This prospective study demonstrated that placing Brånemark implants into fresh extraction sites can be successful over a period of 5 years of loading. One of the outcomes of the study shows that there is a clinical correlation between implant failure and periodontitis as a reason for tooth extraction, even if it is difficult to give it a casual association. It can be hypothesized that periodontitis affected tissues might have a negative local influence because of the presence of infrabony defects that could possibly increase the gap between bone and implant or jeopardize achievement of primary stability.
The present study demonstrated that single-stage surgery and immediate loading of implants with prefabricated bridgework in the mandible can result in high implant success and significant reduction in treatment time, with patient satisfaction.
L’intérêt pour le changement psychique et la particularité du travail en addictologie ont fait naître l’envie chez les auteurs de mener un travail de recherche. 13 patients souffrant d’un trouble de l’usage de l’alcool suivis en psychothérapie ont été examinés au début du traitement (t1) et à stade avancé (t2) un an plus tard. L’examen comprenait un entretien semi-directif et un Rorschach. 11 patients sur 13 ont progressé cliniquement. L’hypothèse était que l’amélioration clinique s’accompagnerait d’un remaniement psychique visible dans le Rorschach. Ce remaniement est étudié au niveau des indices d’angoisse et de représentation psychique. L’impact éventuel de la désirabilité sociale est également examiné. Une étude nomothétique des résultats montre surtout une diminution de l’angoisse. Mais dans une étude idiographique illustrée par trois cas, on observe une modification des représentations. Cette évolution se fait dans un sens plus expressif ou dans un sens plus adaptatif. Au-delà de cette étude, l’expérience de chercheur clinicien a été profitable à la pratique des auteurs et ils poursuivront leurs recherches sur l’exploration des représentations de la dépendance à l’alcool des patients suivis en CSAPA.
