A aprovação da nova LDB, em 1996, levou à elaboração de Diretrizes Curriculares Nacionais (DCN) baseadas em competências necessárias para os médicos atuarem frente às novas tecnologias e para enfrentarem os desafios do desenvolvimento do SUS. Analisar a implantação das DCN no Paraná, verificando as características da formação dos profissionais de Medicina, foi o objetivo desta publicação específica, que faz parte dos dados coletados numa pesquisa mais ampla, apoiada pelo edital universal do CNPq (Proc. 474.029/2003-04). O estudo foi realizado com uma abordagem qualitativa e análise de conteúdo. Fez-se um levantamento das Instituições de Ensino Superior (IES) que oferecem cursos de Medicina, coletando-se os dados por questionário e pelas informações dos Projetos Político-Pedagógicos (PPP) obtidos, com instrumento baseado em sete eixos das DCN. Dois cursos apresentam grau avançado de implantação das DCN, um mostra iniciativa da implantação de alguns aspectos, e dois outros seguem formatos curriculares bem tradicionais. Os dados obtidos sugerem a existência de uma motivação para a inovação do ensino médico no Paraná, o que se traduz em currículos idealizados com vários princípios inspirados nas DCN.
Visando compreender como as dimensões organizacional, política e relacional afetam o cotidiano do trabalho gerencial em Unidades Básicas de Saúde de municípios de pequeno porte do norte do Paraná, realizou-se estudo qualitativo cujos dados foram obtidos por meio de grupos focais. O enfermeiro é o que mais desenvolve a função gerencial, porém, informalmente. O modelo gerencial verticalizado e autoritário predomina, e há fragilidade no exercício frente à autonomia e ao corporativismo de profissionais. Na dimensão política, interesses político-partidários sobrepõem-se às decisões gerenciais; e, na dimensão relacional, há manifestação de conflito, com desrespeito e disputas, mas, também, colaboração, com diálogo e trabalho em equipe. Nesses municípios, os processos gerenciais são pouco desenvolvidos, necessitando de profissionalização e oficialização do cargo, além do desenvolvimento de educação permanente.
A política de Educação Permanente em Saúde (EPS) destina-se ao desenvolvimento dos trabalhadores da saúde. Pretendendo analisar o processo de implantação e desenvolvimento da política no Paraná, uma pesquisa qualitativa, envolvendo as seis regiões do estado, está sendo concluída. Este artigo refere-se aos primeiros resultados da região norte, focalizando a categoria "vivenciando a EPS". Em dezembro de 2006 realizaram-se dois grupos focais, envolvendo representantes da gestão, formação, atenção e participação. Os dados foram submetidos a análise temática de conteúdo. Nas primeiras aproximações com EPS surgiram sentimentos de desconfiança e resistência e o polo foi compreendido como meio de viabilizar cursos e fonte de financiamento. Observaram-se diversidade de interesses e pouca capacidade de negociação. No transcorrer do processo, os integrantes do estudo começaram a conversar, refletir e participar. Experimentaram positivamente o trabalho em equipe. Esta vivência permitiu reconhecer a potencialidade da EPS em articular e mobilizar diferentes atores.
Inter-municipal health consortia emerged in Brazil's Unified National Health System (SUS) policy in the late 1980s. Municipal health administrators adhered to this strategy with the aim of upgrading health services supplied to the population. This research analyzes the profile of such consortia in Paraná State, focusing on specialized medical care. Data were obtained from reports by the State Health Council and questionnaires sent to all 20 existing municipal health consortia. Governmental Decree no. 1,101 and data published in 2000 on the profile of the health system in Paraná were used as references. Of the 399 municipalities in Paraná State, 81.5% have joined municipal consortia. Specialists are allocated by municipalities (4.4%), the State government (13.6%), or Federal Government (12.8%); another 69.2% are hired by the consortia themselves. The supply of consultations with specialists is either insufficient or inadequately distributed, and there are flaws in the referral and counter-referral system. Municipal health consortia serve as viable instrument for expanding and increasing the capacity of municipalities to supply specialized care, although there is a need for well-defined criteria, planning, and improving of the referral and counter-referral system.
Busca-se compreender o processo de implantação, desenvolvimento e sustentabilidade da Política Nacional de Educação Permanente em Saúde (EPS) no Paraná. Os dados são de nove grupos focais realizados em municípios das macrorregiões do Paraná. Participaram 77 sujeitos oriundos dos Polos Regionais de Educação Permanente em Saúde (Preps). A implantação descentralizada de 22 Preps permitiu maior capilaridade e valorização dos espaços locorregionais. As instituições de ensino tiveram papel relevante. Houve ampla articulação em torno da formação e do desenvolvimento dos trabalhadores da saúde, com destaque para o curso de formação de facilitadores em EPS. O processo vivenciado esteve permeado por dificuldades/facilidades. Comprometimento dos gestores com a política, a prática da avaliação e a mudança do modelo gerencial verticalizado foram apontados como condições para a sustentabilidade da EPS. Os modos operativos da política foram praticados/experimentados de forma única no Paraná. Como idealizada a EPS precisa ser uma política e não um programa de governo.
