ResumoIntrodução: A descentralização das ações de saúde pública, operacionalizada a partir da criação do Sistema Único de Saúde, alterou sobremaneira os sistemas de saúde municipais no Brasil. O objetivo deste estudo foi avaliar de forma qualiquantitativa os sistemas de Vigilância em Saúde (VS) de três municípios mineiros de pequeno e médio porte e associar com os serviços de saúde prestados. Métodos: Um instrumento de coleta de dados estruturado e entrevistas semiestruturadas foram utilizados para avaliar a estrutura e os processos realizados na VS dos municípios, respectivamente. Resultados: Vigilância Epidemiológica, Sanitária e Vigilância da Situação de Saúde apresentaram a melhor estrutura. Vigilância ambiental, Vigilância à Saúde do Trabalhador e Promoção da Saúde possuíam estruturas precárias nos três municípios e, na maioria das vezes, compartilhada com outros setores. Os entrevistados atribuíram a boa execução dos processos de vigilância nos municípios muito mais à proatividade dos profissionais e articulação com outros setores do que propriamente pela estrutura física. Conclusão: A VS nos municípios mineiros ainda é incipiente, porém projetos pioneiros como o ProFVS-MG podem alterar sobremaneira a realidade local. AbstractIntroduction: The decentralization of public health measures, operationalized through the creation of the Sistema Único de Saúde (Unified Health System), greatly altered municipal health systems in Brazil. The aim of this study was to qualitatively and quantitatively evaluate small and medium-size health monitoring systems (HS) in three municipalities of Minas Gerais and associate this with the health services provided. Methods: A data collection instrument of structured and semi-structured interviews was used to assess the structure and processes established in HS municipalities, respectively. Results: Epidemiological Monitoring, Sanitation Monitoring and Health Situation Monitoring had the best structure. Environmental Health Monitoring, Occupational Health Monitoring and Health Promotion had poor structures in the three municipalities, and was frequently shared with other sectors. Respondents principally attributed good performance of monitoring procedures to the proactivity of professionals and collaboration with other sectors, rather than to the physical structure itself. Conclusions: HS in the municipalities of Minas Gerais is still incipient, but pioneering projects such as the SHSP-MG can greatly change the local reality.
Objetivo: Avaliar as dificuldades de médicos da Estratégia Saúde da Família (ESF) para proverem assistência à saúde das mulheres em todos os estágios de suas vidas. Métodos: Foi aplicado questionário semiestruturado a 203 médicos que atuam na ESF, em três regiões do Estado de Minas Gerais, com questões específicas relativas à atenção à saúde da mulher: prevenção do câncer do colo e das mamas, cuidado pré-natal e no puerpério, cuidados à saúde reprodutiva e atenção à saúde no climatério. Resultados: O motivo mais comum para serem médicos da ESF foi o interesse em trabalhar na Atenção Primária à Saúde (38,4%). As dificuldades mais frequentes foram atender mulheres no climatério (61,9%) e prover atenção à saúde reprodutiva (41,9%). Comparando os médicos segundo o tempo de formação, verificou-se que os mais jovens (com até três anos de formação) apresentaram mais dificuldades em prover todos os tipos de cuidados à saúde das mulheres. A análise multivariada confirmou as limitações dos médicos mais jovens no atendimento ao climatério (p<0,009) e a sua baixa participação em atividades educativas voltadas para as mulheres (p<0,021). Conclusões: Os médicos mais jovens apresentaram mais dificuldades específicas em todos os tipos de cuidados à saúde das mulheres e menor participação em atividades educativas. Presume-se que as dificuldades desses médicos estejam ligadas a deficiências na sua formação durante a graduação e à escassa participação nas ações programáticas de atenção à saúde das mulheres.
The Fuenzalida-Palacios rabies vaccine has been used in South America for rabies post-exposure prophylaxis. To determine the frequency of adverse reactions associated with this vaccine compared to a control group a quasi-experimental study was carried out in Uberlândia, Brazil, from May 1997 to April 1998. Victims of bites or other injuries by dogs or other mammals and who received or not post-exposure prophylaxis with Fuenzalida-Palacios rabies vaccine were compared as to the occurrence of a list of signs and symptoms. Out of 2,440 victims of bites and other injuries from dogs and other mammals 2,114 participated in the study; 1,004 of them provided follow-up information within 10 to 15 days. Headache and pain at the injection sites were the most commonly found symptoms (125/1,000). No neuroparalytic event was detected. Patients who were given Fuenzalida-Palacios rabies vaccine and those who were not had similar incidences of symptoms (risk ratios close to 1). Regarding the occurrence of adverse reactions, Fuenzalida-Palacios rabies vaccine is a valid alternative for rabies post-exposure prophylaxis.
Os acidentes com motocicletas representam um grave problema de saúde pública em todo o mundo, principalmente devido ao uso das motocicletas como instrumento de trabalho em serviços de entregas rápidas. Este estudo entrevistou 209 sujeitos aprovados na obtenção da carteira de habilitação para dirigir motocicletas em Uberlândia, e ele é homem; entre 20 e 40 anos; solteiro e branco; com ensino médio completo e com mãe com ensino fundamental incompleto; católico; pertenceu e pertence à classe de trabalhadores E; está na atual profissão a menos de três anos; renda pessoal e familiar entre um e quatro e entre quatro e oito salários mínimos respectivamente, referente a 2005; sem filhos; moram em vários bairros de classe média; têm carteira de habilitação para carros e motos; utilizam motos de 100 a 125 cilindradas e com mais de cinco anos de fabricação, principalmente como meio de transporte; frequentemente usam capacete e às vezes usam jaqueta, luvas e capa de chuva; utilizam outro meio de transporte, sendo o carro o mais comum; têm entre seis e 18 horas de lazer por semana; não têm arma de fogo; não consomem álcool, nem outras drogas.
Este relato - elaborado com base nas experiências e vivências oriundas de um projeto de extensão conduzido no ano de 2009 em duas Unidades Básicas de Saúde da Família, na cidade de Uberlândia - discorre a respeito das ações realizadas no sentido de difundir debates e reflexões em torno da questão da saúde do homem no tocante a três doenças: hipertensão arterial, diabetes mellitus e câncer de próstata. Tendo como base a Política Nacional de Atenção Integral à Saúde do Homem e os referenciais da atenção primária em saúde, buscamos conduzir um ciclo de oficinas direcionadas ao público masculino acima de 40 anos, adscrito às unidades, a fim de elucidar aspectos importantes relacionados a sintomas, diagnóstico, tratamento e prevenção. Como produtos deste trabalho, obtivemos resultados quali-quantitativos de dois questionários, cartazes com percepções elencadas pelo público, elaboração e distribuição de uma cartilha educativa e a constatação de que, não obstante o conhecimento desses agravos, persiste uma relutância e descuido por parte da população em efetivamente aplicá-los.
