The proportion of Epstein-Barr virus-associated gastric carcinoma (EBV-GC) was examined in 149 Japanese-Brazilian and 151 non-Japanese-Brazilian gastric-carcinoma cases using in situ hybridization (ISH) assay to detect EBV-encoded small RNA (EBER), and the results were compared with our referent Japanese data. We found that 4.7% of cases in Japanese Brazilians were EBERpositive. This frequency was slightly lower than that of the referent Japanese, among whom 6.2% of 2038 gastric-carcinoma cases were EBER-positive. On the other hand, the non-Japanese-Brazilian series showed a significantly higher proportion of EBV-GC (11.2%) than the referent group did (P = = = =0.01). Although EBV-GC was predominant in males among non-Japanese Brazilians (M/ F = = = =3.6, P = = = =0.047), as was the case in Japanese (M/F = = = =2.7), Japanese Brazilians did not show such a male predominance. The sex-ratio difference between the Japanese Brazilians and Japanese was statistically significant (P = = = =0.005). In conclusion, the present study in Japanese Brazilians and Japanese yielded no evidence suggesting any change in the frequency of EBV-GC caused by migration, except the absence of male predominance, which was observed both in Japanese and non-Japanese Brazilians.
INTRODUÇÃOA dexmedetomidina é o mais recente e seletivo agonista alfa-2 adrenérgico. A ausência de estudos com a administração dessa droga via oral em pacientes pediátricos nos motivou a investigar sua eficácia como medicação pré-anestésica em crianças.
MÉTODOSApós aprovação do Comitê de Ética da instituição e obtenção do Termo de Consentimento Pós-Informado, 24 crianças entre 1 e 10 anos, em estado físico P1 e P2 (antigos ASA I ou II), submetidas a cirurgias ambulatoriais infra-umbilicais sob anestesia geral associada a bloqueio de nervos periféricos ou epidural sacra, foram divididas aleatoriamente em dois grupos. Um grupo recebeu dexmedetomidina 1,5 mcg/kg via oral aproximadamente 60 minutos antes da cirurgia, enquanto o outro grupo recebeu midazolam 0,5 mg/kg via oral 30 minutos antes da cirurgia. A sedação e dificuldade de separação dos pais foram avaliadas na admissão do centro cirúrgico, conforme escala previamente utilizada por Nishina. Avaliamos os seguintes parâme-tros durante o procedimento: freqüência cardíaca (cardioscópio, DII) e pressão arterial não-invasiva, assim como as condições e tempo para alta da sala de recuperação pós-anestésica (SRPA). Consideramos hipotensão e bradicardia como a queda maior que 20% dos valores obtidos antes da indução anestésica.
RESULTADOSNenhuma criança do grupo da dexmedetomidina apresentou dificuldade de separação dos pais, enquanto que duas (16,66%) do grupo do midazolam apresentaram dificuldade de separação dos pais na admissão no centro cirúrgico. No grupo da dexmedetomidina, sete pacientes (58,33%) apresentaram sedação eficaz, já no grupo midazolam apenas dois (16,66%).
DISCUSSÃOOs resultados foram comparados pelo teste exato de Fisher bi-caudal e os valores não foram estatisticamente significantes. Não houve diferença estatística entre os valores de freqüência cardíaca e pressão sanguínea. Observamos maior intervalo de tempo entre a administração da MPA e a extubação e alta da SRPA no grupo da dexmedetomidina. Não existiu diferença significativa entre os grupos quanto à sedação residual na alta da SRPA.
CONCLUSÃONeste estud,o a dexmedetomidina proporcionou sedação e facilitou a separação dos pais de maneira semelhante ao midazolam -medicação consagrada como pré-anestésico em pacientes pediátricos. Apesar da pequena amostra e da falta de estudos sobre a farmacocinética para avaliar a biodisponibilidade da dexmedetomidina após administração via oral em pacientes pediátricos, essa medicação demonstrou-se adequada quanto à dose e tempo de administração preconizados neste estudo. No entanto, o aumento do tempo de permanência na SRPA não faz desse agente a primeira escolha para MPA em pacientes ambulatoriais.
