Anal and urinary dysfunctions are usually associated and have a great impact on a woman's QoL. An integrated approach across specialties should lead to improved patient care. Therefore, our study is relevant because it emphasizes the importance of urogynecologists routinely investigating such symptoms. To do so, standardized questionnaires should be included in the evaluation of all these patients.
Objective To examine women with pelvic floor dysfunction (PFDs) and identify factors associated with sexual activity (SA) status that impacts quality of life (QoL). Methods We conducted a cross-sectional study that included women > 18 years old who presented with at least one PFD symptom (urinary incontinence [UI] and/or pelvic organ prolapse [POP]), in outpatient clinics specializing in urogynecology and PFD in Fortaleza, state of Ceará, Brazil, using a service evaluation form and QoL questionnaires. Results The analysis of 659 women with PFD included 286 SA (43.4%) women and 373 non-sexually active (NSA) (56.6%) women, with a mean age of 54.7 (±12) years old. The results revealed that age (odds ratio [OR] = 1.07, 95% confidence interval [CI] 1.03–1.12) and post-menopausal status (OR = 2.28, 95% CI 1.08–4.8) were negatively associated with SA. Being married (OR = 0.43, 95% CI 0.21–0.88) was associated with SA. Pelvic organ prolapse (OR = 1.16, 95% CI 0.81–1.68) and UI (OR = 0.17, 95% CI 0.08–0.36) did not prevent SA. SF-36 Health Survey results indicated that only the domain functional capacity was significantly worse in NSA women (p = 0.012). Two King's Health Questionnaire domains in NSA women, impact of UI (p = 0.005) and personal relationships (p < 0.001), were significantly associated factors. Data from the Prolapse Quality-of-life Questionnaire indicated that NSA women exhibited compromised QoL. Conclusion Postmenopausal status and age negatively affected SA. Being married facilitated SA. Presence of POP and UI did not affect SA. However, NSA women with POP exhibited compromised QoL.
A taxa de incidência do transtorno da dor sexual feminina/vaginismo varia de 11,7% a 42% entre mulheres que apresentam disfunção sexual. Objetivo: descrever implicações do vaginismo no cotidiano das mulheres. Metodologia: estudo de campo, descritivo, transversal e quantitativo, realizado no Grupo de Apoio às Mulheres com Vaginismo, no período de dezembro de 2016 a março de 2017, composto por 51 mulheres diagnosticadas com vaginismo primário. Foram enviados questionários para serem autoaplicados, contendo aspectos biopsicossociais e da função sexual. Resultados: as participantes (n = 51) eram predominantemente da faixa etária de 29 a 39 anos (51%), evangélicas (29%) e de religião não específica (29%), nível superior completo (35%), casadas/em união estável (55%). A maioria associou o vaginismo à educação rígida. Em relação à abordagem profissional, 68% estavam sendo acompanhadas por ginecologistas, 57% por fisioterapeutas pélvicos, 35% estavam em fase de diagnóstico e 37% em tratamento. O intervalo entre as primeiras queixas e o diagnóstico foi de até um ano (45%). A maioria (53%) encontrava-se também insatisfeita com o intervalo entre tratamento e cura. O vaginismo foi associado à baixa autoestima em 47%, e 70% relataram o incentivo do parceiro ao tratamento. A média do total de escore do Female Sexual Function Index foi de 21 (± 8). Conclusão: as mulheres deste estudo apresentaram baixa função sexual, referiram que o vaginismo afeta sua autoestima e relacionaram a disfunção à educação rígida. Também foi identificada a necessidade de uma abordagem profissional mais efetiva, para reduzir o tempo entre as primeiras queixas e a cura.
Objetivou-se avaliar o perfil sócio-demográfico e clínico das pacientes atendidas em um ambulatório de uroginecologia de uma instituição pública de referência em Fortaleza, Ceará. Trata-se de um estudo transversal cuja amostra foi composta por 85 mulheres com disfunção do assoalho pélvico atendidas entre julho/2011 a maio/2012. Os dados foram analisados por meio programa estatístico SPSS 17.0. A idade variou de 27 a 86 anos (M:54,4±14,6). As profissões mais comuns foram: do lar (21,9%), aposentada (18,8%) e doméstica (13,5%). A maioria das pacientes (51%) era casada ou com união estável. Das que estavam na pós-menopausa, o tempo de amenorreia foi, em média, 16,8 anos (±10,7 anos), e apenas 3,1% estavam em uso de terapia hormonal. O número médio de gestações foi de 4,7±2,9 (0-15), e o número médio de partos vaginais foi de 3,7±2,9 (0-15). O peso do maior recém-nascido variou de 2.075g a 6.900g (média: 3.760g ±778,2). A maioria das pacientes (83,3%) não era tabagista. O diagnóstico final das pacientes incluiu: IUM + POP (35,4%), IUE + POP (19,8%), IUE (13,5%), IUM (10,4%), SBH + POP (7,3%), POP (7,3%), SBH (3,1%), outros (3,1%). As disfunções do assoalho pélvico são prevalentes em mulheres na pós-menopausa.
