Background Myeloproliferative neoplasms are a group of diseases with diverse biological and clinical characteristics. As a provisional separate entity, myeloid/lymphoid neoplasms with eosinophilia and genetic rearrangement have been described, which may present an initial clinical behavior of myeloproliferation and be characterized by varied genetic rearrangements. One of these entities is associated with FGFR1 rearrangements, characterized by its low prevalence and few treatment options. Case presentation We present the case of a 53-year-old Mestizo male patient of Hispanic origin who initially presented weight loss and fatigue, with a complete blood count showing leukocytosis and eosinophilia, with an initial diagnosis of nonspecific myeloproliferative disorder. In a next-generation sequencing study, BCR::FGFR1 rearrangement was documented, a diagnosis of myeloid/lymphoid neoplasia with eosinophilia and BCR::FGFR1 rearrangement was made, and hydroxyurea therapy was initiated. Subsequently, transformation to cortical T-lymphoblastic leukemia/lymphoma and erythroid precursors was documented, requiring management with chemotherapy. Conclusions Myeloid/lymphoid neoplasms with eosinophilia and genetic rearrangements constitute a group of deeply heterogeneous diseases with variable clinical and diagnostic characteristics and whose treatment is not clearly defined.
Caso: presentamos el caso de un paciente masculino de 32 años con historia de tumor de células germinales testicular (TCG) tratado con orquidectomía derecha; la histopatología mostraba tumor mixto de células germinales maligno compuesto por seminoma con células de sincitiotrofoblasto, sin invasión linfovascular y márgenes de resección sin compromiso neoplásico. Cinco meses después consulta por cuadro de dolor en fosa ilíaca derecha asociado a masa abdominal ipsilateral; se evalúan marcadores tumorales con AFP 10151 ng/ml, ß-hCG 5388 mU/ml y LDH 2971 U/L, tomografía de abdomen y tórax que revela gran masa lobulada irregular en el retroperitoneo de 18x11x10cm que oblitera la vena iliaca común, iliaca externa derecha y hidroureteronefrosis derecha severa. Se considera TCG no seminomatoso pT2N2M1b S3 estadio IIIC con pobre pronóstico según IGCCCG (Internacional germ cell cancer collaborative group) se inicia protocolo BEP (etopósido, cisplatino y bleomicina), tras tres ciclos debuta con dolor intenso en fosa ilíaca derecha. Se realiza tomografía de abdomen evidenciando perforación del íleon distal con colecciones pericecales, es llevado a laparotomía exploratoria, encontrándose peritonitis focal de cuatro cuadrantes con gangrena de apéndice cecal y plastrón de epiplón contiguo, realizándose apendicetomía.
Objetivos: describir el caso de un paciente con diagnóstico de neoplasia mieloide/linfoide con reordenamientos del gen FGFR1, que se presenta como mielofibrosis secundaria. Presentación del caso: paciente masculino de 50 años sin antecedentes patológicos con cuadro clínico de un año de astenia, adinamia y pérdida de peso. Previamente había consultado a un centro de atención primaria en donde se realizaron estudios, identificando leucocitosis de 40.000 mm3, hemoglobina en 12.6 g/dl y plaquetas en 120.000 mm3, además presencia de formas mieloides aumentadas en extendido de sangre periférica, sin detectar blastos. Otros estudios realizados incluían aspirado y biopsia de medula ósea con hallazgos consistentes con hiperplasia mieloide además de leve incremento de la trama reticular grado 1. El Cariotipo era 46XY y no se detectó BCR ABL, JAK2 V617F, mutación gen CALR (exón 9) o mutación gen MPL1 (exón 10). Se había considerado diagnóstico de neoplasia mieloproliferativa crónica con un patrón histológico inespecífico iniciándose manejo con hidroxiurea, la cual no fue tolerada por trombocitopenia severa, continuando vigilancia sin tratamiento. En una evaluación posterior, el paciente se encontraba asintomático, sin adenopatías, hepatomegalia o esplenomegalia, en el hemograma presentaba poliglobulia con hemoglobina de 19.6 g/L y hematocrito de 60 %, trombocitopenia con 54.000 x uL plaquetas y leucocitos en 9.600 x uL con monocitos en 1.540 x uL (Tabla 1). Se realizó un nuevo aspirado de médula ósea el cual resulto en “aspirado seco”. En la biopsia de medula ósea se identificó una celularidad del 100 % con una relación mieloide-eritroide 2:1; la serie mieloide presentaba zonas con freno madurativo y otras en menor proporción con formas maduras e incremento de eosinófilos y, adicionalmente, presencia de fibrosis grado III (Figura 1). Otros estudios reportan niveles de eritropoyetina <0.6 mUI/mL (VR 2.5-18mUI/mL) y LDH 683 UI/L (VR 230-460 UI/L). Se solicita estudio de secuenciación de próxima generación para neoplasias mieloides en sangre periférica identificando t(8;22)(p11;q11) BCR/FGFR1. Por lo anterior se define que el paciente cursa con una neoplasia mieloide/linfoide con rearreglos del FGFR1, presentándose como una mielofibrosis secundaria avanzada. Se indicó manejo citorreductor con hidroxiurea y flebotomías a necesidad presentado una evolución clínica satisfactoria, sin deterioro en su condición general luego de un año de tratamiento; sin embargo, ha persistido con leucocitosis de 39.040 x uL, neutrófilos de 17.700 x uL, monocitos de 8940 x uL y eosinófilos de 1.680, además de trombocitopenia con recuento de plaquetas en 55.000 x uL (Tabla 1). Se realizó búsqueda de donante de medula ósea identificando dos posibles donantes haploidénticos emparentados, con miras a realizar trasplante alogénico de medula ósea en caso de que se presente una evolución a leucemia aguda. Conclusiones: neoplasia mieloide/linfoide con reordenamientos del gen FGFR1, previamente conocida como síndrome mieloproliferativo 8p11, es una enfermedad con manifestaciones heterogéneas que debido a su rareza sigue siendo poco conocida. Suele manifestarse como un síndrome mieloproliferativo y en la mayoría de los casos presenta transformación rápida a un linfoma linfoblástico, leucemia linfoblástica aguda o leucemia mieloide aguda. El pronóstico es pobre siendo el trasplante alogénico de médula ósea la única opción curativa.
Resumen Introducción: La heterotopia de mucosa gástrica, hace referencia a la localización ectópica de mucosa gástrica en cualquier parte del tracto gastrointestinal. Es una causa poco frecuente de úlceras gastrointestinales y sangrado digestivo oculto. La videocápsula endoscópica se ha convertido en una herramienta fundamental para el estudio del intestino delgado. Métodos: Estudio descriptivo de reporte de caso. La información de la historia clínica, reporte de patología y estudios endoscópicos, se extrajo desde las bases de datos de la Fundación Valle del Lili. Resultados: Paciente masculino de 71 años, en quien se realizó videocápsula endoscópica por sangrado digestivo oculto y se encontraron segmentos de estenosis y úlceras en yeyuno. La histopatología reveló la presencia de mucosa gástrica heterotópica. Conclusión: La heterotopia de mucosa gástrica debe ser pensada como causa de posible sangrado de intestino delgado.
