These results clearly demonstrate that in CHF patients significant changes in SS and SAS occur, which are thought to relate to the morphofunctional alterations of myocardium. The observed changes in the levels of these biomarkers may serve as potential surrogates to monitor severity of disease, to evaluate response to drug treatment, and as a rational basis for new therapeutic approaches.
резюме неудовлетворенность результатами медикаментозной и немедикаментозной терапии фибрилляции предсердий заставляет клиницистов и ученых искать новые методы лечения этой аритмии. Цель исследо-вания: изучить особенности ремоделирования сердца и характер морфологических изменений миокарда у пациентов с хронической сердечной недостаточностью при фибрилляции предсердий. Материалы и методы. у пациентов с фибрилляцией предсердий при хронической сердечной недостаточности, имею-щих и не имеющих фибрилляцию предсердий изучались эхокардиографически особенности ремодели-рования миокарда и микроскопически морфология миокарда сегмента ушка правого предсердия. резуль-таты. у пациентов с хронической сердечной недостаточностью, имеющих фибрилляцию предсердий, по сравнению с пациентами, не имеющими нарушение ритма, выявлены более выраженные гипертрофия стенок миокарда левого и правого желудочков, дилатация обоих предсердий и левого желудочка, повы-шенное давление в легочной артерии и венах. При постоянной форме в отличие от пароксизмальной фор-мы фибрилляции предсердий структурные нарушения стенок и камер сердца более выражены. выявлен интерстициальный отек миокарда ушка правого предсердия у пациентов с фибрилляцией предсердий по сравнению с пациентами без нарушений ритма. выводы. более выраженная дилатация предсердий у пациентов с фибрилляцией предсердий при хронической сердечной недостаточности сопровождается интерстициальным отеком миокарда, который, возможно, и является одной из причин возникновения и поддержания фибрилляции предсердий. ключевые слова: фибрилляция предсердий, пароксизмальная форма, постоянная форма, дилатация предсердий, интерстициальный отек миокарда.

Dissatisfaction with the results of drug and non-drug therapy of atrial fibrillation causes clinicians and scientists to seek new methods for treatment of this arrhythmia. objective. The paper aims at studying the features of cardiac remodeling and morphological changes of myocardium in chronic heart failure patients with atrial fibrillation. materials and methods. The features myocardial remodeling and the morphology of the myocardium of a right atrial auricle segment were studied, respectively, by echocardiography and by microscopy in chronic heart failure patients with and without atrial fibrillation. results. Compared to patients without cardiac arrhythmia, patients with chronic heart failure, with atrial fibrillation, revealed more pronounced hypertrophy of the walls of the myocardium of the left and right ventricles, increased myocardial mass index, dilatation of both right and left atria, increased pressure in the pulmonary artery and veins. With a constant form, unlike paroxysmal atrial fibrillation, structural disorders of the walls and chambers of the heart were more pronounced. A positive correlation was found between the severity of pulmonary arterial hypertension and concentrations of serotonin (r=0.48) and 5-hydroxyindoleacetic acid in the plasma (r=0.9).
A relationship between morphofunctional changes in the heart and predominance of adrenergic or serotoninergic influences on the strength of myocardial contraction in the right atrium was revealed in patients with chronic heart failure.
