Abstract. An uncommon disseminated Mycobacterium tuberculosis infection is described in a 12-year-old female dog presenting with fever, dyspnea, cough, weight loss, lymphadenopathy, melena, epistaxis, and emesis. The dog had a history of close contact with its owner, who died of pulmonary tuberculosis. Radiographic examination revealed diffuse radio-opaque images in both lung lobes, diffuse visible masses in abdominal organs, and hilar and mesenteric lymphadenopathy. Bronchial washing samples and feces were negative for acid-fast organisms. Polymerase chain reaction (PCR)-based species identification of bronchial washing samples, feces, and urine revealed M. tuberculosis using PCR-restriction enzyme pattern analysis-PRA. Because of public health concerns, which were worsened by the physical condition of the dog, euthanasia of the animal was recommended. Rough and tough colonies suggestive of M. tuberculosis were observed after microbiological culture of lung, liver, spleen, heart, and lymph node fragments in Lö wenstein-Jensen and Stonebrink media. The PRA analysis enabled diagnosis of M. tuberculosis strains isolated from organs.
ResumoO objetivo do presente trabalho foi avaliar os prontuários dos pacientes cuja cultura micológica do pêlo, em meio Ágar Sabouraud dextrose, suplementado com cloranfenicol e ciclohexamida, foi positiva para dermatófitos, e realizar um levantamento da casuística de dermatofitose. Foram avaliados 136 prontuários de pacientes (114 caninos e 22 felinos) com dermatofitose atendidos em período de 54 meses, no Hospital Veterinário da UNESP de Botucatu. Pela análise dos resultados, observou-se que a maioria dos cultivos foi positiva para M. canis. Não foi observada diferença estatística quanto ao sexo dos animais. Os cães de raça definida foram mais acometidos. A maior parte dos felinos era sem raça definida. A maioria dos humanos e animais que mantinham contato com os pacientes não apresentava lesões de pele. 32,5% dos cães apresentavam prurido de intensidade moderada, enquanto nos gatos o prurido estava ausente em 77,3% dos casos. 69,3% dos animais não apresentavam sinais clínicos não dermatológicos. A média aproximada de idades nos caninos foi de quatro anos e de três anos nos felinos. Não houve diferença estatística para a ocorrência da dermatofitose nas diferentes estações do ano. Dos animais avaliados com a Lâmpada de Wood, 40,9% dos cães e 33,3% dos gatos foram positivos. A maior parte dos cães apresentava lesões generalizadas, enquanto na maioria dos gatos eram localizadas. As lesões mais comumente observadas foram: alopecia, crostas e eritema. Bras. 34(5):449-454, maio 2014 449 RESUMO.-O estudo tem o objetivo de identificar efeitos indesejáveis da ribavirina, prednisona e DMSO em cães naturalmente infectados com o vírus da cinomose. Foram utilizados 60 cães apresentando quadro neurológico da cinomose com evolução de 10 dias. Os animais foram internados e receberam tratamento de suporte; foram avaliados diariamente e realizados hemograma, dosagem bioquímica e exame de urina tipo I. Os grupos 1 e 2 foram tratados com ribavirina e sua associação com DMSO; os grupos 3 e 4 com DMSO e prednisona e o grupos 5 com ribavirina e prednisona e o grupo 6 com ribavirina, prednisona e DMSO. Os animais foram anestesiados para a colheita de líquor, medula óssea e sangue, antes do tratamento para diagnóstico através da RT--PCR. As amostras negativas foram analisadas pela técnica de hn-PCR. Todos os animais apresentaram resultado positivo em pelo menos uma das duas reações. O efeito adverso da ribavirina e a sua associação com a prednisona foi a anemia hemolítica, que foi confirmada pela observação de bilirrubina na urina apenas dos cães tratados com ribavirina. The present study aims at the identification of undesirable effects of ribavirin, predinisone and DMSO in dogs naturally infected by canine distemper virus. The research analyzed 60 dogs with clinical neurological signs and 10 days of evolution. The animals were hospitalized for the appropriate support treatment; were daily observed, and complete blood cells count, biochemical analysis, and urine exam type I were conducted. Groups 1 and 2 were treated with ribavirin and its combination with DMSO; Groups 3 and 4 treated with prednisone and DMSO, Group 5 treated with ribavirin and prednisone, while Group 6 with ribavirin, prednisone and DMSO. Before the treatment, animals were anesthetized for the cerebrospinal fluid, bone marrow and blood samples collection for the diagnosis based on RT-PCR. The negative samples were analyzed using the hn-PCR technique. All the animals presented positive results in at least one of the 2 tests. The adverse result of ribavirin and its association with prednisone was characterized by haemolytic anemia, confirmed by the evaluation of bilirrubin occurrence only in the urine of dogs treated with ribavirin.
A cinomose é uma enfermidade infecto-contagiosa, que afeta naturalmente os cães, causada por um Paramyxovirus, do gênero Morbilivirus, de apresentação clínica aguda, subaguda e crônica. De ocorrência mundial, sem sazonalidade, sem predileção de sexo ou raça, apresenta maior incidência em animais jovens, podendo acometer todas as idades. A transmissão ocorre principalmente por aerossóis e gotículas contaminadas. Após o contato do vírus com o epitélio ocorre replicação viral nos macrófagos e disseminação. Entre quatro e seis dias ocorre nova replicação causando aumento inicial na temperatura corporal e leucopenia. No período de 8 a 10 dias, ocorre disseminação viral nos epitélios e no sistema nervoso central. A encefalite aguda acomete animais jovens ou imunossuprimidos e é caracterizada por lesão viral direta. A encefalite multifocal em adultos acomete freqüentemente animais entre quatro a seis anos, apresentando curso crônico. A encefalite do cão velho, comum em cães com mais de seis anos, caracteriza-se por panencefalite e é uma forma rara, resultante da persistência viral nos neurônios. As lesões desmielinizantes clássicas iniciam três semanas pós-infecção e durante período de intensa imunossupressão, sendo conseqüência da replicação viral no interior de oligodentrócitos. A desmielinização na fase crônica coincide com a recuperação do sistema imune, caracterizada pela redução ou perda das proteínas virais, com aumento da regulação do complexo principal de histocompatibilidade e infiltração de células inflamatórias. O curso clínico e o padrão neurológico da encefalomielite variam de acordo com a cepa viral e a idade que o animal foi infectado. O líquor pode indicar anormalidades na fase crônica com aumento de proteínas, principalmente por IgG antivírus e aumento do número de células, com predomínio de linfócitos. Em cortes histológicos é possível confirmar a infecção com a observação de lesões caracterizadas por áreas de necrose bem delimitadas, desmielinização e corpúsculos de inclusão. O isolamento viral em cultivo celular é específico, demorado e pode resultar em falso-negativo, exceto na fase aguda. A técnica da reação em cadeia pela polimerase, precedida de transcrição reversa vem sendo empregada com sucesso na detecção viral. O diagnóstico pode ser confirmado pela técnica de imunofluorescência direta em fluídos corporais, porém o aumento dos títulos de IgM e IgG no soro é ambíguo e pode indicar infecções passadas e presentes com ou sem vacinação. Cães infectados podem apresentar imunossupressão, permitindo a infecção por agentes oportunistas, como o Toxoplasma gondii. Vacinas produzidas com as amostras virais adequadamente atenuadas são eficientes em proteger os animais contra a infecção natural.
