SamenvattingDoelstelling: onderzoeken van de effectiviteit van ademhalingsrevalidatie ter voorkoming van postoperatieve longcomplicaties (PLC) en definie¨ren welke patie¨nten er baat bij kunnen hebben.Ontwerp: een willekeurige klinische toets. Omgeving: een algemeen ziekenhuis.Patie¨nten: eenentachtig patie¨nten die een hoge buikoperatie hadden ondergaan werden verdeeld in twee homogene groepen: een controlegroep (n = 41) en een therapiegroep (n = 40).Therapie: ademhalingsoefeningen in de therapiegroep. Voornaamste uitkomstmetingen: preoperatieve en postoperatieve klinische evaluatie, spirometrie, arterie¨le gasmeting, en primaire ro¨ntgenfoto's van de thorax.Resultaten: de incidentie van PLC was 7,5% in de therapiegroep en 19,5% in de controlegroep; de controlegroep bevatte ook meer radiologische afwijkingen (p = 0,01). Een gelaagde PLC-analyse toonde geen significante verschillen tussen de groepen aan. Maar de patie¨nten in de therapiegroep met een hoog of matig risico hadden minder PLC. Een multivariantieanalyse toonde een groter PLC-risico aan, samenhangend met een pulmonale voorgeschiedenis (p = 0,02) en met een operatieduur langer dan 120 minuten (p = 0,03), terwijl therapie een beschermend effect bewerkstelligde (p = 0,06). Een significante postoperatieve afname in longvolume en in arterie¨le gaswaarden werd in beide groepen geregistreerd, zonder significante verschillen.Conclusies: ademhalingstherapie beschermt tegen PLC en is effectiever bij patie¨nten met een hoog of matig risico, maar heeft geen invloed op functionele veranderingen die door de operatie zijn veroorzaakt.
InleidingDe longfunctie is na een operatie meestal veranderd, vooral bij patie¨nten die een borst-of hoge buikoperatie hebben ondergaan. De waargenomen fysiologische veranderingen hangen direct samen met de anesthesie (algehele of gedeeltelijke) en met het soort incisie en de chirurgische techniek die wordt toegepast en worden weergegeven in de vorm van vermindering van de totale longcapaciteit en longvolumes en door een parallelle vermindering van de PaO 2 .Deze functionele veranderingen spelen op hun beurt een rol bij de ontwikkeling van postoperatieve longcomplicaties (PLC), die de meest voorkomende oorzaken vormen van morbiditeit en mortaliteit onder longoperatiepatie¨n-ten. De gemelde frequentie van PLC is zeer variabel, met een spreiding van 6% tot 76% afhankelijk van de manier waarop complicaties en de methoden om ze te evalueren zijn gedefinieerd, alsmede de manier waarop de onderzoeken gecontroleerd zijn.Ademhalingsoefeningen worden algemeen toegepast om het longvolume te vergroten en de gaswisseling en ventilatiespreiding te verbeteren.
