Акутравма, вплив звуків високої інтенсивності викликає специфічне ураження слухової системи. При бойовій акутравмі страждають як периферичний, так і центральні відділи слухового аналізатора. Слід зважати також на системну дію шуму, звуків високої інтенсивності не лише на структури слухової системи, але і на увесь організм постраждалого. Тому при акутравмі поряд з ауральними (які стосуються стану різних відділів слухової системи) мають місце й виражені екстраауральні прояви (з боку органів і систем, не пов'язаних безпосередньо зі структурами слухової системи, але стан яких впливає на слухову функцію). Насамперед це стосується центральної нервової та серцево-судинної систем (мозкового кровообігу в найбільшій мірі). Тому доцільним є дослідження функціонального стану ЦНС у пацієнтів з акутравмою. Мета дослідження-визначити зміни в стані біоелектричної активності головного мозку за даними електроенцефалографії (ЕЕГ) у військовослужбовців, які отримали бойову акутравму в зоні проведення антитерористичної операції (АТО). Матеріали та методи дослідження. Ми відібрали для аналізу серед усіх обстежених нами хворих з акутравмою групу з 50 бійців з акутравмою з асиметричними сенсоневральними порушеннями та низхідним типом кривої. Методика виконання роботи передбачала проведення ЕЕГ за допомогою комп'ютерного електроенцефалографа фірми «DХ-системи». Електроди накладали таким чином, щоб рівномірно охоплювати лобні, скроневі та потиличні області обох півкуль, згідно зі схемою накладання електродів «10-20», рекомендованою Міжнародною федерацією електроенцефалографії. Результати та їх обговорення. За даними ЕЕГ у обстежених військовослужбовців з акутравмою, отриманою під час бойових дій, було виявлено іритативні зміни як при фоновому записі, так і при функціональних навантаженнях, особливо-гіпервентиляції, виражені в різному ступені. Частіше всього були виявлені: виражені дезорганізація та десинхронізація ритмів, тенденція до згладжування або відсутність зональних розбіжностей, наявність гострих піків і потенціалів, «прискорення» основних ритмів, переважання в структурі біоелектричної активності бета-ритму та значна частка повільних Δ-та θ-ритму. У частини хворих мало місце загальне зниження біоелектричної активності головного мозку, виражене в різному ступені, особливо в скроневих відведеннях. Ці зміни мали переважно загальномозковий (генералізований) характер і були більш вираженими у хворих з більш значними порушеннями у слуховому аналізаторі. У багатьох випадках спостерігалася асиметрія біотоків, переважно в задньолобних, скроневих і тім'яних відведеннях, особливо в тих випадках, коли в пацієнтів було асиметричне ураження слухової системи зі значною різницею в показниках двох сторін. Також часто в обстежених осіб з бойовою акутравмою спостерігалися спалахи гіперсинхронної активності, у тому числі з повільнохвильовими елементами, що свідчить про значне переважання процесів збудження в глиб...
; 2 Клініка отоларингології Військово-медичного клінічного центру південного регіону, Одеса Мета нашої роботи-визначення найбільш значущих показників біоелектричної активності головного мозку за результатами електроенцефалографії (ЕЕГ) у осіб, які отримали акутравму в зоні проведення бойових дій з урахуванням ступеня порушень слухової функції. Обстежено групу військовослужбовців з акутравмою (205 осіб) з найбільш характерними, типовими формами аудіометричних кривих-з низхідним, часто-обривчастим типом кривої, які були розподілені на три групи залежно від ступеня вираженості сенсоневральної приглухуватості. Дослідження здійснювали за допомогою комп'ютерного електроенцефалографа фірми «DХ-системи» (Україна) за загальноприйнятою методикою згідно зі схемою накладання електродів «10-20». Показано, що зміни біоелектричної активності головного мозку проявлялися деформацією домінуючого ритму з поганою модуляцією та ослабленою реакцією на функціональні навантаження, особливо в лобних та скроневих відведеннях. У обстежених достовірно зменшувався відсотковий вміст домінуючого у нормальній картині ЕЕГ α-ритму та збільшувалась представленість β-та Δ-ритму, як при фоновому записі, так і при проведенні функціональних навантажень-фотостимуляції та гіпервентиляції. Найбільш виражені зміни біоелектричної активності порівняно з контрольною групою спостерігали у осіб 2-ї (більш значні порушення слухової функції) та 3-ї (з вираженими порушеннями слухової функції) груп. Так, наприклад, у осіб 3-ї групи у фоновому записі збільшувався вміст Δ-ритму у лобних відведеннях до 19,74±1,53 % щодо 8,5±1,6 % у контрольній групі. Проведений нами порівняльний аналіз відсоткового вмісту ритмів ЕЕГ між досліджуваними групами свідчить про те, що від 1-ї до 3-ї групи збільшується представленість Δ, θ та β-ритму та знижується частка α-ритму як при фоновому записі, так і при функціональних навантаженнях. У 3-й групі в лобних відведеннях спостерігалася достовірна відмінність у значеннях відсоткового вмісту всіх досліджуваних основних ритмів ЕЕГ (α, β, θ, Δ) як при фоновому записі, так і при функціональних навантаженнях. Наприклад, відсоток Δ-ритму у лобних відведеннях при фоновому записі становив 19,47±1,53 %, що вірогідно відрізняється від показників 1-ї-13,69±0,68 % та 2-ї групи-13,04±0,52 %. Таким чином, у військослужбовців із бойовою акутравмою виявлено порушення біоелектичной активності головного мозку. Зі зниженням слухової функції у них відбувається перерозподіл основних ритмів ЕЕГ у напрямку зростання проявів повільно-хвильової активності на дезорганізованому фоні, особливо в лобних і скроневих відведеннях.
