o tratamento de quadros inflamatórios oculares, refratários ao tratamento com corticóide tópico e sistêmico. O uso da via periocular tem o objetivo de aumentar a concentração intra-ocular da droga, através da absorção transescleral, possibilitando a redução da terapia sistêmica e minimizando os efeitos colaterais da corticoterapia prolongada. A liberação do corticóide de depósito é regulada pela composição bioquímica do mesmo, sendo que as preparações insolúveis em água persistem por mais tempo em contato com a esclera (1) . As principais indicações dessa via de administração de corticóide são a baixa acuidade visual associada à inflamação intravítrea crônica e/ou presença de edema macular cistóide. A melhora da acuidade visual após esse tipo de tratamento varia de 65 a 85% (2)(3) , o que estimula seu uso em casos refratários a outras formas terapêuticas.
