IntroductionThe main purpose of the study is to examine the relationship between health expenditure indicators and economic growth in OECD countries.MethodsIn this context, health expenditures and economic indicators data of 21 OECD countries were analyzed by the Driscoll-Kraay standard error approach within the scope of panel data analysis. While Gross Domestic Product (GDP) and income per capita were used as dependent variables, the amount of out-of-pocket health spending, per capita health expenditure, the amount of public health expenditure, the ratio of drug expenditures to gross domestic product, the share of current health expenditures in GDP were used as independent variables.ResultsAccording to the results, in the model (Model 1) where real GDP level was used as the dependent variable, all health expenditure indicators were positively related to the economic growth. When the estimation results of Model 1 are examined, it is predicted that there will be an increase of 0.09% in GDP in case of a 1% increase in the share allocated to health services from GDP. In case of a 1% increase in the amount of out-of-pocket spending on healthcare, it is foreseen that there may be an increase of 0.04% in the real GDP. In the model (Model 2) where the per capita income variable is the dependent variable, it is seen that the increase in out- of-pocket health spending has a decreasing effect on the per capita income level, while the increase in public expenditures has an increasing effect on the per capita income level. From the findings of Model 2, it was found that if a 1% increase in the share of current health expenditures in GDP, there may be an increase of 0.06% in the amount of per capita income.DiscussionConcludingly, it is possible to say that that public resources allocated to health services play an important role in the economic growth.
Measuring performances of the hospitals under Ministry of Health is a must for the efficient use of the resources. To serve this purpose, data envelopment analysis, capable of measuring relative effectiveness of decision units by processing more than one input and output, has been used. By means of data envelopment analysis technique, we have aimed to use the resources efficiently. For this reason, the model has been run input oriented. With the study, effectiveness and profitability measurements were performed in the hospitals under Ministry of Health. For this purpose, by using 2007 data on 5 financial inputs and 1 financial output variables from 16 different hospitals, effectiveness measurement was carried out and profit/loss values of the units were evaluated. According to the results obtained improvement for each unit is described and effects on their profitability are also stated. Considering the profit / loss values of the hospitals, it is concluded that they should cut their expenses at the rate of 10, 43% in order to reach break-even point regarding profit and loss balance.
ÖzBu çalışmada; seçilmiş 4 hastanede bir yıllık DRG verileri içinde Solunum Sistemi Enfeksiyon (E62) vakalarına giren hastalar demografik, klinik ve finansal açıdan karşılaştırmalı değerlendirilmesi amaçlanmıştır. Çalışma verileri, 2011 yılında Sağlık Bakanlığına bağlı Göğüs Hastalıkları Hastanelerinde yatarak tedavi gören E62 vakalarını içermektedir. Bu vakalar demografik verileri, ortalama kalış süreleri, taburculuk durumları, vaka karma indeksleri, bağıl değerler ve geri ödeme miktarları açısından hastane ve ülke bazında karşılaştırılmıştır. E62 vakaları erkeklerde % 67, kadınlarda % 33 oranında görülmüştür. E62C DRG'sinin %52 görülme oranıyla en sık görülen E62 vakası olduğu, E62 vakaları için ortalama hastanede kalış süresinin 18,7 gün olduğu, E62 vakalarının % 91,4 oranla şifa durumu ile taburcu edildiği, hastane geri ödemelerinde bağıl değerlerin ve E62 DRG çeşitliliği frekanslarının doğrudan etkili olduğu ve vaka karma indeksleri ile geri ödeme miktarı arasında pozitif yönlü ilişki olduğu tespit edilmiştir. Hem hastaneler arasında hem de ülkeler arasında DRG verileri bakımından önemli oranda farklılıklar olduğu tespit edilmiştir. DRG verilerinin finansman amaçlı kullanımı yanında vakaların epidemiyolojik ve demografik parametreler açısından karar destek sistemleri için önemli bir kaynaktır. Study data, attached to the Ministry of Health in 2011 in Chest Diseases Hospital inpatient