Background: Diabetes has becoming an increasing cause of morbidity and mortality worldwide. Along with the rising prevalence of diabetes increase in the complications are expected which will further burden our health care services. Poor knowledge of foot care and poor foot care practices were identified as important risk factors for foot problems in diabetes.Methods: The present study was a mixed methods study conducted among diabetic patients attending medicine OPD. Quantitative data for foot care practices and peripheral neuropathy was collected from 190 diabetic patients and free listing among 20 diabetic patients was done to identify perceived barriers for foot care practices. Collected data were entered in Epi Info (3.5.3) and analysed using SPSS version 24 software.Results: The prevalence of peripheral neuropathy among the study participants was 52.9%. Significant association was found between peripheral neuropathy and male sex (p=0.006), occupation (p=0.003), smoking status (p=0.013) and longer duration of disease (p=0.04). The various reasons for poor foot care practices perceived by patients were poor knowledge about foot care, lack of knowledge about complications and health care provider did not teach them.Conclusions: The prevalence of peripheral neuropathy is common among diabetic patients and most of them are having poor foot care practices so there is a need in the community to lay emphasis on health education programs to improve foot care practices.
Laparoscopic surgery is normally performed under general anesthesia, but regional techniques like thoracic epidural and lumbar spinal have been emerging and found beneficial. We performed a clinical case study of segmental thoracic spinal anaesthesia in a healthy patient. We selected an ASA grade I patient undergoing elective laparoscopic cholecystectomy and gave spinal anesthetic in T10-11 interspace using 1 ml of bupivacaine 5 mg ml−1 mixed with 0.5 ml of fentanyl 50 μg ml−1. Other drugs were only given (systemically) to manage patient anxiety, pain, nausea, hypotension, or pruritus during or after surgery. The patient was reviewed 2 days postoperatively in ward. The thoracic spinal anesthetia was performed easily in the patient. Some discomfort which was readily treated with 1mg midazolam and 20 mg ketamine intravenously. There was no neurological deficit and hemodynamic parameters were in normal range intra and post-operatively and recovery was uneventful. We used a narrow gauze (26G) spinal needle which minimized the trauma to the patient and the chances of PDPH, which was more if 16 or 18G epidural needle had been used and could have increased further if there have been accidental dura puncture. Also using spinal anesthesia was economical although it should be done cautiously as we are giving spinal anesthesia above the level of termination of spinal cord.
Objective: The 2, 4-thiazolidinedione containing compounds could lead to most promising scaffolds with higher efficiency toward the targets recognized for its antidiabetic activity when combined with azaglycine moiety. The objective of the present work was to merge functionalized aza glycines with 2, 4-thiazolidinediones, perform in silico evaluation by molecular properties prediction and undertake the molecular docking studies with targets relevant to diabetes, bacterial and viral infections using Swiss Dock programme for unraveling the target identification which can be used for further designing.
Methods: (i)In silico studies were performed using Molinspiration online tool, Swiss ADME website and Swiss Target Prediction websites to compute the physicochemical descriptors, oral bioavailability and brain penetration. (ii) Molecular docking studies were performed using Swiss Dock web service for enumeration of binding affinities and assess their biological potentiality.
Results:The results predicted good drug likeness, solubility, permeability and oral bioavailability for the compounds. All the compounds showed good docking scores as compared to the reference drugs. The N-oleoyl functionalized aza glycine derivative demonstrated superior binding properties towards all the studied target reference proteins, suggesting its significance in pharmacological actions.
Conclusion:The binding interactions observed in the molecular docking studies suggest good binding affinity of the oleoyl functionalized aza glycine derivative, indicating that this derivative would be a promising lead for further investigations of anti-viral, anti-inflammatory and antidiabetic activities.
