The aim of this study was to evaluate the deaths of Syrian refugees caused by mushroom poisoning in the light of patient data, living conditions and autopsy findings. Methods: An evaluation was made of 6 Syrian refugees who died at Malatya Turgut Ozal Medical Centre as a result of mushroom poisoning in 2014. Diagnosis of mushroom poisoning was made from the anamnesis, laboratory test results and clinical evaluation of the patients. The autopsy procedures for all the cases were performed at the Malatya Local Authority Forensic Medicine Institution. All the cases were evaluated in respect of age, gender, height, weight, symptoms of poisoning, duration of hospitalization, month of poisoning, laboratory parameters, autopsy findings and histopathological findings. Results: The 6 cases included in the study comprised 4 males and 2 females with a mean age of 27±18.7 years. Some of the cases were related. It was determined that all the cases were poisoned after eating wild mushrooms that they had collected. In all the cases, the liver function tests were impaired, with values determined of mean ALT 5456.83±2556.47 U/L, AST 2517.66±2351.56 U/L, INR 5.04±2.04 and ammoniac 904.16±308.6 µg/dl. In the autopsy, widespread foci of bleeding were determined in the internal organs, a jaundiced appearance and fluid accumulation in the body cavities (pleural effusion and acid). Thrombosis in the vena porta hepatica was determined in 1 case. In the histopathological examination, massive liver necrosis was seen in all the cases. Conclusion: Mushroom poisoning as a definitive cause of death must be determined with a detailed history, autopsy findings and histopathology together. Furthermore, refugees should be warned that there could be similar species of mushrooms growing in different natural environments and that they could be poisonous.
ÖZETDünyada ve ülkemizde ateşli silahlar nedeniyle ölüm oranı gün geçtikçe artmaktadır. Tüm medikolegal ölümler göz önüne alındığında ateşli silah ile ölümler önemli bir oran oluşturmaktadır. Çalışmamızda ateşli silah ile meydana gelen ölüm olgularının genel özelliklerinin belirlenmesi ve benzer çalışmalarla karşılaştırılma yapılması amaçlanmıştır.Elazığ'da Fırat Üniversitesi Hastanesi Adli Tıp Anabilim Dalında 2010-2012 yıllarında ölü muayenesi ve otopsisi yapılan 838 olgudan ateşli silah sonucu öldüğü tespit edilen 120 (%14.3) olgu retrospektif olarak incelendi. Bu olgular yaş, cinsiyet, orjin, kullanılan silah türü, atış mesafesi, giriş sayısı, yara lokalizasyonu, olay yeri ve olayın meydana geldiği zaman parametrelerine göre değerlendirildi. Olguların 102'si (%85) erkek 18'i (%15) kadın, en küçüğü 9, en büyüğü 86 yaşında olup yaş ortalaması 31.58±14.34'dir. Ölümlerin en sık 20-29 yaş grubunda (%45.8), Mayıs ayında (%14.2) ve yaz mevsiminde (%30,8) olduğu görüldü. Orjin olarak en sık terör çatışması (%32.2) ve intihar (%32.2) görülürken bunu cinayet (%27.3) ve kaza sonucu ölüm izlemektedir. Olguların 81'nde (%66.1) de tek giriş deliği olduğu ve ölümlerin en sık baş-boyun bölgesinden (%37.5) yaralanma sonucu oluştuğu görüldü.Ateşli silahlar ile ölümler; bölgemizde meydana gelen adli ölümlerin önemli bir kısmını oluşturmaktadır. Dünya ve Türkiye verileriyle karşılaştırıldığında terör ve sosyokültürel özellikler gibi bölgesel faktörlere bağlı farklılıklar görülmektedir. Ateşli silahlarla ölüm oranının azaltılması için ruhsatsız silah kullanımının önlenmesi, silah ruhsatı verilmeden önce tıbbı muayene ve ruhsat verme kriterlerine daha fazla dikkat edilmesi, ayrıca bu konularda toplumun eğitilmesi gerektiği kanısındayız.Anahtar kelimeler: Ateşli silah yaralanması, otopsi, intihar ABSTRACTThe rate of deaths due to firearms is gradually increasing in our country and worldwide. Considering all medicolegal deaths, deaths due to firearms constitute a significant proportion. The purpose of our study is to determine general characteristics of death cases due to firearms and to compare them with similar studies. 