Introduction. Les anévrismes de la crosse aortique distale sont plus rares que ceux de l’aorte ascendante. Ils sont le plus souvent découverts à l'occasion d'une radiographie pulmonaire effectuée systématiquement ou pour une autre pathologie. Plus rarement, c'est une complication qui le révèle : soit compression, soit fissuration ou rupture. Le scanner et l'imagerie par résonance magnétique sont les examens de choix dans le diagnostic des anévrismes de la crosse aortique. Le traitement est chirurgical. Le but de ce travail est de monter un cas d’anévrisme de la crosse aortique distale englobant l’origine de l’artère sous clavière. Observation. Nous rapportons l’observation d’un patient âgé de 50 ans sans antécédents hospitalisé dans notre service à la suite de découvert fortuite lors d’un bilan d’une dysphonie d’un anévrisme de la crosse aortique. Préopératoire : Il était au stade II de la NYHA présentant une dysphonie depuis quelques mois. Radiographie pulmonaire : ICT à 0 ,60. ECG : RRS. Échocardiographie : Anévrisme de la drosse distale, insuffisance aortique grade II à valve souple. Per opératoire : Volumineux anévrisme de la crosse aortique distale prenant l’origine de l’artère sous clavière gauche. Angio-TDM thoracique : Anévrisme de la crosse 82/72 mm. Coronarographie : Fistule coronaro- auriculaire gauche. Le geste : Mise à plat de l’anévrisme, rétablissement de la continuité aortique par un tube en Dacron numéro 28 implanté en terminoterminale en hypothermie profonde et arrêt circulatoire et réimplantation de l’artère sous clavière dans la prothèse par l’intermédiaire d’un tube en dacron numéro 10. Les suites post opératoires étaient défavorables avec décès du patient en J 3 par défaillance circulatoire. Conclusion. Les anévrismes de la crosse aortique sont rares, d’étiologies diverses et posent des problèmes de technique chirurgicale très spécifiques. L’abord chirurgical de la crosse est très difficile.
Introduction: Rheumatic heart diseases are redoubtable complication of acute articular rheumatic. Without treatment, pulmonary hypertension and right heart failure may occur. This pulmonary hypertension was admitted as predictor of worse outcome after surgery. The aim of our study is to value the results after mitral surgery in patients with moderate and severe pulmonary hypertension.
Introduction : La dextrocardie est une malformation congénitale dans laquelle le coeur est placé dans la moitié droite du thorax, l'apex orienté vers la droite (1/10 000 naissances), accompagnée ou non d'une inversion des organes (Situs inversus). Le but de ce travail est de rapporter un cas de rétrécissent mitral serré chez une malade en dextrocardie. Observation : Nous rapportons l’observation d’une femme âgée de 46 ans présentant un rétrécissement mitral diagnostiqué il y’a un mois suite à une dyspnée d’aggravation progressive. L’examen physique était sans particularités à part le roulement diastolique au foyer mitral. Radiographie pulmonaire : Dextrocardie avec un ICT a 0,55. ECG : RRS. Echocardiographie : rétrécissement mitral serré avec surface mitrale a 1,12 cm2 sur valve mitrale épaissie, VG : 47/33 mm, VD : 32 mm, FE : 55 %, FR : 26 %, PAPS : 58 mm hg. L’exploration per opératoire objective une dextrocardie avec valve mitrale remaniée. Le geste avait consisté en un remplacement valvulaire mitral. Les suites postopératoires étaient simples. Conclusion : La dextrocardie est une malformation congénitale. Elle peut être isolée ou s’associer à d’autres cardiopathies congénitales ou acquises.
Introduction. La fermeture percutanée du canal artériel persistant est actuellement la méthode de choix ; cependant, certaines conditions socio-économiques défavorables font que la chirurgie reste le seul moyen thérapeutique. Objectif. Evaluer la place de la chirurgie par rapport à la fermeture percutanée. Matériel d’étude. De 2001 à 2018, 254 patients ont été opérés à l’EHS Djeghri Mokhtar. L’Age a varié entre 1 et 48 ans. Le sexe féminin est prédominant. La voie d’abord avait consisté en une thoracotomie postéro-latérale gauche chez 252 patients et une sternotomie chez 2 patients. Résultats. Le traitement a consisté en une section – suture du canal chez 244 patients et une ligature chez 10 patients. La durée d’hospitalisation était 9,5 jours. La mortalité précoce et tardive est nulle. Discussion. Au cours des 20 dernières années, la fermeture percutanée est devenue la principale approche du traitement des canaux artériels. Cependant, tous les patients ne sont pas éligibles à ce type de traitement. En présence d’une forte hypertension artérielle pulmonaire, beaucoup d’auteurs sont en faveur de la chirurgie, le risque d'embolisation du matériel étant très élevé si la fermeture percutanée est envisagée. Chez les nouveau-nés prématurés, la stratégie thérapeutique actuelle est une ligature chirurgicale par thoracotomie, la fermeture percutanée a été récemment proposée comme une alternative à la chirurgie afin d’éviter ainsi la thoracotomie. Conclusion. La fermeture percutanée des canaux artériels a éclipsé la chirurgie, cependant, les conditions socio-économiques défavorables rendent ce procédé souvent illusoire et par conséquent, la chirurgie du canal artériel garde toujours sa place.
Introduction. Les endocardites non infectieuses regroupent des pathologies rares, souvent sévères de diagnostic et de prise en charge difficiles. Le but de cet exposé est de rapporter le cas d’un patient présentant une endocardite non infectieuse survenant dans le cadre d’une maladie de Behçet. Observation. Monsieur Y.A âgé de 45 ans, opéré en novembre 2016 de sa valvulopathie aortique bénéficiant d’un remplacement valvulaire aortique par une prothèse mécanique. Le patient a été réopéré neuf mois après, suite à une désinsertion de sa prothèse sur une endocardite infectieuse probable ; repris dix mois après pour re-désinsersion de cette prothèse avec dilatation anévrismale du culot aortique bénéficiant d’une intervention de Bentall modifiée. Les suites opératoires étaient simples, le patient a été mis sous triple antibiothérapie. Devant la culture négative de la prothèse explantée, la recherche d’une endocardite à germes intracellulaires a été lancée revenant toujours négative. Le diagnostic de maladie de Behçet a été évoqué devant les hémocultures négatives, la notion d’aphtose bipolaire, la désinsertion prothétique dans un contexte aseptique et la dilatation anévrismale de l’aorte dans les suites opératoires. Le patient a été transféré en médecine interne pour complément d’investigations et prise en charge thérapeutique à base d’immunosupresseurs. Conclusion. L’endocardite non infectieuse est une situation clinique de diagnostic difficile en dépit de l’amélioration de techniques d’imagerie et de prise en charge chirurgicale. Le médecin interniste joue un rôle primordial pour guider les investigations. Le pronostic demeure réservé en raison du risque élevé de récurrence des lésions.
