Основную роль в возникновении аритмического синдрома при эпилепсии отводят дисфункции надсегментарных вегетативных образований. Асимметричное влияние левого и правого полушарий головного мозга на частоту сердечных сокращений, влияние противоэпилептических препаратов и других факторов на сердечный ритм все еще находятся в стадии обсуждения. Также обсуждается роль нарушений ритма сердца в патогенезе внезапной необъяснимой смерти при эпилепсии. Мы рассматриваем патогенетические механизмы нарушений ритма сердца при эпилепсии, факторы риска, их частоту возникновения, типы нарушений, роль противоэпилептической терапии и их значение в механизмах развития внезапной смерти больных эпилепсией (Sudden unexpected death in epilepsy, SUDEP). Резюме Цель-изучение группы взрослых пациентов с фокальной эпилепсией и установленными иктальными нарушениями сердечного ритма и проводимости с учетом нозологических, анамнестических, клинических, электроэнцефалографических и нейровизуализационных особенностей. Материалы и методы. В исследование было отобрано 70 пациентов в возрасте от 18 лет до 51 года с подтвержденным диагнозом фокальная эпилепсия с сохраняющимися на момент исследования эпилептическими приступами на фоне проводимой противоэпилептической терапии без сопутствующей кардиальной патологии с установленными иктальными изменениями сердечного ритма и проводимости. Результаты. Среди обследованных больных преобладали пациенты с диагнозом «структурная фокальная эпилепсия», длительным течением заболевания, с частыми эпилептическими приступами, с височной и лобно-височной локализацией эпилептогенного очага, принимающие несколько противоэпилептических препаратов. Наиболее часто нарушения сердечного ритма и проводимости возникали у мужчин, с серийным течением пароксизмов, с билатеральными тонико-клоническими приступами, с приступами, возникающими во время сна. Заключение. Своевременное выявление и обследование пациентов из групп высокого риска по иктальным аритмиям позволит предотвратить развитие нарушений ритма и проводимости сердца во время развития эпилептических приступов, в том числе жизнеугрожающего характера. GLUT 1 transporter deficiency syndrome (De Vivo syndrome, GLUT 1 deficiency syndrome, De Vivo Disease) is a rare genetic disease associated with a deficiency of the glucose transporter GLUT 1. Due to this deficiency, diffusion of glucose through the blood-brain barrier is limited or completely blocked. As a result, a clinical symptom complex of neurological disorders – epileptic seizures, cognitive deficit, and motor disorders – develops. The article provides a review of the literature on the variety of epilepsy manifestations in this syndrome, including idiopathic generalized epilepsy with absences, myoclonic-astatic epilepsy, and focal epilepsy. The inability of the basic antiepileptic therapy to stop the seizures is noted. The main treatment method is the ketogenic diet. A clinical observation made by these authors is also presented.
