Cerebrocardial syndrome is a complex of cardiac disorders that occur against the background of damage to the central nervous system and most often develop as a result of cerebral stroke. The article presents a description of a clinical case of cerebrocardial syndrome in a patient with ischemic stroke, the results of diagnostic measures, the dynamics of the electrocardiogram. The clinical significance of cerebrocardial syndrome is that it can cause diagnostic errors and overdiagnosis of cardiac pathology in situations where it does not exist, and, accordingly, lead to improper treatment. An individual approach to the assessment of cardiac disorders in patients with cerebral stroke and regular changes in the neuroendocrine, immune systems, hemostasis system will provide for a timely diagnosis of cerebrocardial syndrome, optimization of treatment and improved prognosis.
Відповідно до сучасного перегляду діагностичних критеріїв інфаркту міокарда (ІМ) його верифікація ґрунтується на виявленні підвищення рівня серцевих тропонінів (Тп) Т і I у крові за наявності клінічних й електрокардіографічних симптомів ішемії міокарда [4]. Це спричинило визнання ролі Тп Т і I як переконливих біохімічних маркерів ІМ і зменшення значення МВфракції креатинфосфокінази (КФК). Причиною пріоритетного використання серцевих Тп у діагностичному алгоритмі ІМ є їх висока специфічність навіть у випадках незначного некрозу міокарда [52]. Відповідно до чинних рекомендацій діагноз гострого ІМ може бути встановлений на підставі підвищення Тп у крові вище 99-го перцентиля нормальних значень
The aim: To evaluate the structural changes of the brain in relation to the formation of cognitive disorders (CD) in patients with arrhythmias Materials and methods: 147 patients with different clinical forms arrhythmias against the background of ischemic heart disease were examine. At the first stage, all patients with arrhythmias assessed cognitive functions. At the second stage, patients were distributed divided into two groups: the main group patients with CD, control – patients without CD. These groups underwent computed tomography examination of the brain. Results: CD were established in 83% patients with arrhythmias. Mild CD were more often diagnosed in patients with persistent form of atrial fibrillation (AF), severe CD – in patients with permanent form of AF and atrioventricular blockade ІI-III degrees. Neuroimaging changes were found in 73.8% patients with CD and in 36% patients without CD. They were manifested by atrophic changes of the cortex, internal hydrocephalus, a decrease in the density of the brain sub¬stance of the periventricular area. In patients with CD, compared to patients without CD, showed lacunar foci with predominant localization in the parietal and frontal lobes of the brain, periventricular and subcortical leukoaraiosis. Multiple correlations were established between CD and structural changes of the brain. Conclusions: The increase in the severity of CD in patients with arrhythmias is associated with atrophic changes at the cortical-subcortical level, accompanied by the phenomena of internal hydrocephalus, periventricular and subcortical LA, lacunar foci, with a predominant localization in the frontal-temporal-occipital lobes, in the visual hump and basal ganglia of both cerebral hemispheres.
Вивчення впливу порушень серцевого ритму та провідності на когнітивну функцію має осо-бливе значення у кардіоневрології, оскільки саме ці розлади відіграють суттєву роль у розвитку ког-нітивного дефіциту. Особлива увага в літературі приділяється ролі фібриляції передсердь (ФП), яку діагностують у 1 % дорослого населення [18]. Більше 6 мільйонів європейців страждають від цієї аритмії. Поширеність ФП, за попередніми оцінка-ми, зросте у 2 рази впродовж наступних 50 років. За своєю медико-соціальною значимістю ФП сут-тєво випереджає інші види кардіальних аритмій, що пов'язано не лише з її поширеністю, але й з тяжкістю обумовлених нею органних ускладнень, асоційованих зі значними моральними та матері-альними витратами. Розвиток хронічної серцевої недостатності, збільшення ризику раптової серце-вої смерті, емболічні церебральні ускладнення, по-гіршення перебігу постінсультного періоду, стійка інвалідизація, низька виживаність -усе перерахо-ване є наслідком втрати синусового ритму [6]. ФП дає п'ятиразове збільшення ризику інсульту, і один із кожних п'яти інсультів пов'язаний із цією арит-мією. Ішемічні інсульти при ФП часто закінчують-ся смертельним наслідком, а пацієнти, які вижи-ли, мають вищий відсоток інвалідизації та частіше страждають від рецидивів, ніж пацієнти з іншими причинами інсульту. Разом із тим сама специфіка хвороби (клінічні прояви, наслідки, характер лі-кування) визначає її істотний вплив на актуальну життєву ситуацію, інтелект та систему особистіс-них стосунків хворого [1]. ФП вважається чинни-ком, пов'язаним із когнітивними розладами (КР), що розвиваються у результаті перенесеного інсуль-ту, так і не пов'язаним із гострими порушеннями мозкового кровообігу [15,24,28].Питання генетичної залежності розвитку ког-нітивних розладів активно розробляються остан-німи роками і найбільш вивчені для хвороби © «Медицина невідкладних станів», 2017
