RESUMOA taxa de mortalidade infantil (TMI) é reconhecida não só como indicador das condições de saúde da população menor de 1 ano, mas também como a variável que melhor reflete as condições gerais de vida, sendo considerada síntese da qualidade de vida e do nível de desenvolvimento de uma população (1). Assim, é possível estabelecer relações entre a magnitude da TMI e as características do modelo de desenvolvimento (2).Os fatores de risco para a mortalidade infantil, didaticamente, são divididos em três grupos: fatores biológi-cos, sociais e relacionados à assistência médica. Entre os fatores de risco bioló-gicos estão: idade materna, paridade, estatura, peso antes da gravidez, ganho ponderal durante a gestação, doença materna (diabetes, hipertensão), intervalo entre os nascimentos e perdas perinatais prévias, peso ao nascer e duração da gestação. Neste trabalho analisou-se a assistência ao parto, caracterizando o perfil das principais maternidades e o deslocamento da clientela, ou seja, o fluxo entre residência e local de nascimento. Os indicadores utilizados foram construídos a partir do Sistema de Informação sobre Nascidos Vivos (SINASC) em 1995 e da Pesquisa sobre Assistência Médico-Sanitária (AMS), com dados para 1992. Através de classificação multivariada foram identificados dois tipos de maternidades: um com grande número de partos cesáreos, boas condições da parturiente e recém-nato; e outro com maior proporção de partos espontâneos e indicadores que apontam riscos do recém-nascido. As proporções de: mães com escolaridade igual ou superior ao ensino médio, mães adolescentes e partos cesáreos são os indicadores que melhor caracterizaram os grupos. Identificou-se grande heterogeneidade na distribuição espacial das maternidades, concentrados nas regiões mais ricas da cidade, determinando, conseqüentemente, longos trajetos das gestantes na busca da assistência ao parto.

A sample survey was conducted in three public ambulatory medical care units in Rio de Janeiro in order to show the demographic and nosological aspects of the clientele as well as to evaluate the quality of clinic records. The sample included 2,029 patients registered in the health clinics and 3,980 consultations from October 1, 1990 to September 30, 1991. The diagnoses were coded according to the 9th revision of the International Classification of Diseases. Females accounted for about 60% of the sample, and more than a half were between 15 and 49 years of age. Children under 10 years old made up 28% of the total. About 38% of the patients came from the poor areas of the city: the "favelas". The case files were poorly filled out, frequently lacking useful information. The most frequent cause was classified in the XVIo chapter of the ICD -illdefined causes, followed by respiratory, infectious, genitourinary and circulatory diseases. External causes predominated in one of the health clinics, which is an emergency service. There was a great demand for preventive measures, such as prenatal care and child care, even at the clinic that was traditionally devoted only to curative medicine. The most frequent specific diagnoses were upper respiratory tract infection, hypertension, gynecological problems, prenatal care, and skin diseases. There were no records for referrals to other health services. The poor quality of the medical records causes problems in terms of quality of health care. Use of such record-keeping systems, even while criticizing them, is essential to improve them.