cases include E62. These cases demographic data, the average length of stay, discharge status, case mix index, in terms of relative values and repayment amounts were compared on the basis of hospitals and countries. E62 cases in men 67%, women 33% were observed. E62C drg's 52% of the incidence of the most common E62 cases where, E62 cases the average length of hospital stay of 18.7 days, which was the E62 cases of 91.4% compared healing with the condition were discharged, hospital reimbursement in the relative values and E62 DRG diversity frequencies that are directly affected and with case-mix index is a positive correlation between the amount of reimbursement have been identified. Both between countries and between hospitals terms DRG data were found to be significantly different. Besides the use of DRG data for the financing of the cases in terms of epidemiological and demographic parameters is an important resource for decision support systems. Sağlık kurumlarında yapılan işlerin tek profesyonel grubu tarafından yerine getirilmesi mümkün değildir. Bu durum sağlık kurumlarında ekip çalışmasını zorunlu kılmakta ve hizmetler ekip çalışmasının sonucu olarak verilmektedir. Sağlık ekiplerinin çalışmalarını etkili bir şekilde yerine getirmeleri ise ayrı bir önem arz etmektedir. Çünkü etkili ekip çalışmasının sağlıkta birçok faydası bulunmaktadır. Bu çalışmada, ekip çalışması ile ilgili temel kavramlar açıklanmış ve literatür taraması yapılarak etkili ekip çalışmasının genelde ve sağlıktaki önemi faydaları ile ortaya konulmaya çalışılmıştır. Çalışma sonucunda etkili ekip çalışmasının başlıca faydaları şu şekilde sıralanmıştır: yatış sürelerinin ve maliyetlerin düşürülmesi, gelişmiş hizmet sunumu, gelişmiş hasta memnuniyeti, çalışan memnuniyeti, çalışan motivasyonu ve işbirliği ve yenilik.
ÖZETTalasemi hastalığında preimplantaston genetik tanı ve tedavisi (PGTT) hasta çocuğu olan ailelerin bu çocuğun tedavisi için yalnızca kök hücre vericisi kardeş doğmasına yönelik olarak SGK tarafından geri ödeme kapsamına alınmıştır. Bunun dışında SGK tarafından taşıyıcı ve hasta ebeveynlerin hastalıklı çocuk doğurmasının önüne geçilecek bir ödeme yapılmamaktadır. Talasemili hasta harcamaları; konunun tedavi açısından çok boyutlu olması nedeniyle SGK MEDULA verileri üzerinden çalışılmıştır. Harcamaların belirlenmesinde bu işlemler için sağlık tesislerince yapılan gerçek giderler yani maliyetler yerine, geri ödeme kurumu olan SGK'ya olan yükün daha iyi vurgulanabilmesi için bizzat Sağlık Uygulama Tebliği'ndeki işlem puanları üzerinden yapılan harcamalar dikkate alınmıştır.Hasta bireylerin oluşmasının önüne geçilmesi tüm harcamalardan daha büyük bir önem taşımaktadır. Bununla birlikte yapılan projeksiyonlar ve karşılaştırılmalar göstermiştir ki PGTT'nin daha ilk gebelikten itibaren riskli bireylerin evlilikleri yönünden geri ödeme kapsamına alınması SGK harcamalarını büyük ölçüde azaltacaktır. ABSTRACTPre-implantation genetic diagnosis and treatment (PGDT) has been covered under the reimbursement scheme of SSI (Social Security Institution of Turkey) for only those whose treatment is depended on the birth of a sibling stem cell donor. Apart from these payment scheme of SSI, there is no coverage to prevent those who are genetic carriers or has the illness from having a child with the disease. The expenses of Thalassemia patients are studied using SSI MEDULA data because the treatment has many aspects. After the scope of the data has been determined, the data is requested from the SSI's Department of General Health Insurance and a study has been conducted using the MEDULA system. Instead of using the real cost to the healthcare facilities we used the procedure score to determine the expenses so that we can better reflect the financial burden to SSI which is the payer. Prevention of the disease is more important than all the expenditures. Psychosocial effects to the individual patient, families and to the society comes before all the costs. However, the cost forecasts and comparisons show that reimbursement of PGDT starting from the first pregnancy for the parents who carry risk factors, will decrease the financial burden to the SSI a great deal. Moreover, because the genes that carry the disease is not transferred to the next generation, the number of sick people and the genetic carriers will decline as the time goes by.
