Mục tiêu: Xây dựng và thẩm định phương pháp định lượng tacrolimus trong máu toàn phần người bằng phương pháp sắc ký lỏng khối phổ (LC-MS/MS) để ứng dụng trên bệnh nhân (BN) ghép thận. Đối tượng và phương pháp: Tacrolimus và chuẩn nội được chiết từ máu toàn phần bằng cách sử dụng methanol/1,125M ZnSO4 (66/34, v/v) và được tách bằng rửa giải gradient trên cột ACQUITY UPLC® BHE C18 (2,1x100 mm; 1,7 µm) ở 25°C sử dụng pha động là acetonitril và amoni acetat 5 mM. Tốc độ dòng là 0,2 mL/phút, phần dịch trong 5 µL được bơm vào phân tích trong hệ thống LC-MS/MS. Các ion được phát hiện bằng cách theo dõi đa phản ứng trên hệ thống MS/MS Xevo TQD. Phương pháp được thẩm định theo hướng dẫn của FDA và được ứng dụng để định lượng nồng độ tacrolimus trên BN ghép thận, đồng thời so sánh mối tương quan và sự khác biệt với phương pháp miễn dịch hóa phát quang (CLIA). Kết quả: Phương pháp có tổng thời gian sắc ký là 5 phút. Đường chuẩn tuyến tính trong khoảng 1,0 - 100,0 ng/mL với giới hạn định lượng dưới là 1 ng/mL. Độ chính xác trong ngày và khác ngày lần lượt nằm trong khoảng 93,3 - 109,2% và 96,0 - 108,4%. Độ đúng trong ngày và khác ngày dao động trong khoảng 0,8 - 9,4%. Tỷ lệ thu hồi của tacrolimus dao động từ 102,6 - 107,8%. Các chỉ tiêu về độ đúng, độ chính xác và tỷ lệ thu hồi đáp ứng các tiêu chuẩn của FDA. Kết quả về mối tương quan và sự khác biệt nồng độ tacrolimus giữa hai phương pháp cho thấy phương pháp LC-MS/MS có độ tin cậy để ứng dụng trong theo dõi nồng độ tacrolimus trên lâm sàng. Kết luận: Nghiên cứu đã xây dựng, thẩm định và ứng dụng được phương pháp định lượng tacrolimus trong máu toàn phần đạt được các tiêu chí nhanh, nhạy, tính chọn lọc và độ đặc hiệu cao, đáp ứng yêu cầu phân tích trong các nghiên cứu tương đương sinh học của tacrolimus.
