We present a case report of a digital papillary adenocarcinoma (DPA) on the left thumb with slow growth that was completely excised, without recurrence or metastasis during 24-month follow-up. DPA is a rare malignant tumor often located on the digits of the hand. Due to its slow growth and a non-specific appearance, the diagnosis is often missed or delayed. Treatment should be surgical with clear resection of the margins due to high risk of recurrence and/or metastasis.
Pertrochanteric hip fractures are among the most common and the use of short cephalomedullary nails as the treatment of choice is increasing. A systematic review regarding distal locking options for short cephalomedullary nails was undertaken using Medline/PubMed®, Embase® and Cochrane Library® in order to evaluate current indications, associated complications and to provide treatment recommendations. The results seem to support the use of distal static locking for unstable fractures, dynamic locking for length stable/rotational unstable fractures and no locking for stable fractures. Complications associated with distal locking include iatrogenic fractures, thigh pain, delayed union and nonunion, implant failure, screw loosening and breaking, drill bit breaking, soft tissue irritation, femoral artery branch injury, intramuscular haematoma and compartment syndrome. It is also associated with longer operative time and radiation exposure. In unlocked constructs, dorsomedial comminution and nail/medullary canal mismatch contribute to peri-implant fractures. Anterior cortical impingement is associated with cut-out and nonunion. Most studies comparing distally locked and unlocked nails report a short follow-up. Background: Glomus tumors are benign and rare vascular tumors, with an incidence of less than 2% in relation to all soft tissue tumors. About 75% occur in hand, especially in the nail bed, even though these tumors may also occur in atypical locations. They are characterized by a triad composed of pain, cold intolerance, and point tenderness.
Case Presentation: We present a clinical case of a male patient with 67 years old with a painful lesion on the subcutaneous tissue of the knee with 5 years of evolution. The pain exacerbated with the cold. He underwent surgical excision with pain resolution, and no recurrence or metastasis was found at 30-month follow-up.
Conclusion: The rapid diagnosis of glomus tumors is important to avoid treatment delays leading to chronic pain. It is important to consider glomus tumors in the differential diagnosis of subcutaneous lesions.
