AbstrakDiabetes Self Care (DSC) merupakan bagian dari pengelolaan diabetes. Tujuan penelitian ini adalah menganalisis hubungan DSC dengan score PEDIS (perfusion, extend, depth, infection, sensation) dan kontrol glikemik pasien Diabetes Melitus (DM). Penelitian ini merupakan cross sectional study yang melibatkan 106 pasien, dengan kriteria inklusi pasien DM tipe 1 dan 2 dengan atau tanpa ulkus kaki yang berumur <65tahun, dapat membaca, menulis dan berkomunikasi menggunakan bahasa Indonesia dengan baik. Penelitian dilakukan di Pusat Pelayanan Kesehatan Masyarakat Denpasar Barat II Bali Uji statistik yang digunakan adalah Mann Whitney dan Chi square. Statistik menunjukkan ada hubungan bermakna antara DSC dengan score PEDIS (p= 0,0005) dengan korelasi kuat (r= -0,74) dan kontrol glikemik (p= 0,0005) dengan korelasi sedang (0,45). DSC dapat digunakan untuk memprediksi kondisi kaki pasien diabetes melalui hasil score PEDIS dan dapat digunakan untuk memprediksi kontrol glikemik pasien diabetes. Menurut teori self care Orem, perawatan diri dipandang sebagai sumber daya kesehatan dalam individu. Strategi diabetes self care membutuhkan kesadaran dan tanggung jawab dalam mengelola diabetes untuk pengontrolan gula darah dan perawatan kaki yang adekuat. Kegiatan ini seharusnya dapat diarahkan tenaga keperawatan dengan cara pemberian informasi mengenai pengetahuan terkait penyakit diabetes dan perilaku pencegahan komplikasi.Kata kunci: diabetes self care, kontrol glikemik dan score PEDIS with PEDIS (perfusion, extend, depth, infection, sensation) score and glycemic control in diabetes patients. This study applied a cross sectional design, involving 106 patients, the inclusions are patient diabetes type 1 and 2 with or without diabetic foot ulcers who are < 65 years old, can read, write and use Bahasa. This research was conducted in the Community Health Center of West Denpasar II Bali. The statistic analyze used are Mann Whitney, and Chi square Statistics showed a significant association between DSC and PEDIS score (p= 0.0005) a strong correlation (r= -0.74) and glycemic control (p= 0.0005) a moderate correlation (0.451). DSC can be used predict the condition of diabetic foot ulcers through the PEDIS score and predict glycemic control in diabetes. In Orem's self-care theory, self-care is a health resource in individual. The strategy of diabetes self-care requires awareness and responsibility in managing diabetes for controlling blood sugar and adequate foot care. Introduction: Complications due to diabetes can prevented if patients able to independently handle the disease properly. According several studies, there is a significant relationship between diabetes self-management (DSM) and glycemic control. Diabetes Self Management education (DSME) is the provision of health education by experienced medical personnel about the daily behavior of diabetics such as self-monitoring blood glucose (SMBG), diet, health care, monitoring of medication and physical activity. Objective: To understood the education response in DM patient using video approached. Methods: The design of this study used a Research and Development approach with a quasi-experimental research design pretest-posttest with control group. The research began with the making of a DSME educational video which was carried out in three stages : exploration, development and validation stages. After the video was produced, further testing was carried out to determine the effectiveness of the video in increasing knowledge of DM. Result: The analysis showed that after the intervention there was an increase in knowledge in the treatment group from 10% to 43.3% while in the control group from 13.3% to 23.3%. Chi square results p value 0.023 (p <0.05), there was a significant difference between the experiment group and the control group after intervention. Conclusion: The DSME through audio-visual media can be more effective in increasing knowledge of diabetes. The use of video as a learning medium would help someone get better information because used of interesting features, authentic and real language. Audio visual as a learning medium made it easier for nurses in diabetes as educators.
ABSTRAKLatar Belakang: Diabetes self-care merupakan perilaku perawatan diri pasien diabetes yang meliputi pengaturan diet, penggunaan insulin, olahraga dan perawatan kaki. Diabetes self-care memiliki efek langsung pada kontrol glikemik dalam terjadinya ulkus kaki diabetic. Skor PEDIS merupakan form pemeriksaan yang digunakan perawat dalam menilai keparahan ulkus kaki. Keterbatasan intervensi ulkus kaki yang diberikan oleh perawat saat ini disebabkan oleh terbatasnya data tentang self-care pada pasien diabetes. Tujuan penelitian mengetahui self-care pasien diabetes dengan komplikasi ulkus kaki menggunakan metode pengukuran skor PEDIS yang teridiri dari pemeriksaan perfusi, luas luka, kedalaman luka, keberadaan infeksi dan sensasi kaki.Metode: Penelitian ini termasuk penelitian kuantitatif, dimana penentuan responden menggunakan teknik Convenience sampling dengan jumlah sampel adalah 125, analisa data yang digunakan dengan metode deskriptif.Hasil: Pada penelitian ini 53,8 % reponden memiliki diabetes self-care yang adekuat dan rata- rata responden memiliki Skor PEDIS 2,08.Kesimpulan: dalam penelitian ini sebagian besar responden memiliki diabetes self-care yang tidak adekuat. Nilai Diabetes self-care dapat digunakan oleh perawat untuk mengetahui tingkat kualitas perawatan diri pasien selama ini sehingga memudahkan dalam menentukan intervensi yang tepat untuk mencegah terajadinya perburukan ulkus kaki. ABSTRACTBackground: Diabetes self-care is a diabetes patient self-care behavior that includes diet management, insulin use, exercise and foot care. Diabetes self-care has a direct effect on glycemic control in the occurrence of diabetic foot ulcers. The PEDIS score is an examination form that nurses use in assessing the severity of foot ulcers. The limitations of the foot ulcer intervention given by nurses at this time are due to limited data on self-care in diabetic patients. The aim of this study was to determine the self-care of diabetic patients with complications of foot ulcers using the PEDIS score measurement method which consists of examination of perfusion, wound area, wound depth, presence of infection and foot sensation.Methods: This research is a quantitative study, where the determination of the respondents using the convenience sampling technique with a sample size of 125, the data analysis used is the descriptive method.Results: In this study 53.8% of respondents had adequate diabetes self-care and the mean of respondents had a PEDIS score of 2.08. Conclusion: in this study most of the respondents had inadequate self-care diabetes. The value of diabetes self-care can be used by nurses to determine the level of quality of patient self-care so far, making it easier to determine the right intervention to prevent worsening of foot ulcers.
