I Introduction. In generalizing the experience of providing medical care to the wounded in armed conflicts, a special place belongs to the analysis of the magnitude and structure of casualties, which depend on the conditions, forms and methods of hostilities. The purpose. Conduct a clinical and epidemiological analysis of the proportion of gunshot and mine injuries in the structure of combat surgical trauma during the Anti-terrorist operation / Joint Forces operation. Materials and methods. The total number of wounded as a result of the armed conflict in eastern Ukraine in the period from 2014 to 2019 was more than 41 thousand people, from them killed among the civilian population – 3350 people and 4100 – servicemen. Results. It was proved that the wounded with non-severe combat surgical trauma are 36.5%, severe – 48.9%, extremely severe – 14.6%. Shrapnel wounds were received by 35.3%, bullet wounds – 48.3%, mine injuries – 16.6%. Isolated combat surgical trauma was found in 16.8%, multiple – in 34.3%, combined – in 48.9% of the wounded. Impenetrable combat surgical trauma was diagnosed in 63.7% of the wounded, penetrating into the pleural cavity – in 17.2%, in the abdominal cavity – in 16.0%, in the pelvic cavity – in 3.1%. Conclusions. In the structure of sanitary losses of the surgical profile during the ATO / OOS, the wounded with injuries of the extremities are 56.7%, with injuries of the chest – 10.1%, abdomen – 5.1%, pelvis – 3.0%.
Реферат. У структурі сучасної бойової хірургічної травми поранення тільки м’яких тканин становить 68%, тому важливим є визначення хірургічної тактики лікування на етапах медичної евакуації для швидкого повернення поранених у стрій. Матеріали і методи. Проведений аналіз наявних класифікацій ранових дефектів і методів вимірювання розмірів ран. Результати. На сьогодні не існує єдиного методу обчислення площі та об’єму ранового дефекту та уніфікованого алгоритму пластичного закриття вогнепальних дефектів м’яких тканин. Визначена методика обчислення площі та об’єму ранового дефекту, на основі якої запропонована класифікація дефектів м'яких тканин з урахуванням їх лінійних характеристик та локалізації. Обговорення. Запропонований комплексний клінічно–організаційний підхід щодо пластичного закриття дефектів м’яких тканин із застосуванням адаптованої для етапів медичної евакуації концепції «реконструктивної драбини». Висновок. Впровадження уніфікованого алгоритму пластичного закриття вогнепальних дефектів м’яких тканин дозволить скоротити терміни лікування, зменшити кількість ускладнень і покращити функціональні результати лікування поранених і травмованих.
Resume. The experience of providing medical care during the anti-terrorist operation in eastern Ukraine has shown that in the structure of modern combat surgical trauma gunshot wounds with soft tissue defects are 64.9-68.2%, of which 36.4-37.5% small and medium, 28.5-30.7% are large and very large defects. Goal: To improve the results of surgical care for soft tissue defects by introducing differentiated surgical tactics for wound closure to each level. Material and methods. The total amount of the study was 136 wounded with shrapnel, bullet and explosive injuries from April 2014 to September 2018. Determination of surgical tactics for closing soft tissue defects was performed on the basis of metric classification, taking into account the area, volume and anatomical areas of the lesion. Results. The combination of metric characteristics of wound defects by area, volume and location of wounds in a single classification allowed to offer a comprehensive approach to sorting the wounded by the level of medical care and determine further reconstructive surgical tactics to close soft tissue defects. Conclusions. The introduction of differentiated surgical tactics in wounded with gunshot defects of the soft tissues at the all levels of medical care improved functional results: the proportion of satisfactory increased from 46.9% to 53.7%, the relative number of unsatisfactory decreased from 18.8% to 11.6%
Мета дослідження: покращити результати надання травматологічної допомоги пораненим із дефектами м’яких тканин на основі впровадження диференційованої хірургічної тактики закриття ран на етапах медичної евакуації (ЕМЕ). Матеріали та методи. Загальний масив дослідження становили 2051 поранений з осколковими, кульовими та мінновибуховими ушкодженнями кінцівок, що трапилися з квітня 2014 р. по вереснь 2016 р. Визначення хірургічної тактики закриття дефектів м’яких тканин здійснювали на основі метричної класифікації з урахуванням площі, обсягу й анатомічних зон ушкодження. Результати. Поєднання метричних характеристик ранових дефектів за площею, обсягом із локалізацією поранень в єдину класифікацію дозволило запропонувати комплексний підхід до сортування поранених на ЕМЕ і визначити подальшу хірургічну тактику закриття дефектів м’яких тканин. Відповідно до сортувальноевакуаційного призначення поранених із вогнепальними ушкодженнями стопи та кисті (третя зона ушкодження) лікували в спеціалізованих центрах ІІІ та IV рівнів. При середніх та великих ранах стегна, гомілки, плеча та передпліччя медичну допомогу надавали на ЕМЕ ІІ та ІІІ рівнів, при великих та надвеликих ранах означеної локалізації — в спеціалізованих клініках IV рівня. Висновки. Впровдження диференційованої хірургічної тактики в поранених із дефектами м’яких тканин дозволило покращити функціональні результати: збільшити питому вагу добрих — з 46,9 до 53,7 %, зменшити відносну кількість незадовільних — з 18,8 до 11,6 %.