120 cases (14.3%) who were determined to be died due to firearms out of 838 cases that were undergone death examination and autopsy between 2010-2012 in Fırat University Hospital, Department of Forensic Medicine in Elazig were retrospectively evaluated. These cases were assessed based on the parameters such as age, sex, origin, type of gun used, shooting distance, number of entry, localization of wound, place of event and time of the event. 102 (85%) of the cases were men and 18 (15%) were women. Television, which emerged in parallel with the scientific developments in the last century, is the most common and effective mass media component, and became an indispensible part of the daily life of the modern societies. Because of its widespread use, television, especially as the result of falling on someone in the house, can cause injuries even death.In this study, 8-month and 1-year old male infants who died after the fall of television and forensic autopsy were evaluated. In the first case, the television fell on the 8-month-old male infant as the result of the incident of two siblings of the subject (ages: 2 and 3) were playing with television stand, while the mother was folding the clothes next to him. In the second case, television and television stand fell on the unaccompanied subject while he was playing with them. In both cases, forensic autopsy revealed skull fractures, brain hemorrhage and lung damage.It is important to raise the awareness of the families, and to take the simple and adequate precautions in the home environment in order to protect the children as the future of the society from the domestic accidents.

CO zehirlenmelerinin ülkemizde sık görülmesi, sosyal ve önlenebilir bir sağlık sorunu olması nedeni ile önemi bir kat daha artmaktadır.Çalışmamız Fırat Üniversitesi Hastanesi Adli Tıp Anabilim Dalı tarafınca 2006-2012 yılları arasında otopsisi yapılan karbonmonoksit zehirlenmesine bağlı ölümlerin tamamını kapsamaktadır. Bu yıllar arasında otopsileri yapılan toplam 1964 adli olguya ait kayıtlar incelenmiş olup CO zehirlenmesi sonucu ölen 29 olgu yaş, cinsiyet, olayın meydana geldiği ay, orijin, ölüm süresi, CO kaynağı ve karboksihemoglobin (COHb) değerleri açısından değerlendirilmiştir. Ölüm meydana gelen 29 adli olgu Elazığ'da 7 yılda otopsisi yapılan 1964 adli olgunun %1.47'sini oluşturmaktadır. Olguların yaş ortalaması 31,5 ±19,5 olup 19'u (%65,5) erkektir. Tüm olguların %3,4'ünde orijin intihardır. Ölümlerin %31'i şubat ayında , %75,9'u evlerde, %79,3'ü olay anında gerçekleşmiştir. Olguların %41,4'ünde CO kaynağı olarak soba kömürü bulunmuştur. Olgularda tespit edilen ortalama COHb düzeyi %49.05 ±16.32'dir.Karbonmonoksit zehirlenmeleri genellikle önlenebilir kazalar sonucu meydana gelmesine rağmen hala toplumsal bir sağlık sorunu olarak karşımıza çıkmaktadır. ABSTRACTChemical asphyxia first comes to mind at the mention of carbon monoxide poisoning, are common in developed and developing countries. It's becoming more important since it is very common in our country and it is a social and avoidable health problem. O u r s t u d y c o n s i s t s o f a l l d e a t h s d u e t o carbonmonoxide poisoning and autopsied by Elazig FıratUniversity Hospital, Department of Forensic Medicine between 2006 and 2012. Records of a total of 1964 criminal cases who were autopsied between these years were examined and 29 cases who died due to CO poisoning were evaluated for age, sex, the month of the event occurred, origin, death time, CO source and carboxyhemoglobin (COHb) values. 29 Death occurred criminal cases were constituting 1.47% of 1964 criminal cases who were autopsied within 7 years in Elazig. Mean age of the cases was 31.5±19.5 years, and 19 (65.5%) were men. Origin in 3.4% of all cases was suicide. 31% of deaths occurred in february, 75.9% at home and 79.3% at the moment of the event. CO source was found to be charcoal in 41.4% of cases. Mean COHb level detected in cases was 49.05 ±16.32%.Although carbonmonoxide poisonings generally occur as a result of avoidable accidents, it is still a social health problem.
