A estratégia da Saúde da Família é a principal proposta de reorganização do modelo de atenção à saúde, sendo apoiada político, institucional e economicamente pelo Estado, como alternativa de consolidação dos princípios do Sistema Único de Saúde. A expansão e implementação dessa proposta, em grandes centros urbanos, tem encontrado dificuldades, tendo em vista a complexidade dos contextos sociais, políticos e econômicos que envolvem as famílias residentes nesses territórios, sendo objeto de discussão de diferentes especialistas no campo da Saúde Coletiva. Este artigo faz uma análise dos usos e sentidos atribuídos aos termos integralidade, vínculo e acolhimento, no que concerne à gestão do cuidado em saúde, identificando essas estratégias como elementos das práticas de saúde em programas de atenção integral e da construção do direito à saúde como direito de cidadania.

Pautadas pelo princípio de participação da comunidade no Sistema Único de Saúde (SUS), as Conferências Nacionais de Saúde constituem espaços públicos de deliberação coletiva sobre as diretrizes que devem guiar a estruturação e condução do SUS. Tendo em vista que o referido princípio é significado e construído na prática cotidiana e institucional dos atores implicados, saber como a participação tem-se afirmado nessa instância e quais sentidos têm emergido desse processo social é fundamental para que se compreenda tanto sua presença já concreta, como sua capacidade de intervir no SUS. O objetivo deste artigo é analisar, através de relatórios recentes, as Conferências Nacionais de Saúde e a trajetória dos argumentos relativos ao princípio constitucional de participação da comunidade no sistema. Optamos pela análise retórica dos relatórios da VIII, IX, X e XI Conferências, procurando identificar as questões que se colocaram acerca da participação social, desde a instituição do SUS, suas grandes continuidades e os principais deslocamentos argumentativos existentes.
This article examines integrality as one of the doctrinal principles of the Brazilian State Health Policy - the Unified Health System (SUS) - whose aim is to offer health care as a right and as a service. Integrality is the foundation around which managerial activity practices are organized and whose main challenge is guaranteeing access to the health care system's most complex assistance levels. We developed an analytical reference grounded on three dimensions: service organization, knowledge, the practices of health workers and government policy formulation with input from the population. Managerial practices are fertile ground for integrality and are the political arena in which public managers of different government levels, private service providers, health care workers and organized civil society participate. Integrality in health care can only occur through the democratic interaction of subjects involved in the creation of government responses which are capable of contemplating the differences expressed in the health care needs.
A proposta deste estudo é analisar a experiência de participação de um movimento popular em Conselhos de Saúde e sua relação com o poder executivo. Propõe discutir como se concretiza essa participação, já que a presença quantitativa assegurada com o requisito jurídico da paridade não significa uma correspondência direta com a capacidade de intervenção, principalmente se considerados os representantes dos usuários. Objetivo que se busca explorar, por meio da experiência da Pastoral da Saúde da Arquidiocese de Vitória, em Vitória e Vila Velha (ES). O principal recurso metodológico foi a observação participante. Como técnicas de pesquisa utilizaram-se entrevistas semi-estruturadas com informantes-chave e grupos focais com agentes de pastoral. Entre os resultados destacam-se os usos e efeitos da argumentação técnico-científica no diálogo com as experiências populares e o condicionamento desse espaço institucional à decisão dos setores administrativos. Por fim, embora tenha sido percebida a necessidade de discutir criticamente a representação institucional, os resultados da pesquisa identificam também a potencialidade desses espaços e revelam a premência em construí-los como artefato radicalmente democrático.
